Development of a Novel Predictive Model for the Clinical Course of Crohn's Disease: Results from the CONNECT Study

Background: A considerable number of patients with Crohn's disease (CD) develop irreversible intestinal damage, although the early administration of immunomodulatory or biological therapies might prevent this. The aims of our study were to develop and validate a novel predictive model that can be used to predict the risk of surgical intervention in Korean patients with CD. Methods: The prognostic model was derived from the multicenter longitudinal CONNECT (CrOhn's disease cliNical NEtwork and CohorT) study cohort consisting of 1338 patients with CD, who were split into training and validation sets. The Korean Crohn's Disease Prediction (KCDP) model was developed with the training set data using the Cox proportional hazards model and multivariate analysis, and was then validated using the validation set. Results: A total of 1271 patients with CD were analyzed. During the follow-up period of 10,188 patient-years (median 7.1 yrs), 361 patients (28.4%) underwent CD-related surgery. Age at diagnosis, jejunal involvement, initial disease behavior, and perianal disease at diagnosis were associated with a poor prognosis and included in the KCDP model, which showed a modest discrimination ability with a Harrel's c-index of 0.731 at 5 years, and was well calibrated (Hosmer-Lemeshow x(2) = 8.230, P = 0.511). Conclusions: This is the first validated surgery risk prediction model for Korean patients with CD; it provides accurate individualized estimates of the probability of surgery using clinical parameters collected at diagnosis. This model might guide appropriate patient selection for the early intensive treatment of CD.

Summary: Current cancer immunotherapeutic strategies mainly focus on remodeling the tumor microenvironment (TME) to make it favorable for antitumor immunity. A galactan-enriched nanocomposite (Gal-NC) has been developed as a carbohydrate nanoadjuvant with a macrophage-targeting feature, which can effectively restore and enhance the weakened antitumor immunity. Gal-NC mediates Toll-like receptor 4 (TLR4) activation, inducing the repolarization of tumor-associated macrophages (TAMs) towards immunostimulatory/tumoricidal M1-like phenotypes, leading to increased intratumoral population of cytotoxic T cells and synergistically enhancing T-cell-mediated antitumor response.

The results showed that B. breve CBT BR3 improved colitis symptoms, increased the number of goblet cells, and protected inflammation-induced epithelial cell permeability. The results showed that CF in CD patients had a large number of genes related to immune cell responses, while genes with a decremental pattern were associated with cell migration. Cell analysis revealed significant changes in cellular composition between CD-CF and Con-MAT groups.

Influence of endoscopists & apos; expertise level on clinical outcomes after bridge-to-surgery stenting in obstructive colorectal cancer

Summary: This study aimed to investigate the effect of stenting-related factors, including endoscopists' expertise, on clinical outcomes after bridge-to-surgery (BTS) stenting for obstructive colorectal cancer (CRC). The results showed that the expertise of endoscopists was associated with better short-term outcomes, including high stenting success rate and low rate of stent-related complications after BTS stenting. The interval between stenting and surgery also played a role in postoperative complications.

Treatment of primary sclerosing cholangitis combined with inflammatory bowel disease

Summary: Primary sclerosing cholangitis (PSC), strongly associated with inflammatory bowel disease (IBD), is a progressive disease characterized by cholestasis, inflammation, and fibrosis. PSC-IBD represents a unique entity with increased risk of malignancy, including colorectal cancer and cholangiocarcinoma. Despite advancements, no effective medical therapy has significantly impacted PSC prognosis. Treatment options for PSC-IBD are the same as for PSC alone, but potential drugs targeting bile acids, inflammation, fibrosis, and gut dysbiosis have been developed. This review summarizes current medical treatments and emerging therapeutic agents for PSC-IBD.
