Diagnostic value of cone beam computed tomography and panoramic radiography in predicting mandibular nerve exposure during third molar surgery

The aim of this study was to evaluate the diagnostic accuracies of cone beam computed tomography (CBCT) and panoramic techniques in predicting inferior alveolar nerve (IAN) exposure. The sample size was deteimined based on a pilot study. This prospective clinical series study included 59 third molar extraction sites with any of seven previously suggested panoramic signs of IAN exposure. The diagnosis of nerve exposure was done on panoramic and CBCT images. Molars were extracted and nerve exposure was evaluated clinically. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of CBCT method, and sensitivity and PPV of panoramic method were estimated). The panoramic and CBCT methods correctly classified 67.7% and 93.3%, respectively, of 60 cases. This difference was statistically significant (chi(2) = 13.333, P = 0.000). The sensitivity, specificity, PPV, and NPV for CBCT were 97.4%, 85.7%, 92.7%, and 94.7%, respectively. The sensitivity and PPV of panoramic radiography were 67.8% and 97.6%, respectively. The signs with the highest sensitivity were interruption of the mandibular canal border and abrupt canal narrowing. None of the Pell and Gregory criteria, molar angulations, or three-dimensional canal apex relationships was significantly associated with clinically confirmed IAN exposure. Panoramic radiography may miss about one-third of exposure cases, but a positive panoramic diagnosis is most likely to be a real exposure and should be taken seriously.

Summary: The study found sex dimorphism in both skeletal and dental development, with chronological age being the best predictor of skeletal growth and maturation. In girls, the Demirjian method was better than CVM for estimating age, while in boys, Demirjian was better for molars but not canines. Cutoff points for reflecting skeletal growth spurt were determined for boys at age 12 and for girls between ages 11 and 12, with specific tooth stages identified.

Summary: The study showed that the combination of compound topical anesthetic and photobiomodulation therapy provided adequate local anesthesia for the placement of miniscrews in the buccal side of the maxillary bone during the procedure and effectively reduced postoperative pain levels.

Diagnostic Accuracy of Periapical Radiography in Detection and Measurement of the External Root Resorption in Primary Molars: A Single-Blind Prospective Clinical Study

Summary: This study aimed to evaluate the accuracy of digital radiography in the diagnosis of external root resorption in deciduous teeth. The results showed that digital radiography can provide a good accuracy in diagnosing and measuring external root resorption in deciduous teeth, but there might be an overestimation in the size of resorption in girls and cases with actual lesions smaller than 3mm.

Effects of Rigid and Nonrigid Connections between the Miniscrew and Anchorage Tooth on Dynamics, Efficacy, and Adverse Effects of Maxillary Second Molar Protraction: A Finite Element Analysis

Summary: This study compared the effects of rigid and nonrigid connecting wires on the stress, anchorage loss, and risk of root resorption of miniscrews using finite element analysis. The results showed that using a rigid connecting wire can provide better control of the anchorage and reduce the risk of root resorption, but it increases the risk of miniscrew failure. The stress on the miniscrew is higher with a rigid connection, while it is lower with a nonrigid connection. Parametric analysis showed that a higher elastic modulus of the connecting wire in the nonrigid anchorage reduces the anchorage loss and risk of root resorption, but increases the stress on the miniscrew.

Combined Effects of Different Bracket Bonding Adhesives and Different Resin Removal Methods on Enamel Discoloration: A Preliminary Study

Summary: This study investigated the effects of three orthodontic bracket adhesives and three resin removal methods on enamel discoloration. The results showed that all nine combinations of adhesives and resin removal techniques caused visible discolorations. Self-etch composites or RMGI might be recommended over total etch composites for adhesive selection. Using Stainbuster burs together with tungsten carbide burs is recommended to reduce discoloration. However, the choice of adhesive removal technique can greatly affect the coloration caused by each composite type.

Associations among the occurrence or types of maxillary canine impaction, labial frenum attachment types, lateral incisor anomalies, and midline diastema in patients with and without impaction: A case-control study

Summary: This study investigated the influence of labial frenum attachment types on the occurrence or type of maxillary canine impaction. It was found that buccal type of impaction was positively associated with papillary labial frenums. Canine impaction occurred 3.6 times more frequently in cases with lateral abnormality. Midline diastema was positively and independently associated with canine impaction, papillary frenums, and abnormal laterals.

Anatomy of the Posterior Superior Alveolar Artery: a Cone-Beam Computed Tomographic Study

Summary: The diameter of PSAA in this Iranian population is smaller compared to other populations, with very few large canals present. Although the average diameter increased in the anterior regions, large canals were not present in these regions. Males tend to have larger PSAAs, age might not change canal parameters, gender might affect canal diameter, and right/left sides are similar in terms of PSAA anatomy.
