Idiopathic myointimal hyperplasia of mesenteric veins: Rare case of ischemic colitis mimicking inflammatory bowel disease

Idiopathic myointimal hyperplasia of mesenteric veins (IMHMV) is a rare and poorly understood ischemic colitis that occurs in the rectosigmoid colon of predominantly young, previously healthy, male patients. A 76-year-old Japanese man presented to our hospital with a 1-year history of worsening diarrhea, lower abdominal pain, and weight loss (-6 kg). Laboratory evaluation revealed white blood cell count of 13200/mu L, C-reactive protein level of 2.0 mg/dL (normal range, 0.0-0.3), and negative results for stool culture (including Clostridium difficile). Colonoscopy showed circumferential and edematous narrowing of the sigmoid colon with deep longitude ulceration. Biopsy was done and examination of the specimen demonstrated no specific ischemia. The patient was treated with bowel rest, antibiotics, and i.v. fluids; however, his symptoms worsened. Finally, sigmoidectomy was carried out. Histological examination demonstrated significant myointimal hyperplasia of mesenteric veins leading to thickening and stenosis of the venous lumen. Therefore, the final diagnosis was IMHMV. Three months following sigmoidectomy, he was asymptomatic.

Summary: This study aimed to clarify the morphological characteristics of colorectal cancer (CRC) in Japanese familial adenomatous polyposis (FAP) patients. Depressed superficial CRC lesions rarely developed but were detected in our study group, and ulcerated non-superficial CRC lesions were also present with similar ratios. Clinicians should pay attention to depressed superficial lesions during endoscopic surveillance of FAP patients.

The results showed changes in patient and oncological characteristics, a transition in treatment strategy, and different independent prognosticators compared to previous reports.

Summary: This study investigated the utility of the JSHBPS nomogram in patients with colorectal liver metastases. The results showed that the nomogram was beneficial for patients undergoing upfront hepatectomy, but less effective for patients undergoing hepatectomy after chemotherapy. Adjuvant chemotherapy should be recommended for high-risk patients.

Selection bias due to delayed comprehensive genomic profiling in Japan

Summary: In Japan, patients with advanced cancer only undergo comprehensive genomic profiling after treatment options have been exhausted. However, length bias may affect the accuracy of survival analysis, and no appropriate methods for adjusting this bias have been developed. This study established a simulation-based model for length bias adjustment and found that patients who underwent profiling tests early had a worse prognosis. Adjusting for length bias allowed for the analysis of prognostic relevance of oncogenic mutations and treatments.

This research identified certain dendritic cell- or macrophage-related molecules in SMs in human lymph nodes (LNs). The study found that CD209, known as a DC-related molecule, was expressed in human SMs, while the expression of CXCL10 mRNA was not observed.

Summary: Juvenile polyposis syndrome (JPS) is an inherited disorder caused by genetic variants in SMAD4 or BMPR1A genes. Some JPS patients also develop hereditary hemorrhagic telangiectasia (HHT). In this study, a novel pathogenic variant in the SMAD4 gene was identified in a Japanese family with JPS and HHT. This variant has not been previously reported and is predicted to cause significant changes in the structure of the SMAD4 protein.

This study reviewed the quality and safety of colonoscopy surveillance in patients with Lynch syndrome nationwide in Japan and found that high-volume experienced endoscopists and appropriate surveillance intervals may minimize the risk of developing colorectal cancers.
