Summary: This study investigated the time trend of axillary surgery in Asian patients with breast cancer and found a downward trend in the proportion of axillary lymph node dissection (ALND) in Korean patients. However, the rate of decrease was slower than in the Dutch cohort.

Summary: The study used the National Cancer Database to analyze the utilization of axillary lymph node dissection (ALND) after the publication of the ACOSOG Z0011 trial, which showed no survival advantage for certain breast cancer patients. The results showed a decrease in ALND rates over time for patients meeting Z0011 criteria, with significantly higher rates of nonadherence in Community Cancer Programs compared to Academic Programs.

Summary: The study found that the standard MRI protocol is effective in excluding high-level and advanced axillary lymph node metastasis in most patients with breast cancer. However, the presence of peritumoral edema or positive axillae in MRI findings emphasizes the benefits of a combined MRI protocol.

Summary: This study compared the real-world outcomes of SLNB alone and SLNB + ALND in early-stage breast cancer patients with limited positive SLN metastasis post-Z0011. It found that for patients with one or two SLN metastases, SLNB alone showed no significant difference in overall survival, disease-free survival, and recurrence rate compared to SLNB + ALND, and had a lower incidence of lymphedema.

Summary: The SENOMIC trial found that omitting axillary lymph node dissection in patients with breast cancer and sentinel node micrometastases had excellent 3-year event-free survival rates. However, patients who underwent mastectomy had worse tumor characteristics and higher risk of recurrence, especially without adjuvant radiotherapy. Long-term follow-up and enrollment of mastectomy patients are crucial.

Summary: Sentinel lymph node biopsy can provide survival results equivalent to axillary lymph node dissection for patients with cN0 and T1-2 breast cancer; however, whether it can be performed on patients with T3-4c breast cancer is still controversial.

Summary: This study investigated the survival outcomes of patients with limited nodal burden in breast cancer. The data suggest that upfront SLNB and RNI for cN+ T1-2 tumors can provide favorable survival outcomes comparable to ALND with or without RNI.

Summary: The SARS-COVID-2 pandemic had a profound impact on healthcare systems, leading to a decrease in breast cancer surgeries and longer waiting lists. We conducted a study on breast cancer surgeries at the University Hospital of Cagliari, Italy, from February 2018 to March 2022. Two phases were identified based on epidemiological circumstances, and the surgeries performed in each phase were compared. The use of the OSNA method and ACOSOG Z0011 criteria in axillary treatment for breast cancer resulted in a significant reduction in reoperations for metastatic sentinel lymph nodes.

Summary: The aim of this study was to determine whether breast cancer patients with mastectomy and false-negative frozen section in sentinel lymph node biopsy (SLNB) could forgo axillary lymph node dissection (ALND). The study found that SLNB and axillary radiation therapy could effectively replace ALND in selected patients who met specific criteria.

Summary: This pilot study conducted in Thailand confirms the low re-operation rate for sentinel lymph node biopsy (SLNB) without intraoperative frozen sections (FS). The results suggest that implementation of SLNB with permanent section (PS) analysis alone in patients who meet the ACOSOG Z0011 criteria does not require additional FS analysis.

Summary: The study evaluated the impact of the ACOSOG Z0011 trial on axillary breast cancer surgery management in Brazil, showing substantial changes in the management of axillary surgery in cN0/pathologically positive SLN cases, especially following publication of the updated Z11 results and similar studies. Factors such as better education environment and long-term follow-up were associated with the incorporation of Z11-related changes in practice.

Summary: The SINODAR-ONE trial aimed to assess whether SLN biopsy only would worsen the prognosis compared with ALND in patients with T1-2 breast cancer and one or two macrometastatic SLNs. The results showed that the 3-year survival and relapse rates of patients treated with SLNB only were not inferior to those treated with ALND.

Summary: This study developed a predictive model based on a multi-center retrospective database to accurately predict whether HR+/HER2- patients can undergo genomic tests. Through multivariate logistic regression analysis, imaging abnormal nodes, the number of positive SLNs, the number of negative SLNs, pathological tumor stage, and lympho-vascular invasion were identified as independent predictors for patients having <= 3 total metastatic ALNs.

Summary: Intravenous administration of monoclonal antibodies results in low concentrations in the central nervous system, posing challenges in neuro-oncology, particularly in cases of leptomeningeal carcinomatosis of HER2-overexpressing breast cancer. This study describes the fate of trastuzumab after intrathecal administration, revealing significant fluctuations in cerebrospinal fluid concentration and rapid transfer to serum. Transfer of trastuzumab from serum to cerebrospinal fluid is limited, and intrathecal administration is associated with rapid transfer to serum, with no correlation to survival without neurological relapse.

Summary: The tumor doubling time in breast cancer has remained stable over the past 80 years, despite recent studies identifying fast-growing tumors and raising concerns about overdiagnosis. Prospective studies with more precise measurement of tumor size and adjustment for tumor characteristics are needed to further understand the prognostic and predictive impact of tumor doubling time in breast cancer.

Summary: The study designed HPV droplet digital PCR (ddPCR) assays for blood detection and monitored blood HPV circulating tumor DNA (HPV ctDNA) levels in patients with LACC undergoing CRT. Results showed successful detection of HPV ctDNA in 69% of patients before CRT, with a correlation between HPV-ctDNA levels and HPV copy number in tumors. Detection of HPV-ctDNA before CRT was associated with tumor stage and lymph node status, while positive detection during follow-up was linked to lower disease-free and overall survival rates.

Summary: The sentinel node is the first lymph node reached by metastatic cells from a primary tumor. Its resection is a minimally invasive technique that can help select patients with occult lymph node metastases for adjuvant treatment. The development of functional lymphatic mapping techniques in the 1990s validated the concept of a sentinel node as an alternative to traditional lymphadenectomy.

Summary: Ovarian transposition is an established method for protecting the ovaries from radiation, while surgical procedures for protecting the uterus are still under investigation. This study conducted a systematic review of uterine displacement techniques and performed dose simulation to assess the radiation dose received by the uterus. The results showed that the transposition approach was the most protective.

Summary: The Italian Research Group for Gastric Cancer developed a prospective database to evaluate the impact of a pragmatic attitude on the management of stage IV gastric cancer patients. The study found that different metastatic sites did not affect survival rates, but multiple metastatic sites were associated with worse survival. Patients who could undergo curative resection had better survival rates. A more accurate diagnostic workup and staging had a favorable impact on survival.

Summary: This study evaluated the outcome of tonsillar cancer managed with neoadjuvant chemotherapy followed by surgery.
