Summary: The overall prevalence of hypertension in childhood is 2% to 5%, with primary hypertension being the leading type, especially in adolescence. Excess adiposity, suboptimal lifestyles, and other factors like environmental stress, low birth weight, and genetic factors contribute to primary hypertension in children. Hypertensive children are at high risk of developing hypertension in adulthood and experiencing target organ injuries. Ambulatory and home blood pressure monitoring can aid in diagnosis. Primordial prevention through healthier diet and increased physical activity, along with evidence-based treatment guidelines, is crucial in reducing the prevalence of primary hypertension. Further research and clinical trials are needed to enhance recognition, diagnosis, and treatment outcomes.

Summary: Durable left ventricular assist device (dLVAD) support is an important treatment option for patients with advanced heart failure. Innovations in dLVAD technology have improved survival rates and reduced adverse events. However, there are challenges in promoting wider adoption of this therapy, including delayed patient referral, limited clinician knowledge, and deprioritization of patients waiting for heart transplantation. Novel devices with smaller size and less invasive implantation methods are being developed.

Summary: Sarcopenic obesity (SO), the coexistence of excess adiposity and low muscle mass/function, is common in individuals with obesity and negatively impacts patient outcomes. This article presents an initiative by ESPEN and EASO to establish a consensus on the definition and diagnostic criteria for SO. The proposed diagnostic procedure includes assessment of skeletal muscle function and body composition to confirm the diagnosis of SO. The authors advocate for the implementation of these criteria in routine clinical practice.

Summary: The endocrine care of pediatric and adult patients is affected by disparities due to the basic structures of health systems, research modalities, and policies that impact access to care and social determinants of health. This scientific statement focuses on endocrine disease disparities in the pediatric population and sexual and gender minority populations. Important findings include disparities in medical attention for short stature, underrepresentation of racially and ethnically diverse populations and males in studies of pubertal development and peak bone mass, and higher burden of obesity and diabetes in racial, ethnic, and LGBTQIA populations. Multilevel interventions and changes in health policies are needed to address these disparities.
