Relationship between maternal hemodynamics and plasma natriuretic peptide concentrations during pregnancy complicated by preeclampsia and fetal growth restriction

OBJECTIVE: A proper maternal cardiovascular adaptation to the pregnancy plays a key role for promoting an adequate uteroplacental perfusion, for ensuring normal fetal development and for preventing gestational hypertensive complications such as preeclampsia. This study aims to evaluate hemodynamic measurements obtained by noninvasive methods among preclamptic women with and without fetal growth restriction (FGR) and the relationship with plasma levels of natriuretic peptides. STUDY DESIGN: The study compared 98 pregnant women (n = 48 with preeclampsia; n = 50 normotensive pregnant women) and 50 nonpregnant normotensive control subjects undergoing anultrasonic cardiac output monitor (USCOM) and plasma assessment of atrial N-terminal pro B-type natriuretic peptide (NT-proBNP). The statistical analysis was carried out by analysis of variance and correlation analysis. RESULTS: Preeclampsia state is associated with increased vascular resistance (mean 1587 +/- 236 vs 978 +/- 153 dyn s cm(-3)) and lower cardiac output (mean 5.7 +/- 1.1 vs 6.78 +/- 0.8 l) and this hemodynamic state is associated with higher levels of NT-proBNP (mean 121.2 +/- 26.3 vs 42.5 +/- 11.4 pg ml(-1)); furthermore, we found an inverse correlation between maternal cardiac output and plasma levels of NT-proBNP only if preeclampsia is associated with FGR. CONCLUSION: The elevated NT-proBNP in preeclampsia may reflect ventricular stress and subclinical cardiac dysfunction worsening if FGR is present. This may have implications for the acute management of the preeclampsia and FGR women and for appropriately timed therapeutic interventions later in life.

Should attention be paid to the cone depth in the fully visible transformation zone? Retrospective analysis of 517 patients with cervical intraepithelial neoplasia grade 3

Summary: This study aimed to evaluate the independent variables associated with positive endocervical margin (EM) in women with high-grade cervical intraepithelial neoplasia (CIN) and fully visible transformation zone (TZ). The study found that TZ type and lesion extension were positively associated with positive EM, while cone depth was inversely related to positive EM. In women with TZ type 2 and lesion extension ≥2 cervical quadrants, an 8-mm cone depth provided a good cutoff value.

Summary: The objective of this study was to establish a prognostic model for endometrial cancer that individualizes a risk and management plan based on patient and disease characteristics. Machine learning algorithms were applied to analyze patient and disease characteristics, and three models were developed to predict cancer-specific survival and disease-free survival. The Endometrial Cancer Individualized Scoring System (ECISS) developed in this study showed high accuracy in assessing patient-specific survival probability.

Gestational Diabetes Mellitus and Small-for-Gestational-Age: An Insight into the Placental Molecular Biomarkers

Summary: Gestational diabetes mellitus (GDM) and small-for-gestational-age (SGA) are metabolic-related diseases that can occur during pregnancy. This study investigated the changes in the placental chorionic villi (CVs) of GDM and SGA patients. The results showed higher levels of SOD-1 and CAT in GDM placentae, which may be related to the hyperglycemic environment. Lipid composition and structure of the placenta were altered in both pathologies, with SGA inducing lipid peroxidation and reducing collagen deposition in the CVs. The cannabinoid system does not seem to be involved in these two pathologies.

Summary: Human papillomavirus (HPV) is a common sexually transmitted infection that can cause various diseases, including cervical cancer. Prophylactic HPV vaccines have been developed to prevent the spread of HPV and reduce the prevalence of HPV-related diseases. Despite their high efficacy, HPV vaccines are underutilized in Italy, with decreasing coverage rates. Action is needed to improve HPV awareness and education to promote widespread adoption of HPV vaccination.

Recent research has shown the important role of microbiota changes in health and disease, particularly during pregnancy. This literature review focuses on the significant changes in the microbiota associated with four common obstetric diseases: hypertensive disorders, gestational diabetes mellitus, preterm birth, and recurrent miscarriage. The review also discusses potential advancements in developing personalized therapies for disease prevention and tailored treatments by modulating the microbiota.

Summary: This study evaluates the impact of healthcare reorganization during the COVID-19 pandemic on Italian colposcopy clinic activities, specifically focusing on cervical excision procedures, follow-ups for conservative management of low-grade lesions, and follow-ups post cervical excision. The results show a reduction in the number of cervical excisions by 8.8% during the pandemic period compared to pre-pandemic. There were also significant reductions in the follow-ups for low-grade lesion management and follow-up appointments after cervical excision.

Extramammary Paget's Disease of the Vulva and Concomitant Premalignant/Malignant Vulvar Lesions: A Potential Challenge in Diagnosis and Treatment

Summary: Extramammary Paget's disease of the vulva (EMPDV) is a rare disease, but it is often associated with other premalignant/malignant vulvar lesions. This study aimed to evaluate the incidence of concomitant vulvar cancers or premalignant lesions in women surgically treated for EMPDV. The medical records of 69 women with EMPDV were analyzed, and 31.9% of them had concomitant vulvar lesions, including VIN and LS. One patient had synchronous vulvar SCC. Women with EMPDV and concomitant premalignant/malignant vulvar lesions had a higher rate of invasive EMPDV and wider lesions. The significance of this association remains unclear, highlighting the importance of specialized healthcare providers.
