Obesity compounds the metabolic response to critical illness and increases the risk for overfeeding complications due to its comorbidities. Hypocaloric, high-protein nutrition therapy affords the hospitalized patient with obesity the opportunity to achieve net protein anabolism with a reduced risk of overfeeding complications. The intent of this review is to provide the theoretical framework for development of a hypocaloric high-protein regimen, scientific evidence to support this mode of therapy, and unique considerations for its use in specialized subpopulations. Macronutrient goals and practical suggestions for patient monitoring are given.

Augmented Renal Clearance Following Traumatic Injury in Critically Ill Patients Requiring Nutrition Therapy

Summary: This study aimed to determine the prevalence of augmented renal clearance (ARC) in trauma patients receiving nutrition therapy. Results showed that 50% of patients exhibited ARC, with factors such as male gender, traumatic brain injury, higher injury severity score, younger age, lower serum creatinine concentration, and protein intake being associated with ARC. Patients with multiple risk factors for ARC should be closely monitored for renal dosing of electrolytes, nutrients, and medications.

Protein requirements for critically ill ventilator-dependent patients with COVID-19

Summary: Propofol, commonly used sedative in the ICU, is formulated in a 10% lipid emulsion providing 1.1 kcals per mL. However, its high caloric content can lead to potential overfeeding in patients. Clinicians should strive to balance caloric and protein intake to avoid overfeeding complications.

Summary: The study aimed to evaluate the effectiveness of cholecalciferol therapy in reducing hypercalcemia in critically ill vitamin D-deficient patients with traumatic injuries. The findings showed that adjusting the target 25-OH vit D concentration to a lower level significantly reduced the incidence of hypercalcemia and improved the safety of the therapy.

It is recommended that pharmacists be familiar with these articles as they are important to their practice.

Gastric feeding intolerance in critically ill patients during sustained pharmacologic neuromuscular blockade

Summary: This study aimed to evaluate the tolerance of gastric feeding in critically ill patients receiving sustained neuromuscular blocker (NMB) pharmacotherapy. The findings showed that most patients tolerated intragastric enteral nutrition during NMB pharmacotherapy, indicating that NMB pharmacotherapy is not an absolute contraindication for gastric feeding.

A list of important articles published in 2022 was compiled by clinical pharmacists. The authors assessed the importance of each article and identified the ones considered important by at least 5 out of 8 participants. A total of 162 articles were identified, with 8 primary literature articles ranked as highly important. Additionally, 10 guideline and consensus papers were included.

Enteral feeding tolerance during pharmacologic neuromuscular blockade

Summary: A common misperception is that critically ill patients who receive neuromuscular blocker pharmacotherapy (NMB) cannot tolerate enteral nutrition. This review examines the evidence regarding enteral feeding tolerance for critically ill patients given NMB. Limited evidence suggests that enteral feeding intolerance during NMB is more likely due to underlying illnesses and concurrent opioid analgesia, sedation, and vasopressor therapies. Most critically ill patients can be successfully fed during NMB, and prokinetic pharmacotherapy may be needed in some cases.
