Summary: The cortisol level after 1 mg dexamethasone (cortisol(DST)) >= 50 nmol/L is a criterion for mild autonomous cortisol secretion (MACS) with an accuracy of approximately 85%. This study aimed to investigate the associations of cortisol(DST) with age, BMI, and renal function. The results showed that cortisol(DST) was strongly associated with the size of adrenal incidentalomas (AI) and weakly associated with age, BMI, and estimated glomerular filtration rate (eGFR).

Summary: This study conducted a 10.5-year prospective follow-up of NFAI and found that overweight and obese individuals were more likely to progress to mild autonomous cortisol excess (MACE). Higher baseline BMI and baseline cortisol levels after a suppression test may predict long-term progression of hormonal activity. The observed progressions in growth or hormonal activity were clinically insignificant, supporting the view that long-term monitoring of patients with NFAI may not be necessary.

Summary: This study aimed to analyze the reliability of the 1mg-dexamethasone suppression test (DST) as a marker for glucocorticoid excess and cardiometabolic risk in patients with adrenal incidentalomas (AIs). The urinary steroid profile (USP) was determined using gas chromatography coupled to mass spectrometry. The results showed a positive yet modest correlation between post-DST cortisol and total excretion of glucocorticoid metabolites, and the diagnostic accuracy of the DST for predicting ACS-related comorbidities improved when combined with other markers.

Summary: This study aimed to compare the evolution of cardiometabolic parameters in patients with nonfunctioning adrenal incidentalomas (NFAI) and autonomous cortisol secretion (ACS) who underwent adrenalectomy and those who were conservatively managed. The results showed that adrenalectomy had positive effects on glucose and triglyceride levels in ACS patients, and improved systolic blood pressure in NFAI patients.

Summary: The follow-up strategy for unresected non-functional adrenal tumors (NFAT) is controversial due to the unknown natural history of adrenal incidentalomas (AI) and lack of consensus on their management. A recent study evaluated possible radiological variations and their relationship with autonomous cortisol secretion (ACS) in conservatively treated AI patients. A literature review of similar longitudinal studies suggests a significant risk of morphological and functional changes after 5-10 years of follow-up, making it risky to interrupt follow-up in NFAT patients, particularly those with larger adenoma diameter and higher cortisol levels at diagnosis. These findings should inform the optimal management of these patients.

Summary: This study details the treatment of a man who attempted suicide after experiencing symptoms of anxiety and aggressiveness associated with the use of AAS. Regular use of fluoxetine was effective in preventing anxiety, depression, aggressiveness, and suicide ideation, even in the presence of AAS. Urinary concentration of androgens, AAS metabolites, and fluoxetine were monitored through urine samples analysis at the Brazilian Laboratory of Doping Control.

There was no significant difference in psychiatric symptoms between males and females, and no direct association between symptoms and drug dosage.

Summary: MLWH have alterations in bone volumetric density and microstructure compared to controls, regardless of aBMD, which are associated with lower E-2 and BMI. Lower BMI and fat mass in MLWH are positively associated with total vBMD, cortical bone area, and thickness. E-2 and E-2/T ratios are lower in MLWH than in controls and significantly correlated with several cortical and trabecular bone parameters.
