Purpose of reviewTo review the current management of myomas with the advent of selective progesterone receptor modulators.Recent findingsSelective progesterone receptor modulators have proved effective and recent publications on the use of ulipristal acetate (UPA) have analyzed the performance of long-term intermittent utilization of 10mg UPA given in repeated courses of 3 months. This long-term intermittent therapy maximizes the efficacy of UPA. Indeed, control of bleeding is achieved sooner after each course. With each subsequent course, a statistically greater number of patients show a fibroid volume reduction of more than 50%.SummaryThe choice of therapy is influenced by different factors, such as the severity of symptoms, tumor characteristics, age, and wish to preserve the uterus (and fertility). Use of UPA will undoubtedly modify the surgical approach.

A synopsis of the 2021 International Society of Fertility Preservation bi-annual meeting

Summary: The International Society for Fertility Preservation (ISFP) held its first virtual meeting on November 19, 2021. The presentations covered various topics, including ovarian stem cell therapy for premature ovarian insufficiency, practical aspects of oocyte vitrification, ovarian stimulation for patients with breast cancer, and more.

Follicle outcomes in human ovarian tissue: effect of freezing, culture, and grafting

Summary: This experimental study investigated the effects of freezing, in vitro culture (IVC), and grafting to the chorioallantoic membrane (CAM) on follicle outcomes in human ovarian tissue. The results showed that IVC led to a decrease in primordial follicles (PF), while FT-CAM tissue maintained a higher proportion of PF. Additionally, grafting to CAM suppressed follicle activation, apoptosis, and autophagy, providing further protection for the follicles. Therefore, enhancing neoangiogenesis and exploring the potential benefits of administering antimullerian hormone are crucial for follicle preservation.

Impact of first chemotherapy exposure on follicle activation and survival in human cryopreserved ovarian tissue

Role of apoptosis and autophagy in ovarian follicle pool decline in children and women diagnosed with benign or malignant extra-ovarian conditions

Summary: The size of the ovarian follicle pool is regulated by both apoptosis and autophagy. Apoptosis is responsible for the physiological decline of the follicle pool over time and increases in the event of prior chemotherapy. Autophagy plays a major role in the diminution of the follicle pool before puberty.

M2 macrophages enhance endometrial cell invasiveness by promoting collective cell migration in uterine adenomyosis

Summary: This study found that M2 macrophages play an important role in the invasiveness of adenomyosis. M2 macrophages accumulate more in patients with adenomyosis compared to healthy tissue. Co-culture with M2 macrophages significantly enhances the invasion capacity of endometrial cells. The study did not find any gene expression differences indicating epithelial-mesenchymal transition (EMT).

Could IVF replace reproductive surgery? No, reproductive surgery is still very much alive

Summary: IVF cannot replace reproductive surgery because reproductive surgery can heal or harm the organs involved in reproduction. This paper explores different diseases and conditions that affect fertility, and highlights the potential benefits of technological innovations, novel applications, and the expertise of reproductive surgeons. The promise of restoring functional anatomy for enhanced pregnancy chances keeps reproductive surgery alive, and it is our responsibility to properly train young residents to provide the right treatment at the right time.

Assessment of folliculogenesis in ovarian tissue from young patients with Turner syndrome using a murine xenograft model

Summary: The impact of aneuploid granulosa and stromal cells on folliculogenesis of small ovarian follicles from patients with mosaic Turner syndrome (TS) was studied using a murine xenograft model. It was found that despite the presence of aneuploid cells, small follicles from TS patients were able to undergo normal development, suggesting that ovarian tissue cryopreservation could be a valid option to preserve fertility in young TS patients, especially before the age of 12.

Importance of oxygen tension in human ovarian tissue in vitro culture

Summary: There is a difference in viability and quality of human follicles cultured in ovarian cortex between 20% and 5% oxygen tension. The viability and quality of follicles are higher under 5% oxygen tension after 6 days of in vitro culture (IVC).

Topics covered included fertility preservation approaches in cancer and benign pathologies, male factor infertility, in vitro maturation and poor responders, the impact of chemotherapy on the ovary, and future perspectives.

Profile of Linzagolix in the Management of Endometriosis, Including Design, Development and Potential Place in Therapy: A Narrative Review

Summary: Estrogens play a critical role in the pathogenesis of endometriosis, and lowering estradiol levels with oral GnRH antagonists, such as linzagolix, can be an effective option for managing symptomatic endometriosis in premenopausal women. Linzagolix is an orally administered GnRH receptor antagonist that reduces LH and FSH production, leading to a dose-dependent decline in estrogen. Phase 2 and 3 trials have shown promising results for the use of linzagolix in the treatment of endometriosis.

Hysteroscopic management of molar pregnancy: A series of 36 cases

Summary: This study presents the results of hysteroscopic resection for the management of hydatidiform mole in 36 patients. The main complications encountered during the surgery were uterine perforation and glycine resorption. Retained product of conception was found in six patients during the ultrasound control one month after the intervention. In conclusion, hysteroscopic resection is a feasible procedure for the treatment of molar pregnancy.
