Endothelial Alterations in Systemic Lupus Erythematosus and Rheumatoid Arthritis: Potential Effect of Monocyte Interaction

Patients with systemic autoimmune diseases such as rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE) are prone to develop atherosclerosis and cardiovascular diseases five times more often than the general population; this increase in frequency could be partially explained by an increase in the macrovasculature endothelial damage. In these autoimmune diseases, a microvascular endothelial injury has also been reported in different organs and tissues, especially in sites where ultrafiltration processes occur. Different components that are characteristic to the immunopathology of RA and SLE could be involved in the endothelial cell activation, permeability increase, functional alteration, and vascular injury. Circulating immune complexes (IC) detected in SLE and RA have been proposed to participate in the endothelial injury. In the vascular environment, IC can generate different responses that could be mediated by monocytes, because these cells have patrolling and monitoring functions on the endothelium. However, with certain stimuli such as TLR ligands, the monocytes are retained in the lumen, releasing proinflammatory mediators that participate in the endothelial damage. This paper aims to review some aspects about the endothelial activation and dysfunction in the context of SLE and RA, as well as the potential role that monocytes apparently play in this process.

Atypical phenotype and response of B cells in patients with seropositive rheumatoid arthritis

Summary: Patients with seropositive RA exhibit an increased frequency of atypical B cell populations, decreased expression of CD22 in memory B cells, and increased frequency of CD86(+) B cells. They also show reduced interleukin-10 secretion, plasma cell differentiation, and IgM production compared to healthy donors.

Summary: In patients with active SLE, there is an increase in plasma MPs concentration that correlates positively with the SLEDAI score. CD14+ MPs are significantly more abundant in patients with SLE compared to healthy controls. SLE MPs have decreased levels of miR-146a, but increased levels of miR-126 in aSLE MPs, which correlates positively with the SLEDAI score.

Summary: Vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic syndrome (VEXAS syndrome) is a recently described genetic disorder characterized by autoinflammatory symptoms and myeloid dysplasia. This paper reports the first case of VEXAS syndrome in Colombia and South America, presenting with supraglottic larynx involvement and costochondritis. The unique features and poor response to treatment can aid in the diagnosis of VEXAS syndrome, which can be confused with other diseases such as relapsing polychondritis and polyarteritis nodosa.

Summary: This study investigated the activation and pro-inflammatory profile of monocytes in individuals with latent tuberculosis infection (LTBI) and their association with coronary artery disease (CAD). The results showed distinct monocyte alterations in individuals with LTBI, suggesting an exacerbated inflammatory response and tissue migration. These alterations may contribute to the pathogenesis of cardiovascular disease.

Modulation of B cell activation by extracellular vesicles and potential alteration of this pathway in patients with rheumatoid arthritis

Summary: The study found that m/lEVs have a certain regulatory effect on the activation and response of B cells, especially affecting the expression of CD69(+) and CD86(+) B cells. Similar responses were observed in patients with rheumatoid arthritis. However, despite the direct inhibitory effect of m/lEVs on early B cell responses, an increased frequency of CD69(+) B cells from patients with RA was observed when B cells were co-cultured with autologous monocyte-derived macrophages exposed to m/lEVs or m/lEV-ICs, but not with cells from healthy donors.
