Cervical spinal canal stenosis first presenting after spinal cord injury due to minor trauma: An insight into the value of preventive decompression

Purpose: Patients with pre-existing cervical spinal canal stenosis (CSCS) may have minimal or no symptoms. However, performing preventive decompression is controversial as the incidence of CSCS leading to severe cord injury is unknown. Hence, this study aims to revisit the threshold for surgery in silent CSCS by reviewing the neurologic outcomes of patients with undiagnosed CSCS who sustained a cervical spinal cord injury (CSCI). Methods: Two groups of subjects were recruited for analysis. Firstly, patients with trauma-induced CSCI without fracture or dislocation were included. Pre-existing CSCS was diagnosed by MRI measurements. The second group consisted of asymptomatic subjects recruited from the general population who also had MRIs performed. Canal sizes were compared between this control group and the patient group. Within the patient group, neurological assessments and outcomes by Frankel classification were performed in patients treated surgically or conservatively. Results: 32 patients with CSCS were recruited. The mean spinal canal sagittal diameter (disc-level) of all CSCS cases was 5.3 +/- 1.4 mm (1.3-8.2). In comparison, the diameter was 10.5 +/- 1.7 mm (6.6-14.6) in the 47 asymptomatic individuals recruited from the general population. Decompression was performed in 17 patients and conservative treatment in 15. Mean follow-up was 193 +/- 17.0 months (6-84). At the final follow-up, 3 patients (9.3%) returned to their pre-injury Frankel grade, whereas 26 patients (83.3%) lost one or more neurological grade. Three patients (9.3%) died. Conclusions: Majority of patients with silent CSCS who sustained cervical cord injuries did not return to their pre-injury neurological status. All of these subjects have pre-existing canal stenosis hence the risk of cord injury. Given the poor neurological outcome of CSCS, a lower threshold for surgery could be indicated to avoid these disastrous injuries. However, before making any conclusive recommendation we must first identify the prevalence of silent CSCS in the general population and the risk of developing spinal cord injury with more prospective population-based studies. Post-Maturity Progression in Adolescent Idiopathic Scoliosis Curves of 40° to 50°

Summary: The aim of this study was to determine the progression of curves of 40 degrees to 50 degrees following skeletal maturity. The study found that although most of the patients showed progression, the rate of progression was slow, suggesting that yearly observation was a reasonable management approach. Apical wedging and coronal imbalance could potentially serve as indicators for closer monitoring and early spinal fusion in these patients.

Quality of life of postmenopausal women with teriparatide, denosumab and alendronate: One-year prospective study with a propensity score-matched comparison

Summary: This study compared the effects of parathyroid hormone analogues, receptor activators of nuclear factor kappa-B ligand inhibitors, and bisphosphonates on the quality of life of postmenopausal women. The results showed that continuous one-year teriparatide treatment was most effective in improving the quality of life outcomes in postmenopausal osteoporotic women. Baseline visual analogue scale back pain was the only predictive factor for one-year back pain progression.

Can the proximal humeral ossification system (PHOS) effectively guide brace weaning in patients with adolescent idiopathic scoliosis?

Summary: The study aimed to determine whether the proximal humeral epiphyseal ossification system (PHOS) can be used to determine the timing of brace weaning in adolescent idiopathic scoliosis (AIS) by assessing the rate of curve progression after brace weaning. The results showed that patients with PHOS Stage 5 had no curve progression after brace weaning, regardless of whether the curve angle was less than or equal to 40 degrees. For curves greater than or equal to 40 degrees, PHOS Stage 5 and a radius grade of 10 were also effective in indicating the timing of weaning.

Summary: This systematic review compares the impact of AVBT and PSF on ROM in treating AIS. The study found that AVBT outperforms PSF in maintaining ROM and achieving comparable Cobb angle correction. The comparison of QOL outcomes remains inconclusive. It should be noted that AVBT may cause over-correction and distal adding-on. Additionally, AIS patients treated with AVBT show better muscle strength and endurance when compared to those treated with PSF.

Patient and surgical predictors of 3D correction in posterior spinal fusion: a systematic review

Summary: The use of 3D parameters derived from reconstruction of biplanar radiographs can help evaluate the prognostic factors for surgical correction of scoliosis. Preoperative 3D thoracolumbar kyphosis (TK) should be used as a basis for rod contouring and selection of upper and lower instrumented vertebrae (UIV/LIV). For Lenke 1 patients with high-lying rotations, fusion to NV-1 is recommended while > 50% LIV rotation counterclockwise is recommended for Lenke 1C curves to achieve optimal correction.

Proprioception-related gene mutations in relation to the aetiopathogenesis of idiopathic scoliosis: A scoping review

Summary: As idiopathic scoliosis is a multifactorial disorder, one of its possible causes is a proprioceptive defect. Genetic studies have identified several genes, including LBX1, PIEZO2, RUNX3, and NTF3, that are related to proprioception and may contribute to the development and pathology of idiopathic scoliosis. However, the relationship between these genes and the initiation, progression, and treatment outcomes of scoliosis is still unclear and requires further investigation.

Inter-screw index as a novel diagnostic indicator of tether breakage

Summary: A retrospective review of AIS patients who underwent VBT showed that an increase in inter-screw angle of 5 degrees or more is not a reliable indicator for tether breakage, as breakage can occur without a loss of correction. Therefore, the use of the inter-screw index is proposed for diagnosing tether breakage radiographically.

Directed Versus Nondirected Standing Postures in Adolescent Idiopathic Scoliosis: Its Impact on Curve Magnitude, Alignment, and Clinical Decision-Making

Summary: This study investigated the difference in major curve Cobb angle and alignment between directed and nondirected positioning for adolescent idiopathic scoliosis (AIS) and evaluated its implications on treatment decision-making. The results showed that the major curve Cobb angle was smaller in nondirected positioning, and postural variation may lead to overestimation or underestimation of the curve size.

Biomechanical Comparison of Different Surgical Approaches for the Treatment of Adjacent Segment Diseases after Primary Transforaminal Lumbar Interbody Fusion: A Finite Element Analysis

Summary: The study compared the biomechanical effects of different surgical approaches for the treatment of adjacent segment disease (ASD) after transforaminal lumbar interbody fusion (TLIF). The results showed that ALIF and LLIF surgeries had similar effects on adjacent segment range of motion (ROM) and intradiscal pressure (IDP), while TLIF surgery had greater effects on adjacent segment ROM and IDP. The stress in cages was highest in the TLIF revision model, indicating stress concentration due to direct extension of the surgical segment in the same direction.

Deep Learning Model to Classify and Monitor Idiopathic Scoliosis in Adolescents Using a Single Smartphone Photograph

SpineHRformer: A Transformer-Based Deep Learning Model for Automatic Spine Deformity Assessment with Prospective Validation

Summary: SpineHRformer is a reliable learning-based approach that provides consistent and highly accurate measurements of Cobb angle (CA) from posteroanterior (PA) X-rays, surpassing the state-of-the-art method. It identifies anatomical landmarks and calculates CAs using the extracted features from an HRNet and transformer blocks. Experimental results show that SpineHRformer outperforms SpineHRNet+ in landmark detection, CA prediction, and severity grading, offering substantial potential for improving the efficiency and reliability of CA measurements in clinical settings.

Adapting a Clinical Practice Guideline for Management of Patients with Knee and Hip Osteoarthritis by Hong Kong Physiotherapists

Incidence of back pain from initial presentation to 3 years of follow-up in subjects with untreated adolescent idiopathic scoliosis

Summary: This study longitudinally evaluated the incidence of back pain in adolescents with AIS. The results showed that the occurrence of back pain in scoliotic adolescents ranged from 14.7% to 19.0%, with most cases experiencing mild pain. Furthermore, subjects with new onset back pain exhibited poorer function, self-image, and mental health scores.
