A consensus statement for trauma surgery capacity building in Latin America

Mohini Dasari, Erica D. Johnson, Jorge H. Montenegro, Dylan P. Griswold, Maria Fernanda Jimenez, Juan Carlos Puyana, Andres M. Rubiano

Summary: Trauma is a significant public health issue in Latin America, but the region lacks an integrated trauma system. Experts reached a consensus that action is needed from academic bodies, scientific societies, and ministries of health to encourage the collection and use of health data in trauma. The recommendations developed by the panel may help to promote capacity building in trauma research and health policy in the region.

Management of severe traumatic brain injury in regions with limited resources

Andres M. Rubiano, Dylan P. Griswold, Manuel Jibaja, Alejandro A. Rabinstein, Daniel Agustin Godoy

Summary: Severe traumatic brain injury (sTBI) is a critical health issue in regions with limited resources, with road traffic injuries (RTIs) and violence being the principal mechanisms. Challenges in care include lack of pre-hospital care, overloaded emergency services, insufficient trained personnel, and deficiencies in surgical and intensive care. Collaborations between high-income countries (HICs) and low- and middle-income countries (LMICs) have focused on capacity building for sTBI care to address the high burden of disease in RLRs.

International Neurotrauma Training Based on North-South Collaborations: Results of an Inter-institutional Program in the Era of Global Neurosurgery

Andres M. Rubiano, Dylan P. Griswold, P. David Adelson, Raul A. Echeverri, Ahsan A. Khan, Santiago Morales, Diana M. Sanchez, Robson Amorim, Alvaro R. Soto, Wellingson Paiva, Jorge Paranhos, Jose N. Carreno, Ruy Monteiro, Angelos Kolias, Peter J. Hutchinson

Summary: This paper presented the results of a neurotrauma fellowship program aimed at neurosurgeons from LMICs, providing them with the opportunity to receive training within the framework of global neurosurgery collaborations. By establishing international competencies and exposure to different healthcare systems, the program aims to bridge the gap in specialized neurotrauma care in low resource settings.

Severe Pediatric TBI Management in a Middle-Income Country and a High-Income Country: A Comparative Assessment of Two Centers

Jorge Arango, Laeth George, Dylan P. Griswold, Erica D. Johnson, Maria N. Suarez, Laura D. Caquimbo, Milton Molano, Raul A. Echeverri, Andres M. Rubiano, P. David Adelson

Summary: By comparing the management and outcomes of pediatric patients with severe TBI between Phoenix Children's Hospital USA and Neiva University Hospital in Colombia, differences were found in intracranial pressure monitoring frequency, surgical decompression, and subdural evacuation, while similarities were observed in mortality rates and functional outcomes.

Neurotrauma Registry Implementation in Colombia: A Qualitative Assessment

Erica D. Johnson, Sangki Oak, Dylan P. Griswold, Sandra Olaya, Juan C. Puyana, Andres M. Rubiano

Summary: This study conducted a qualitative research in Colombia to investigate the factors affecting the creation and implementation of a multi-institution TBI registry, identifying barriers such as incomplete clinical data, limited resources, and lack of IT support, as well as facilitators like ease of use, local support, personal motivation, and future potential uses of the registry data.

Deep Brain Stimulation for Severe and Intractable Aggressive Behavior

Oscar Andres Escobar Vidarte, Dylan Paul Griswold, Javier Orozco Mera, Gabriel Jose Arango Uribe, Juan Camilo Salcedo

Summary: Deep brain stimulation (DBS) of the posteromedial hypothalamus (PMH) may be a safe and effective treatment option for improving severe and refractory aggressive behavior in patients with long-term intellectual disability. Clinical trials have shown significant reduction in aggressive behavior in most patients during follow-up, with an overall clinical improvement rate of 63%.

Implementing a Neurotrauma Registry in Latin America and the Caribbean

Myron L. Rolle, Deen L. Garba, Dylan P. Griswold, Laura L. Fernandez, Diana M. Sanchez, Angelica Clavijo, Andres M. Rubiano

Summary: This study aims to ascertain the interest of Latin America and the Caribbean (LAC) nations in establishing a shared neurotrauma registry. A descriptive review was conducted on the interests of LAC nations, and it was found that all representatives agreed to adopt the registry for tracking the burden of traumatic brain injury (TBI) and associated pathologies within the region. The study suggests that implementing a neurotrauma registry can benefit the region through a shared database to track disease, improve outcomes, build research, and influence policy.

Hyperosmolar therapies for neurological deterioration in mild and moderate traumatic brain injury: A scoping review

Nicolo Marchesini, Laura Lucia Fernandez Londono, Alessandro Boaro, Isla Kuhn, Dylan Griswold, Francesco Sala, Andres M. Rubiano

Summary: This study aimed to explore the evidence on hyperosmolar therapies (HT) in mild and moderate traumatic brain injury (TBI) and evaluate their effects on outcomes. A scoping review was conducted and only three studies met the inclusion criteria. The findings suggest that HT, particularly hypertonic saline (HTS), may have some benefits in improving outcome measures in TBI patients, with limited adverse effects. Further research in this area is needed.
