Summary: This review examines the evidence on the effects of skin-to-skin contact during the third stage of labor for postpartum hemorrhage prevention. The results show that skin-to-skin contact is effective in reducing postpartum hemorrhage, shortening the third stage of labor, promoting uterine contractility and involution, and reducing the need for synthetic oxytocin and/or ergometrine.

Summary: This study aimed to assess the effect of placental cord drainage on the duration of the third stage of labor and its impact on postpartum hemorrhage and related complications. The results showed that placental cord drainage significantly reduced the duration of the third stage and decreased the occurrence of postpartum complications.

Summary: This study found evidence of a risk allele in the oxytocin receptor gene that may be involved in the development of postpartum hemorrhage among individuals undergoing vaginal birth. A-carriers had a higher risk of bleeding and needing second-line treatment. These findings could be useful in pharmacogenomic strategies for predicting, preventing, or treating postpartum hemorrhage.

Summary: This study aimed to reassess the normal duration of each stage of labor in a contemporary Japanese cohort and determine its impact on the rate of postpartum hemorrhage (PPH) in vaginal deliveries. The results showed that the reference range of each stage of labor was shorter than previously reported. Prolonged second and third stages of labor were associated with an increased risk of PPH, while prolonged first stage did not increase the risk. The prolonged third stage of labor was found to be a more significant contributing factor to PPH than the prolonged first or second stages.

Summary: This study analyzed the clinical characteristics, management, maternal outcomes, and factors associated with severe maternal outcomes (SMO) in postpartum women with postpartum hemorrhage (PPH) who were admitted to an obstetric intensive care unit (ICU) in northeastern Brazil. The results showed that 37.9% of the PPH patients experienced SMO, 34.9% experienced maternal near miss (MNM), and 3.0% experienced maternal death. Multiple logistic regression analysis revealed that having two or more previous deliveries, gestational age less than 34 weeks, and delivering elsewhere were significantly associated with SMO.

Summary: Pregnancies conceived by in vitro fertilization (IVF) are associated with a higher prevalence of complications in the third stage of labor, including postpartum hemorrhage, manual removal of the placenta, and blood transfusion. Therefore, women who give birth vaginally following IVF pregnancy should be managed differently to anticipate and prevent complications in the third stage, even in the absence of other risk factors.

Summary: Postpartum hemorrhage is a major cause of maternal morbidity and mortality in the United States. Opportunities for improvement in postpartum hemorrhage management include delays in recognizing and responding to hemorrhage. Comprehensive and interdisciplinary approaches to care have been found to improve patient outcomes and reduce racial disparities. Implementation of stage-based hemorrhage protocols can help reduce delays and avoid cognitive biases, but evidence regarding the effectiveness of quality metrics for adherence to postpartum hemorrhage bundles is limited.

Summary: Severe postpartum hemorrhage was not associated with maternal age, smoking during pregnancy, or prepregnancy body mass index. However, nulliparous and multiparous women with a previous cesarean birth, gestational age >40 weeks, induction of labor, augmentation of labor, irregular fetal position, instrumental birth, and birthweight >4000 g were identified as risk factors. The combination of induction and augmentation of labor doubled the risk of severe PPH, while larger vaginal tears presented the highest risk. Proper management of these risk factors may help reduce the occurrence of severe PPH.
