Analysis of the RENAL and mRENAL Scores and the Relative Importance of Their Components in the Prediction of Complications and Local Progression after Percutaneous Renal Cryoablation

Purpose: To determine if modified RENAL (mRENAL) score and its individual components have superior predictive value relative to the RENAL nephrometry score in prediction of complications and recurrence after percirtaneous renal cryoablation. Materials and Methods: Primary masses treated with CT guided percutaneous renal cryoablation between June 2007 and May 2016 were retrospectively'reviewed. AENAL'and mRENAL scores were used to stratify Masses into low, medium, and high complexity tertiles. Complications were characterized by SIR criteria. Predictors of complications and local progression were analyzed using multivariate logistic regression and Kaplan-Meier analysis. Results: There were 95 renal cryoablation procedures in 86 patients. Of ablations, 89 had at least 1 follow-up imaging study, with median follow-up of 29 months. There were 11 (12.4%) complications, including 5 (6.5%) major complications. Mass complexity, as measured by mRENAL complexity tertile, was associated with increased risk of complications on multivariate analysis (P = .045). Endophytic location was the only individual ordinal component of the RENAL and mRENAL scores associated with complications (P = .021). Local progression occurred in 7 (8.3%) masses. Complexity as measured by either scoring system was not associated with local progression. Only diameter > 3 cm was associated with increased risk of local progression (hazard ratio = 9.9, 95% confidence interval = 2.1-45, P = .003). Conclusions: mRENAL score was predictive of complications and tumor size was predictive of recurrence. Use of mRENAL score for complications and tumor size for recurrence should allow for simpler risk stratification and mote accurate patient, counseling.

Summary: Complications of portal hypertension, such as ascites and gastrointestinal bleeding, pose significant risks to patients. The use of transjugular intrahepatic portosystemic shunt (TIPS) creation has emerged as a crucial therapeutic option. In North America, a multidisciplinary approach involving gastroenterologists, hepatologists, and interventional radiologists is employed to perform TIPS creation. However, there are variations in practices and outcomes, highlighting the need for further research. The Advancing Liver Therapeutic Approaches group has critically reviewed the application of TIPS and developed practice-based recommendations to address these issues.

Percutaneous Biliary Neo-anastomosis or Neo-duct Creation Using Radiofrequency Wires

Summary: This study reports a minimally invasive technique using a radiofrequency wire to cross complete biliary occlusions. By creating a biliary-enteric neo-anastomosis, this technique offers a safe, effective, and durable treatment option for patients with biliary occlusion. The procedure has a high success rate with no intra-operative complications, and maintains patency of the neo-anastomosis.

Role of interventional oncology in hepatocellular carcinoma: Future best practice beyond current guidelines

Summary: Hepatocellular carcinoma (HCC) is a major cause of cancer-related deaths worldwide. While liver transplant is the preferred curative treatment, limited organ supply poses challenges in accessibility and waiting time. As a result, interventional oncology therapies have been used to treat the majority of HCC patients, including those awaiting transplant. The Barcelona Clinic Liver Cancer (BCLC) staging system is widely utilized to guide treatment allocation. The most recent update in November 2021, shaped by expert opinions outside the BCLC group, includes additional treatment options for early-stage disease, further categorization of intermediate disease, and alternative treatments to significantly improve survival in advanced-stage disease. The update also introduces a new staging schema for disease progression and supports treatment stage migration (TSM) within the main staging and treatment algorithm. This article reviews the recent developments in the BCLC guidelines and emphasizes ongoing research, particularly in radioembolization, that will inform future best practices.

Summary: This study compares the outcomes of patients with chronic obliterative portal vein thrombosis (PVT) who received portal vein recanalization-transjugular intrahepatic portosystemic shunt (PVR-TIPS) followed by liver transplantation (LT) to those with partial nonocclusive PVT who underwent LT without an intervention. The results show that PVR-TIPS is effective in resolving PVT and simplifies the surgical aspects of LT.
