Oral glutamine reduces myocardial damage after coronary revascularization under cardiopulmonary bypass. A randomized clinical trial

Background: Glutamine is the most abundant free amino acid in the body. It modulates immune cell function and is an important energy substrate for cells in critically ill patients. Reduction of injury cardiac markers had been observed in patients receiving intravenous glutamine and in a pilot study with oral glutamine. The aim of this study was to analyze the effect of preoperative oral supplementation of glutamine on postoperative serum levels of cardiac injury markers. Methods: A randomized clinical trial was performed in 28 Mexican patients with ischemic heart disease who underwent cardiopulmonary bypass with extracorporeal circulation. Patients were randomly assigned to receive oral glutamine (0.5 g/kg/day) or maltodextrin 3 days before surgery. Cardiac injury markers as troponin-I, creatine phosphokinase, and creatine phosphokinase-Mb were measured at 1, 12, and 24 hours postoperatively. Results: At 12 and 24 hours serum markers levels were significantly lower in the glutamine group compared with controls (p = 0.01 and p = 0.001, respectively) (p = 0.004 and p < 0.001, respectively). Overall, complications were significantly lower in the glutamine group (p = 0.01, RR = 0.54, 95% CI 0.31-0.93). Mortality was observed with 2 cases of multiple organ failure in control group and 1 case of pulmonary embolism in glutamine group (p = 0.50). Conclusion: Preoperative oral glutamine standardized at a dose of 0.5 g/kg/day in our study group showed a significant reduction in postoperative myocardial damage. Lower cardiac injury markers levels, morbidity and mortality were observed in patients receiving glutamine.

Comparison of Fear of COVID-19 in Medical and Nonmedical Personnel in a Public Hospital in Mexico: a Brief Report

Summary: The COVID-19 pandemic and social distancing measures have led to stress, psychological instability, stigmatization, fear, and discrimination worldwide. A survey conducted on hospital staff found significant differences between men and women in their fear levels. Medical students showed higher levels of fear compared to experienced medical personnel, and medical and nursing staff had higher levels of fear compared to non-medical staff who did not work directly with COVID-19 patients. These findings suggest that greater knowledge of medicine or infectious diseases could help mitigate the psychological impact of the pandemic.

Summary: This study evaluated the effects of PFD on anastomosis healing and adhesion genesis in a low-risk rat model of dehiscence of colonic anastomosis. The results showed that the bursting pressure was higher, and the macroscopic adhesions were soft and had less inflammatory infiltration and higher collagen content in the PFD group compared to the control group.

Summary: This observational cross-sectional study examined changes in substance use during the COVID-19 pandemic in Mexico, and whether depression or anxiety was associated with these new consumption patterns. An online survey was distributed to the general population, and results showed that substance use patterns were affected by the pandemic, with an increase in the number of users and consumption rate, as well as reported psychiatric symptoms.

The results suggest that the pandemic has negatively affected the well-being of postpartum women, and encouraging breastfeeding and skin-to-skin contact can improve emotional stability and mental health in this population.

Summary: The study aimed to investigate the clinical and biochemical characteristics associated with remission of acute kidney injury (AKI) in Mexican patients. The results showed that the presence of chronic kidney disease, higher serum creatinine levels at admission, lower estimated glomerular filtration rate, higher maximum serum creatinine during hospitalization, higher fractional excretion of sodium and 24-hour urine protein, higher serum potassium levels at admission, abnormal levels of procalcitonin, and increased risk of death were associated with nonremitting AKI. These findings can help identify patients at risk for nonremitting AKI based on clinical and biochemical characteristics, and inform the design of strategies for the surveillance, prevention, and treatment of AKI.
