Detailed cost of robotic-assisted surgery in the Australian public health sector: from implementation to a multi-specialty caseload

Kate McBride, Daniel Steffens, Christina Stanislaus, Michael Solomon, Teresa Anderson, Ruban Thanigasalam, Scott Leslie, Paul G. Bannon

Summary: The study conducted a retrospective descriptive costing review of all RAS cases at a large public tertiary referral hospital in Sydney, Australia from August 2016 to December 2018. The results showed substantial variation in costs across surgical specialties, with RAS specific costs accounting for 46% of the total costs.

Silent brain infarcts and early cognitive outcomes after transcatheter aortic valve implantation: a systematic review and meta-analysis

Kei Woldendorp, Ben Indja, Paul G. Bannon, Jonathon P. Fanning, Brian T. Plunkett, Stuart M. Grieve

Summary: Silent brain infarcts are common after TAVI, with diabetes, kidney disease, and pre-dilation increasing the overall SBI risk. Higher numbers of new SBIs appear to adversely affect early neurocognitive outcomes.

Pleural effusion post coronary artery bypass surgery: associations and complications

John D. L. Brookes, Michael Williams, Manish Mathew, Tristan Yan, Paul Bannon

Summary: Pleural effusion is a common complication post-cardiac surgery, associated with significant morbidity and resource utilization. Associated complications of pleural effusion post-CABG include post-operative renal impairment and pericardial effusion.

Validated specialty-specific models for multi-disciplinary microsurgery training laboratories: a systematic review

Gareth Crouch, Gerald Wong, Jonathan Hong, Alex Varey, Roger Haddad, Zane Zhanxiang Wang, James Wykes, Nick Koutalistras, Jonathan R. Clark, Michael Solomon, Paul Bannon, Kate E. McBride, Sydney Ch'ng

Summary: Laboratory simulation is increasingly important for teaching microsurgical skills, and training microsurgeons of different specialties within the same simulation laboratory increases efficiency of resource use. A systematic review identified 141 papers describing specialty-specific microsimulation models, with 49 including evidence of validation. The quality of evidence for the available specialty-specific models in the microscope laboratory is poor.

Surgical outcomes for people with serious mental illness are poorer than for other patients: a systematic review and meta-analysis

Kate E. McBride, Michael J. Solomon, Paul G. Bannon, Nicholas Glozier, Daniel Steffens

Summary: The study found that adults with serious mental illness undergoing surgery were more likely to experience post-operative complications and longer hospital stays, but not significantly higher in-hospital or 30-day mortality rates. Targeted pre-operative interventions may help improve surgical outcomes for these vulnerable patients.

Hemostasis, coagulation and thrombin in venoarterial and venovenous extracorporeal membrane oxygenation: the HECTIC study

Bruce Cartwright, Hannah M. Bruce, Geoffrey Kershaw, Nancy Cai, Jad Othman, David Gattas, Jacqueline L. Robson, Sarah Hayes, Hayden Alicajic, Anna Hines, Alice Whyte, Nophanan Chaikittisilpa, Timothy James Southwood, Paul Forrest, Richard J. Totaro, Paul G. Bannon, Scott Dunkley, Vivien M. Chen, Mark Dennis

Summary: This study found significant differences in coagulation profiles and thrombin generation between patients supported with VA and VV ECMO, with VA patients showing a more pronounced consumptive coagulopathy and higher thrombin generation. VA patients required a lower heparin dose to suppress thrombin generation. Despite these differences, there were no significant variations in bleeding or thrombotic event rates between VV and VA patients when adjusted for days on ECMO support. More research is needed to develop tailored anticoagulation strategies based on the mode of ECMO support.

Minimally-invasive versus transcatheter aortic valve implantation: systematic review with meta-analysis of propensity-matched studies

Mathew P. Doyle, Kei Woldendorp, Martin Ng, Michael P. Vallely, Michael K. Wilson, Tristan D. Yan, Paul G. Bannon

Summary: Comparison of early outcomes between MiAVR and TAVI showed that MiAVR had lower rates of postoperative pacemaker requirement and aortic insufficiency/paravalvular leak, but higher rates of acute kidney injury compared to TAVI, with no significant difference in postoperative stroke or short-term mortality.

Summary: LVADs can serve as a bridge to transplantation or an alternative to heart transplantation, prolonging life and improving quality of life but also associated with a significant number of adverse events.

The Impact of COVID-19 on the Provision of Adult Cardiac Surgery at a Dedicated COVID Hospital in Australia

Nicholas McNamara, Benjamin Robinson, Paul Bannon

Summary: The COVID-19 pandemic led to a 21% reduction in cardiac surgery cases at a major cardiac center and dedicated COVID-19 hospital in Australia, with no changes in the urgency or type of surgeries performed during this period.

Five-Year Survival of Transcatheter Aortic Valve Implantation in High-Risk Patients

Andrew B. Haymet, Michael Seco, Christopher Brown, Cristina Cristoloveanu, Scott Murray, Jun Wu, Bruce Cartwright, Mark Adams, Michael P. Vallely, Paul G. Bannon, Michael K. Wilson, Martin K. C. Ng

Summary: This study demonstrated acceptable survival rates in high-risk patients undergoing TAVI, comparable to larger international experiences. There was a trend of worsening hemodynamics that needs to be monitored, and future research should focus on examining patient quality of life and the performance of newer generation prostheses.

Intracardiac Echocardiography for Point-of-Care Guided Left Ventricular Assist Device Implantation: Surgical Implications for COVID-19

Konstantin Yastrebov, Laurencie Brunel, Hugh S. Paterson, Zoe A. Williams, Innes K. Wise, Christopher S. Burrows, Paul G. Bannon

Summary: Data from animal models is being used to assess the human safety and feasibility of using 3D ICE to guide implantation of left ventricular assist devices in critical care settings. The assessment of these new technologies is best done in a surgical institution with expertise in ICE and Impella separately.

Optimizing the discovery and assessment of therapeutic targets in heart failure with preserved ejection fraction

Gabrielle Fusco-Allison, Desmond K. Li, Benjamin Hunter, Dan Jackson, Paul G. Bannon, Sean Lal, John F. O'Sullivan

Summary: There is an urgent need for models that faithfully replicate heart failure with preserved ejection fraction (HFpEF), but current approaches have limitations from cellular to animal models. Recent advances include three-dimensional 'organoids' incorporating mature iPSCs and porcine models that approximate human physiology, but cost and time considerations restrict their widespread use.

Michelle S. Lim, David S. Celermajer, Paul G. Bannon

Summary: This study investigated patients undergoing aortic and/or aortic valve surgery for BAV, classifying them based on the number of raphes. Results showed significant differences in age at surgery and need for surgical intervention among patients with different raphes types, highlighting the importance of individualized clinical monitoring and management.
