Summary: Juvenile idiopathic arthritis (JIA) is a common rheumatic disease in pediatrics, and intra-articular corticosteroid injections (IACIs) are an important treatment strategy to reduce systemic adverse reactions and improve joint symptoms, thus enhancing the quality of life for children with JIA.

Summary: This study aimed to assess predictors of remission in children with juvenile idiopathic arthritis (JIA) treated with intra-articular corticosteroid injection (IACI) as monotherapy or in combination with methotrexate. The results showed that early IACI is a strong predictor of remission.

Summary: Oligoarticular juvenile idiopathic arthritis (oligo JIA) is characterized by Th1 polarization, particularly prominent in CD4+ T cells, while not compromising the regulatory identity of Tregs. Single-cell RNA-Seq revealed heterogeneous effector and regulatory subpopulations in synovial fluid, including IFN-induced Tregs, peripheral helper T cells, and cytotoxic CD4+ T cells.

Summary: Early diagnosis is crucial for reducing long-term disability in JIA patients. This study evaluated salivary gland function in JIA patients, finding differences between oligoarticular and polyarticular subtypes. Impairment of salivary gland function may serve as a risk indicator for JIA1.

Summary: This study evaluated the expression of microRNAs in synovial fluid, plasma, and leukocytes from patients with juvenile idiopathic arthritis (JIA). The results showed dysregulated expression of multiple microRNAs in JIA, both locally and systemically. These findings provide important insights into the pathogenesis of JIA and may have implications for targeted therapies.

Summary: This study compared the efficacy and safety of triamcinolone acetonide (TA) and triamcinolone hexacetonide (TH) for juvenile idiopathic arthritis (JIA) patients. The results showed similar recovery rates for both regimens, but TA had a more than double relapse rate compared to TH.

Summary: The aim of this study was to assess the predictors of relapse in patients with oligoarticular juvenile idiopathic arthritis (oJIA) who achieved clinical remission off medication. A retrospective observational study was conducted on 126 patients who achieved remission off medication. The study found that the number of intraarticular corticosteroid injections (IACIs) and the length of follow-up were significantly related to relapse in oJIA patients who achieved remission off medication.

Summary: Oligoarticular juvenile idiopathic arthritis (oligoJIA) is the most common type of chronic arthritis in children, but there are no evidence-based treatment guidelines, particularly for the use of methotrexate (MTX). This analysis found that patients with persistent oligoJIA treated with MTX are as likely to enter remission as patients with extended oligoJIA or polyarticular JIA, but patients with extended oligoJIA achieved remission significantly later.

Summary: This study is a multicentre, phase 3 trial that aimed to investigate the efficacy of oral fluoropyrimidines S-1 in combination with endocrine therapy for ER-positive, HER2-negative primary breast cancer. The results showed a significant reduction in invasive disease-free survival events in the S-1 plus endocrine therapy group compared to the endocrine therapy alone group at the interim analysis, indicating that S-1 in combination with endocrine therapy could be a potential treatment option.

Summary: Children with early onset juvenile idiopathic arthritis, especially with positive antinuclear antibodies, may develop chronic anterior uveitis, which requires regular slit lamp examination and prompt treatment. Therapy options range from local steroid treatment to systemic immunomodulatory therapy, tailored to individual patient needs, with involvement of expert ophthalmologists and pediatricians. Treatment should not be easily interrupted and may need to be intensified in certain cases for optimal disease control.

These subclinical inflammations may indicate the risk of disease flare and joint deterioration, which could have important implications for the management of the disease.

Summary: The study aimed to develop and validate a weighted score, the ONCOREUM score, for differentiating cancer with arthropathy from juvenile idiopathic arthritis (JIA). Three statistical approaches were used to develop the score, and the highest area under the ROC curve was obtained by model 1, which was selected as the ONCOREUM score. The score is a powerful and easily applicable tool for early differentiation of malignancies with articular complaints from JIA.

Summary: The potential role of Streptococcus salivarius K12 (SSK12) in controlling febrile flares in patients with Periodic Fever, Aphthous stomatitis, Pharyngitis, and cervical Adenitis (PFAPA) syndrome was evaluated. The results showed that SSK12 significantly reduced the number and duration of febrile flares, lowered body temperature, provided a steroid-sparing effect, and reduced accompanying symptoms related to the syndrome.

Summary: This study aimed to understand the incidence of MIS-C in southern Italy. The cumulative incidence rate of MIS-C in children aged 0-18 years was found to be 3.27 per 100,000 residents. The study analyzed clinical features, treatment, and outcomes, and found gastrointestinal, mucocutaneous, and cardiac involvement to be the most common. Treatment was effective, as no intensive care unit admissions were required, and no cardiac sequelae were found in follow-up echocardiograms after 6 months.
