A national evaluation of the use and survival impact of adjuvant chemotherapy in Stage II colon cancer from the national cancer database

Summary: In Stage II colon cancer, receiving AC significantly improves 1, 3, and 5-year OS. The study findings suggest that AC is associated with improved OS in low-risk Stage II disease, questioning current guidelines and the need for better risk stratification. Further research with more robust variables is needed to determine best practices for AC.

Human factors in pelvic surgery

Summary: Pelvic surgery presents challenges due to its anatomical complexity and difficulties in visualization and access. Surgeons may suffer work-related musculoskeletal injuries caused by poor design and flow of their work environment, which are exacerbated by the demands of pelvic surgery. Human factors, combining physiology, psychology, and ergonomics, contribute to a better understanding of the challenges posed by pelvic surgery. The choice of operative approach and improved understanding of ergonomics are important factors in mitigating risks to surgeons. Applying human factors approaches such as standardization and checklists can enhance patient safety in the operating theatre.

Summary: Fluorescence-guided surgery has mainly focused on the NIR-I window in the past decade. However, there are limitations in the NIR-I window, such as limited penetration and scattering. Therefore, evaluating the performance of NIR-I dyes in the longer wavelength NIR-II window is crucial for expanding its application. This study used two fluorophores on three pigs and evaluated the mean fluorescence intensity (MFI) using commercially available NIR-I and NIR-II cameras. The results confirmed the feasibility of using NIR-I dyes in the NIR-II domain.

Summary: This needs assessment revealed significant gaps in patient understanding of robotic assisted surgery (RAS), as well as in surgeon communication skills and their ability to understand the patient perspective. Developing patient-centered education and outcome metrics for RAS could help address these gaps.
