Summary: This systematic review aims to explore the role of matrix metalloproteinases (MMPs) in routine cardiac operations, especially cardiopulmonary bypass (CPB). The results show that MMP levels are significantly correlated with CPB and the gelatinases MMP-2 and MMP-9 are highly released during cardiac surgery with CPB. Increased MMP levels are associated with adverse post-operative outcomes. Future large-scale prospective multicenter randomized clinical trials are needed to investigate the impact of mechanical and pharmacological strategies on the inflammatory response and postoperative outcomes in cardiac surgery and CPB.

Summary: Patients undergoing coronary revascularization with extracorporeal circulation or cardiopulmonary bypass may develop systemic inflammatory response due to biochemical changes in the microcirculation. Inflammatory signaling pathways can be activated by surgical incision, reperfusion injury, and blood contact with non-endothelial membranes, leading to the production and activation of inflammatory cells, cytokine production, and oxidative stress. Addressing new perspectives for intraoperative treatment and management may reduce this inflammatory storm, improve prognosis, and potentially lower mortality rates in these patients.

Summary: Acute kidney injury after cardiac surgery presents specific patterns of damage and recovery, which are crucial for management and outcomes. The KDIGO classification only partially covers the conceptual framework and is insufficient for a comprehensive diagnosis. This review discusses the strengths and limitations of recent criteria for CSA-AKI and provides an overview of biomarkers. The evolving understanding of CSA-AKI as a time-sensitive condition has led to an increased demand for enhancing diagnostic criteria and implementing biomarkers in clinical practice.

Summary: The pharmacokinetics and plasma concentrations of methylprednisolone have not been comprehensively studied in children with congenital heart disease undergoing cardiac surgery with cardiopulmonary bypass. There may be a significant influence of cardiopulmonary bypass on the plasma concentrations of methylprednisolone, which could explain the limited efficacy of steroid administration in this context.

Summary: Recent advances in perioperative management have significantly reduced mortality rate in open heart surgery and cardiopulmonary bypass for heart disease repair. However, the morbidity rate remains unchanged. The measurement of neuro-biomarkers, particularly S100B protein, can be useful in detecting early stage brain damage during the perioperative period. This review provides an updated overview of the pros and cons of using S100B protein for perioperative monitoring in both adult and pediatric patients.

The activity of indoleamine 2,3-dioxygenase and IL-10 plasma level increase after CPB, while arginine plasma level decreases. Only arginine supplementation partially restores T-cell proliferation.

Summary: The COVID-19 pandemic and social distancing measures have led to stress, psychological instability, stigmatization, fear, and discrimination worldwide. A survey conducted on hospital staff found significant differences between men and women in their fear levels. Medical students showed higher levels of fear compared to experienced medical personnel, and medical and nursing staff had higher levels of fear compared to non-medical staff who did not work directly with COVID-19 patients. These findings suggest that greater knowledge of medicine or infectious diseases could help mitigate the psychological impact of the pandemic.

Summary: This observational cross-sectional study examined changes in substance use during the COVID-19 pandemic in Mexico, and whether depression or anxiety was associated with these new consumption patterns. An online survey was distributed to the general population, and results showed that substance use patterns were affected by the pandemic, with an increase in the number of users and consumption rate, as well as reported psychiatric symptoms.

Summary: The study aimed to investigate the clinical and biochemical characteristics associated with remission of acute kidney injury (AKI) in Mexican patients. The results showed that the presence of chronic kidney disease, higher serum creatinine levels at admission, lower estimated glomerular filtration rate, higher maximum serum creatinine during hospitalization, higher fractional excretion of sodium and 24-hour urine protein, higher serum potassium levels at admission, abnormal levels of procalcitonin, and increased risk of death were associated with nonremitting AKI. These findings can help identify patients at risk for nonremitting AKI based on clinical and biochemical characteristics, and inform the design of strategies for the surveillance, prevention, and treatment of AKI.
