Summary: This study aimed to investigate whether regional nodal irradiation (RNI) improves breast cancer recurrence-free interval (BCRFI) in women with biologically low-risk N1 breast cancer. The population-based study found that RNI was not associated with improved BCRFI for this specific population.

Summary: This study aimed to examine whether the inclusion of the posterolateral supraclavicular node (SCL) region to the standard medial SCL target volume improves SCL control and survival outcomes in high-risk node-positive breast cancer patients. Retrospective analysis of 544 patients treated with post-operative radiation therapy was conducted, and the results showed that irradiating the posterolateral SCL region did not significantly improve SCL control and survival outcomes.

Summary: The addition of regional nodal irradiation (RNI) may not provide long-term survival benefits for breast cancer patients who convert to pathologically node negative after neoadjuvant chemotherapy (NAC) and breast conserving surgery with sentinel lymph node biopsy (SLNB) alone.

Summary: This study investigated the acute toxicities on skin and hematologic function in breast cancer patients who received hypofractionated whole breast irradiation with or without regional nodal irradiation. It was found that the whole body integral dose and the incidence of acute toxicities were higher in patients receiving regional nodal irradiation, but there was no statistically significant difference in the occurrence of grade 2 or more acute toxicities between the two groups.

Summary: The objective of this study was to assess the incidence and dose-volume predictors of radiation esophagitis (RE) in breast cancer patients undergoing hypofractionated regional nodal irradiation. The study found that the incidence of grade 2 and 3 RE was 40.9% and 0.3% respectively, with no grade 4 or 5 RE occurring within 6 months after radiation therapy.

Summary: Elective para-aortic (PA) nodal irradiation using modern treatment techniques for node positive cervical cancer patients showed comparable survival outcomes with pelvic field irradiation, with possibly slightly higher rates of non-severe toxicities. Prospective randomized trials are needed to fully evaluate the benefits of elective PA nodal coverage, as retrospective data may be limited by higher risk features in the study population.

Summary: The study focused on identifying esophageal dose-volume constraints associated with grade 2 esophagitis in breast cancer patients undergoing regional nodal irradiation (RNI). It was found that the rate of grade 2 esophagitis was higher with intensity modulated radiation therapy (IMRT) compared to 3D conformal radiation therapy (3DCRT). Esophageal mean dose, V10, and V20 were identified as key dosimetric parameters associated with grade 2 esophagitis.

Summary: The Danish Breast Cancer Group's study showed that internal mammary node irradiation (IMNI) in node-positive early breast cancer patients led to improved overall survival (OS). Patients treated with IMNI had lower risk of distant recurrence and death from breast cancer, ultimately improving long-term survival.

Summary: The study examined the efficiency of molecular classifiers in stratifying breast cancer patients with clinically positive nodes to pN1 versus > pN1 disease. The results showed that epigenetic signatures based on DNA methylation patterns had high accuracy in predicting > pN1 disease. This study may provide an accurate and cost-effective method of identifying patients who could be spared the morbidity of axillary lymph node dissection.

Summary: In cN1 patients rendered cN0 with neoadjuvant chemotherapy, when >= 3 sentinel lymph nodes (SLNs) are removed, the clipped node is an SLN in the majority of cases. Failure to retrieve the clipped node should not be an indication for axillary lymph node dissection (ALND).

Summary: The study identified predictors of nodal pathological complete response in Asian women with breast cancer, including receptor status, breast pCR, and absence of lymphovascular invasion. It also highlighted the benefit of NAC in downstaging the axilla independently of ethnicity.

Summary: In node-positive breast cancer patients treated with mastectomy, post-mastectomy RT, and taxane-based chemotherapy, IMNI is associated with improved disease-free survival (DFS) and overall survival (OS), despite a low rate of locoregional recurrence.

Summary: This study examined nodal recurrence rates in patients with clinically node-positive cancer treated with sentinel lymph node biopsy after neoadjuvant chemotherapy, showing low rates of axillary failure at a median follow-up of 40 months, supporting potential omission of axillary lymph node dissection in patients achieving nodal pathologic complete response with SLNB alone.

Summary: Hypofractionated adjuvant breast radiotherapy can reduce the incidence of acute skin toxicity, especially in patients receiving regional nodal irradiation, compared to conventional fractionated radiotherapy. Further evaluation of the short- and long-term effects of fractionation in real-life settings is warranted.

Summary: This study compared the accuracy and treatment time of using conventional tattoos versus not using tattoos for setup alignment in patients undergoing regional nodal irradiation for breast cancer. The results showed that using a combination of surface and X-ray imaging without tattoos can achieve excellent alignment accuracy with reduced treatment time, eliminating the need for skin-based tattoos for these patients.
