Variability of cut-off values for the detection of lupus anticoagulants: results of an international multicenter multiplatform study

Background: Cut-off values for interpretation of lupus anticoagulant (LA) detection are poorly investigated. Aims: (i) To assess whether results from healthy donors were normally distributed and (ii) the betweenlaboratories differences in cut-off values for screening, mixing and LA confirmation when calculated as 99th or 95th centiles, and (iii) to assess their impact on the detection rate for LA. Methods: Each of 11 laboratories using one of the three widely used commercial platforms for LA detection was asked to collect plasmas from 120 healthy donors and to perform screening, mixing and LA confirmation with two methods (activated partial thromboplastin time [APTT] and dilute Russell viper venom [dRVV]). A common set of LA-positive or LAnegative freeze-dried plasmas was used to assess the LA detection rate. Results were centralized (Milano) for statistical analysis. Results and conclusions: (i) Clotting times or ratios for healthy subjects were not normally distributed in the majority of cases. The take-home message is that cut-off values should be determined preferably by the non-parametric method based on centiles. (ii) There were relatively large inter-laboratory cut-off variations even within the same platform and the variability was marginally attenuated when results were expressed as ratios (test-to-normal pooled plasma). The take-home message is that cut-off values should be determined locally. (iii) There were differences between cut-off values calculated as 99th or 95th centiles that translate into a different LA detection rate (the lower the centile the greater the detection rate). The take-home message is that cut-off values determined as the 95th centile allow a better LA detection rate.

Monitoring of heparin therapy beyond the anti-Xa activity assay: Evaluation of a thrombin generation assay

Summary: This study compared the effectiveness of global coagulation assays, including thrombin generation assay (TGA), with the anti-Xa assay for monitoring heparin therapy. The results showed that the TGA may not be sensitive enough for low concentrations of low molecular weight heparin (LMWH), but it could be a useful additional tool for monitoring heparin therapy in heparin-resistant patients.

Influence of clinical presentation, site, and extent of venous thrombosis on decision about duration of anticoagulation: Data from the international, prospective, observational WHITE study

Summary: This study investigated the influence of clinical presentation, extent of venous thrombosis, and presence of residual vein obstruction on the decision about the duration of secondary prophylaxis after a first venous thromboembolism. The results showed that the decision varied among countries, with most patients with proximal or associated deep vein thrombosis continuing treatment, while patients with isolated distal thrombosis were more likely to stop treatment. Factors such as presence of concomitant diseases and risk of post-thrombotic syndrome also influenced the decision.

Qualitative risk assessment of homogeneity, stability, and residual concentrations of antimicrobials in medicated feed and drinking water in pig rearing

Summary: This study conducted a qualitative risk assessment to estimate the risks associated with reduced homogeneity and stability, as well as increased residual concentrations of antimicrobial drugs in medicated feed and drinking water on pig farms. Factors such as human error during the preparation of medicated feed and dosing pump type for medicated drinking water were found to pose moderate to high risks. It is crucial to identify and understand these factors in order to ensure the effectiveness and safety of antimicrobial drug treatment.

Summary: This study investigated the impact of risk factors on pregnancy- or combined oral contraceptive (COC)-associated venous thromboembolism (VTE). Results showed that family history was a significant and independent risk factor for COC-VTE and pregnancy-associated VTE. Therefore, family history plays a crucial role in predicting the occurrence of VTE in the context of pregnancy or COC use.

Application of the thrombin generation assay in patients with antiphospholipid syndrome: A systematic review of the literature

Summary: This systematic review examined if the thrombin generation (TG) assay could be useful for the diagnosis and risk assessment of antiphospholipid syndrome (APS). The findings suggest that APS patients have increased aPC resistance compared to healthy and thrombotic controls, but its diagnostic and prognostic value compared to current strategies is unclear. More research is needed to determine the potential added value of other TG parameters in APS diagnosis.

Incidence and clinical outcomes of heparin-induced thrombocytopenia: 11 year experience in a tertiary care university hospital

Summary: Heparin-induced thrombocytopenia (HIT) is a rare immuno-mediated adverse reaction with high thrombotic and mortality risk. A retrospective study was conducted to evaluate the incidence and outcomes of HIT cases diagnosed at a tertiary care hospital from 2007 to 2018. The HIT incidence was 0.16/1000 patient/years in surgical patients and 0.15/1000 patient/years in medical patients.
