Lipoprotein-X in cholestatic patients causes xanthomas and promotes foam cell formation in human macrophages

BACKGROUND: Lipoprotein-X (Lp-X) is an abnormal phospholipid-rich lipoprotein found in patients with cholestatic liver disease. Some patients exhibit skin xanthomas and severe hyperlipidemia. OBJECTIVE: We investigated whether Lp-X induces foam cell formation in human-derived macrophages. METHODS: To compare the atherogenic properties of Lp-X and modified LDL, we isolated Lp-X from 2 patients who had drug-induced cholestasis and xanthoma striata in the interphalangeal folds. We prepared oxidized LDL and acetylated LDL from healthy volunteers for the positive control experiments. RESULTS: When human monocyte-derived macrophages were incubated with these lipoproteins, the isolated Lp-X induced more prominent lipid accumulation than oxidized LDL or acetylated LDL. One case underwent liver biopsy, with the bile ducts showing marked damage, fulfilling the criteria for vanishing bile duct syndrome. The other case was clinically diagnosed as drug-induced hypersensitivity syndrome. In both cases, Lp-X levels decreased markedly and the xanthomas disappeared completely after the improvement of cholestasis. CONCLUSION: This study indicates that Lp-X induces foam cell formation in human-derived macrophages. Our findings strongly suggest that persistently elevated Lp-X may cause xanthomas. The results showed that PT-INR on day 1 is the most accurate independent predictor for prognosis, and patients with PT-INR>=1.3 on day 1 have significantly lower transplant-free survival rates.

Summary: The study found that mature Plasmodium falciparum parasites are rarely found in the peripheral blood because they are sequestered and attached to the endothelium. Intensive sequestration is associated with a delay in parasite clearance after artemisinin-based treatment. Sequestration plays an important role in the efficacy of artemisinin treatment for malaria.

Summary: HDL serves as dynamic transporters of various molecular cargo and plays important roles in lipid metabolism and inflammation. This study overcomes technical barriers and sheds light on the pathways involved in HDL-mediated bidirectional transport of sRNAs between immune cells. By utilizing a lipid-penetrating RNA-specific fluorescence dye, the absolute concentration of HDL-transported sRNAs is quantified, and HDL-mediated intercellular RNA transport between immune cells is characterized.

Summary: Despite global COVID-19 vaccination campaigns, the pandemic remains a significant medical and social problem. The Ortho VITROS SARS-CoV-2 spike-specific quantitative IgG assay has been developed to assess neutralizing antibodies against SARS-CoV-2 spike antibodies. This study evaluated the clinical performance of VITROS S-IgG in Japan and found a good correlation with the surrogate virus neutralizing antibody test. The results suggest that VITROS S-IgG can be a useful diagnostic tool for assessing immune responses to vaccination and herd immunity, but careful analysis is necessary to interpret the results.

Summary: This study investigated the levels of oxysterols, cholesterol oxidation derivatives, in treatment-naive patients with type 2 diabetes and their association with type 2 diabetes and atherosclerosis. The results demonstrated that the concentration of 25-hydroxycholesterol was significantly higher in type 2 diabetes patients and showed a significant association with type 2 diabetes.

Summary: Recent studies have shown that using machine learning to predict treatment response in patients with primary biliary cholangitis (PBC) may predict prognosis. This study used machine learning and pretreatment data to predict treatment response in PBC patients and evaluated it using the Paris II criteria. The results showed that machine learning models could improve treatment response prediction and enhance prognosis.

Summary: This study investigated the impact of the COVID-19 pandemic on the time between blood culture collection and administration of antibiotics in the outpatient Department of Emergency Medicine in a single university hospital in Japan. The findings showed a decrease in the implementation rate of antibiotic administration within 3 hours after blood culture collection during the COVID-19 pandemic, with an increase as the number of COVID-19-positive patients decreased. However, AST intervention improved the implementation rate and maintained it during subsequent COVID-19 waves.
