Summary: The study found that pulmonary arterial hypertension (PAH) in the elderly is often associated with left heart disease (LHD), but is still treated as PAH. LHD patients typically have higher pulmonary artery wedge pressure and lower pulmonary vascular resistance due to the prevalent presence of diabetes and chronic kidney disease.

Summary: Registry-based studies on Pediatric Pulmonary Arterial Hypertension (PPAH) in China are limited, and the PPAH risk assessment tool needs further evaluation and improvement. This study found that Chinese PPAH patients have severely compromised hemodynamics and significant diagnostic delay, leading to poor long-term survival. The simplified noninvasive risk model demonstrated good performance in predicting survival among Chinese children with PAH.

Summary: This study suggests that a Doppler index, TRPG/AcT, improves the assessment of symptomatic pulmonary embolism (PE) survivors. TRPG/AcT > 0.6 indicates a high probability of chronic thromboembolic pulmonary hypertension (CTEPH) or chronic thromboembolic pulmonary disease (CTED), while TRPG/AcT < 0.6 allows for the safe exclusion of CTEPH + CTED in patients with a low echocardiographic probability of pulmonary hypertension (PH).

Summary: This study analyzed data from 180 BPA procedures in CTEPH patients and found that the procedure significantly improved mean pulmonary arterial pressure, pulmonary vascular resistance, and NT-proBNP levels, with a modest improvement in cardiac index. Although there were some complications associated with BPA, the overall survival rate was high, indicating that BPA is a promising therapeutic option for CTEPH patients in Greece.

Summary: The primary benefit of identifying a valid surrogate marker is the ability to use it in a future trial to test for a treatment effect with shorter follow-up time or less cost. However, previous work has shown potential heterogeneity in the utility of surrogate markers. Therefore, ignoring this heterogeneity may lead to inaccurate conclusions about the treatment effect. This article introduces a novel test that accounts for the heterogeneity in surrogate marker utility.

Summary: This retrospective observational study analyzed data from 12,891 COVID-19 patients and found that age, severe COVID-19, severe acute kidney injury (AKI), and ischemic heart disease were associated with worse mortality outcomes. The severity of AKI was associated with poorer kidney function recovery, while the use of remdesivir was associated with better recovery. In patients without chronic kidney disease, age, male sex, severe AKI, and hypertension were associated with post-AKI kidney function impairment. COVID-19-associated AKI was also linked to higher mortality and worse long-term kidney function recovery.

Summary: The acute hemodynamic effect of vasopressin on the Fontan circulation was investigated in this study. The results showed that vasopressin administration significantly increased systolic blood pressure, systemic vascular resistance, and left atrial pressure, while decreasing pulmonary vascular resistance. However, it had no effect on cardiac index or pulmonary artery pressure. These findings suggest that vasopressin may be an option for treating systemic hypotension during sedation or general anesthesia in Fontan patients.

Summary: This prospective cohort study aimed to investigate the impact of bronchopulmonary dysplasia (BPD) on the health-related quality of life (HRQL) of infants from NICU hospitalization to one year post-discharge. The study found that lower HRQL during NICU stay was associated with earlier gestational age, postnatal corticosteroid usage, outborn status, and gastrostomy tube placement. Lower HRQL at 3 and 12 months post-discharge was associated with readmissions, home oxygen use, parent-reported difficulty breathing, lower developmental scores, and not playing with other children. Most parents reported similar or improved HRQL after discharge, but parents of infants with respiratory symptoms experienced less improvement. Efforts to improve parent HRQL should focus on respiratory symptoms and social isolation.

Summary: This study aims to determine if children with elbow flexion contracture (EFC) caused by brachial plexus birth injury (BPBI) are more likely to develop shoulder contracture and undergo surgical treatment. A retrospective review was conducted on children under 2 years old with BPBI who presented to a single children's hospital. The results showed that patients with EFC had reduced shoulder range of motion and higher odds of shoulder contracture and surgical treatment. Prompt referral to a BPBI specialty clinic is recommended for evaluation and potential surgery.
