Adding Defibrillation Therapy to Cardiac Resynchronization on the Basis of the Myocardial Substrate

BACKGROUND Patients with nonischemic dilated cardiomyopathy (DCM) may be at lower risk for ventricular arrhythmias compared with those with ischemic cardiomyopathy (ICM). In addition, DCM has been identified as a predictor of positive response to cardiac resynchronization therapy (CRT). OBJECTIVES The aim of this study was to investigate the impact of an additional implantable cardioverter-defibrillator over CRT, according to underlying heart disease, in a large study group of primary prevention patients with heart failure. METHODS This was an observational, multicenter, European cohort study of 5,307 consecutive patients with DCM or ICM, no history of sustained ventricular arrhythmias, who underwent CRT implantation with (n = 4,037) or without (n = 1,270) a defibrillator. Propensity-score and cause-of-death analyses were used to compare outcomes. RESULTS After a mean follow-up period of 41.4 +/- 29.0 months, patients with ICM had better survival when receiving CRT with a defibrillator compared with those who received CRT without a defibrillator (hazard ratio for mortality adjusted on propensity score and all mortality predictors: 0.76; 95% confidence interval [CI]: 0.62 to 0.92; p = 0.005), whereas in patients with DCM, no such difference was observed (hazard ratio: 0.92; 95% CI: 0.73 to 1.16; p = 0.49). Compared with recipients of defibrillators, the excess mortality in patients who did not receive defibrillators was related to sudden cardiac death in 8.0% among those with ICM but in only 0.4% of those with DCM. CONCLUSIONS Among patients with heart failure with indications for CRT, those with DCM may not benefit from additional primary prevention implantable cardioverter-defibrillator therapy, as opposed to those with ICM. The results showed that the presence of atrial fibrillation (AF), borderline low systolic function (BLSF), and male gender were independent predictors for RVP induced LVD. Age and BLSF were independent predictors of earlier development of LVD after implant.

Summary: This study examines the real-world performance of implantable loop recorders (ILRs) and finds that ILRs can alter management in a portion of patients. Age and baseline electrocardiogram are important factors influencing the treatment management changes resulting from ILR data. In younger patients, ILR diagnoses rarely lead to treatment changes, while in older patients, ILRs frequently provide new diagnoses and trigger important treatment changes.

Incidence, morbidity, mortality and disparities in dementia: A population linked electronic health records study of 4.3 million individuals

Summary: This study analyzed longitudinal cohort data of 4.3 million individuals in the UK to examine dementia incidence and mortality. The results showed that dementia incidence was higher in individuals living in high deprivation areas. Prior to dementia diagnosis, patients experienced repeated hospitalizations and multiple primary care visits. The findings highlight the importance of addressing the disparities in dementia for prevention and patient care.

Successful percutaneous extraction of malpositioned pacemaker lead in the left ventricle after proper dabigatran treatment

Summary: We present a case of transient ischemic attacks after pacemaker implantation due to the malpositioned pacemaker lead in the left ventricle. The misplaced ventricular lead was identified during regular echocardiography and successfully percutaneously extracted after dabigatran treatment. No neurologic events were observed during a 4-year follow-up.

Ventricular longitudinal function by cardiovascular magnetic resonance predicts cardiovascular morbidity in HFrEF patients

Major gaps in the information provided to patients before implantation of cardioverter defibrillators: a prospective patient European evaluation

Summary: This study aims to evaluate the information transmission to patients prior to ICD implantation and its impact on their quality of life. The findings show that although 71.5% of patients felt optimally informed at the time of implantation, many were not provided with information about ICD-related complications, driving restrictions, and end-of-life ICD deactivation. Women were less involved in the decision-making process and reported to be less often optimally informed before ICD implantation compared to men.

Rhythm monitoring, success definition, recurrence, and anticoagulation after atrial fibrillation ablation: results from an EHRA survey

Summary: This study conducted a survey to show how clinicians currently approach the knowledge gap in dealing with recurrences after a failed ablation for atrial fibrillation (AF). The majority of respondents perform routine monitoring for AF recurrences after ablation and prefer a long-term monitoring strategy. The combination of absence of symptoms and recorded AF is considered as the definition of success by most respondents.

Regional disparity on patient characteristics and perceptions after implantable cardioverter-defibrillator implantation: results from an EHRA patient survey

Summary: The aim of this study was to identify potential regional disparities in characteristics of ICD recipients, patient perceptions and perspectives after implantation and level of information provided to patients. The study found that patients in Western Europe reported higher satisfaction after ICD placement, while patients in Central/Eastern Europe felt more optimally informed at the time of device implantation. Therefore, novel strategies are needed to address regional differences in patients' quality of life and provision of information.

Sex-based differences in cardiac resynchronization therapy upgrade and outcome for patients with pacemaker and new-onset heart failure

Summary: This study evaluated sex-based differences in patients with new-onset heart failure post right ventricular pacing. The results showed that women with pacemaker due to AV block were older but had fewer comorbidities than men. They were less likely to develop new-onset heart failure, but also less likely to receive a CRT upgrade if they did develop heart failure.

The results showed that optimizing device parameters to maximize QRS duration reduction is feasible and associated with improved clinical outcomes.
