Randomized, Double-Blind, Placebo-Controlled Trial of N-Acetylcysteine Augmentation for Treatment-Resistant Obsessive-Compulsive Disorder

Objective: To evaluate the efficacy of serotonin reuptake inhibitor (SRI) augmentation with N-acetylcysteine (NAC), a glutamate modulator and antioxidant medication, for treatment-resistant obsessive-compulsive disorder (OCD). Methods: We conducted a randomized, double-blind, placebo-controlled, 16-week trial of NAC (3,000 mg daily) in adults (aged 18-65 years) with treatment-resistant OCD, established according to DSM-IV criteria. Forty subjects were recruited at an OCD-specialized outpatient clinic at a tertiary hospital (May 2012-October 2014). The primary outcome measure was the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) scores. To evaluate the variables group, time, and interaction effects for Y-BOCS scores at all time points, we used nonparametric analysis of variance with repeated measures. Secondary outcomes were the severity scores for anxiety, depression, specific OCD symptom dimensions, and insight. Results: Both groups showed a significant reduction of baseline Y-BOCS scores at week 16: the NAC group had a reduction of 4.3 points (25.6 to 21.3), compared with 3.0 points (24.8 to 21.8) for the placebo group. However, there were no significant differences between groups (P =.92). Adding NAC was superior to placebo in reducing anxiety symptoms (P =.02), but not depression severity or specific OCD symptom dimensions. In general, NAC was well tolerated, despite abdominal pain being more frequently reported in the NAC group (n [%]: NAC = 9 [60.0], placebo = 2 [13.3]; P <.01). Conclusions: Our trial did not demonstrate a significant benefit of NAC in reducing OCD severity in treatmentresistant OCD adults. Secondary analysis suggested that NAC might have some benefit in reducing anxiety symptoms in treatment-resistant OCD patients. (C) Copyright 2017 Physicians Postgraduate Press, Inc.

Summary: The study found that the identification accuracy of the whole brain functional profile in children and adolescents is moderate at 38%, while the cortical volumetric profile has an accuracy of 95%. Among large-scale networks, the default-mode, frontoparietal, and dorsal-attention networks are the most discriminative. These results support the protracted development of specific individual structural and functional connectivity profiles.

Pharmacological and somatic treatment effects on suicide in adults: A systematic review and meta-analysis

Summary: This study found that lithium has a protective effect against suicide in bipolar disorder, as well as in mixed diagnostic samples. Clozapine was also shown to reduce the odds of suicide in psychotic disorders. However, other treatments such as electroconvulsive therapy and antipsychotics did not show significant associations with suicide risk.

Our findings suggest that flexible titration of stimulant medications can improve efficacy and acceptability.

Meta-Analysis: Efficacy and Tolerability of Vesicular Monoamine Transporter Type 2 Inhibitors in the Treatment of Tic Disorders

Summary: This study conducted a systematic review and meta-analysis of randomized controlled trials to evaluate the efficacy, acceptability, and tolerability of Vesicular monoamine transporter type 2 (VMAT2) inhibitors for chronic tic disorders (CTD), including Tourette syndrome (TS). The results showed that VMAT2 inhibitors did not have a significant effect on the short-term treatment of tic disorders and had no clinically meaningful impact on tic symptoms, but there were some adverse effects on tolerability and acceptability.

Mispatterning and interneuron deficit in Tourette Syndrome basal ganglia organoids

Summary: This study investigates the pathophysiology of early development in Tourette Syndrome (TS) using induced pluripotent stem cell (iPSC)-derived basal ganglia (BG) organoids. The findings suggest that TS individuals show impaired development of medial ganglionic eminence and reduced differentiation of cholinergic and GABAergic interneurons in the BG organoids. Transcriptome analyses reveal mispatterning in TS, with altered expression of GLI transcription factors and cilia disruption in the early stages of BG organoid differentiation.

The Relationship Between Acute Dissociative Effects Induced by Ketamine and Treatment Response in Adolescent Patients with Treatment-Resistant Depression

Summary: This study aims to investigate the association between acute dissociative symptoms and antidepressant response to ketamine in adolescents with treatment-resistant depression. The results indicate no significant relationship between dissociation symptoms and depression symptom improvement with ketamine treatment in this population. However, the interpretation of the findings may be limited due to the small sample size. Future research should include larger samples to more accurately measure the magnitude of association between acute dissociative symptoms and later antidepressant response to ketamine.

The genetic epidemiology of obsessive-compulsive disorder: a systematic review and meta-analysis

Summary: This study is the first systematic review and meta-analysis of obsessive-compulsive disorder (OCD) genetic epidemiology, aiming to update the current knowledge in the field. The findings include the high prevalence and familial nature of OCD, especially among the relatives of children and adolescent probands, with a phenotypic heritability of around 50%, and the higher correlations of OCD symptoms between monozygotic twins are mainly attributed to genetic and non-shared environmental factors.
