Summary: Children with inflammatory bowel disease are more susceptible to Clostridium difficile infection, with ulcerative colitis patients being more affected than Crohn's disease patients. Managing pediatric patients with both IBD and CDI is challenging and requires a multidisciplinary approach. Treatment for underlying IBD may need to be escalated after CDI treatment.

Summary: The risk of major bleeding is increased in patients with inflammatory bowel disease during anticoagulation, with exposure to anticoagulation and ulcerative colitis as independent risk factors for major bleeding events. This risk may be outweighed by the high VTE recurrence rate in patients with IBD.

Summary: Inflammatory bowel disease (IBD) patients have a higher risk of Clostridium difficile infection (CDI) after hospitalization for IBD flare. The 90-day CDI-specific readmission rate is 0.1%, with risk factors such as history of Parkinson's disease, prior CDI, HIV-positive status, and alcohol abuse disorder identified. Health care costs, charges, and burden for CDI-associated readmissions are high.

Summary: This review discusses the differential regulation of miRNAs in inflammatory bowel disease (IBD) and their potential diagnostic and therapeutic applications. Through literature curation and computational analysis, a consensus set of differentially expressed miRNAs in mucosal tissues was obtained. The role of these miRNAs and their potential targets in IBD are highlighted.

Summary: Patients with longstanding inflammatory bowel disease, such as ulcerative colitis and Crohn's disease, are at high risk of developing intestinal cancers. This study found significant differences between intestinal cancers associated with ulcerative colitis and Crohn's disease. Crohn's disease-associated cancers were diagnosed at more advanced stages, detected less frequently by surveillance, and had a poorer prognosis compared to those associated with ulcerative colitis.

Summary: This study investigates the associations of immunoglobulin A1 and A2 glycosylation with inflammatory bowel diseases (IBD). Differences in IgA O- and N-glycosylation between patient groups were found to be associated with the diseases, and a statistical model was constructed to predict the disease group of the patients. These findings contribute to the development of noninvasive biomarkers for IBD and improvement in patient care.

Summary: This study aimed to identify the perioperative risk factors for venous thromboembolism (VTE) in patients undergoing colorectal surgery for inflammatory bowel disease (IBD) and create a predictive scoring system. The study found that ASA grade, ulcerative colitis, sepsis, low serum sodium, open abdomen, and preoperative inter hospital transfer were associated with an increased risk of VTE. A risk model was constructed using these 6 significant factors, and the risk of VTE increased exponentially with the number of risk factors.

Summary: This study evaluated the practice of EUS-BD and identified barriers to its widespread adoption. The uptake of EUS-BD as a first-line treatment for MDBO was low, mainly due to lack of high-quality data, fear of adverse events, and limited access to EUS-BD dedicated devices. In salvage situations following failed ERCP, most favored EUS-BD, but in potentially resectable disease, the percutaneous approach was preferred due to concerns about complicating future surgery.

Summary: This study comprehensively reviewed the effectiveness of topical hemostatic agents in the management of upper gastrointestinal bleeding (UGIB) in different clinical settings. The results showed that topical hemostatic agents achieved immediate hemostasis effectively and had similar overall rebleeding rates compared to conventional endoscopic modalities. However, due to methodological limitations of available data, additional studies are needed to confirm their effectiveness in the management of patients with UGIB.

Summary: Background and study aims: Malignant gastric outlet obstruction (MGOO) is traditionally treated with surgical gastrojejunostomy (SGJ) or endoscopic stenting (ES). This study compared the effectiveness and safety of endoscopic ultrasound-guided gastroenterostomy (EUS-GE) to SGJ and ES for MGOO. Results showed that EUS-GE had higher clinical success rates and lower adverse event rates compared to ES or SGJ, making it the most successful approach.

Summary: This systematic review and meta-analysis compared the predictive abilities of different pre-endoscopic scores in prognosticating adverse events in patients with upper gastrointestinal bleeding (UGIB). The Glasgow-Blatchford score (GBS) showed good discriminative ability and clinical utility in predicting the need for hospital-based interventions. Setting the GBS cutoff at 1 or lower effectively identified low-risk patients and allowed for outpatient management.

Summary: Due to the high effectiveness and lack of adverse effects, research has focused on using biological agents and natural products as an alternative method for treating incurable diseases like cancer. This study tested NDV and lavender oil on cancer and normal cell lines, and found that both substances showed cytotoxic effects on cancer cells. Pathological abnormalities were observed in the treated cells, indicating the harmful impacts of the substances on cell appearance. Additionally, programmed cell death was examined using acridine orange and ethidium bromide staining.

Summary: Upper gastrointestinal bleeding (UGIB) is a common gastrointestinal emergency that has significant morbidity and mortality. The epidemiology of UGIB has been changing with a decrease in peptic ulcer disease and an increase in other etiologies. Risk assessment and patient stratification are important for optimal care, with some tools able to identify a low-risk group for outpatient management. Primary care providers play a crucial role in resuscitation, stabilizing patients, providing blood transfusions, and arranging urgent endoscopic assessment and intervention. Managing antithrombotic agents in the acute setting remains challenging.

Summary: This study aimed to understand the experiences and perspectives of patients living with inflammatory bowel disease (IBD) in accessing healthcare. The findings revealed that IBD patients face numerous challenges including issues related to timely access to specialty care. It is crucial to have a comprehensive understanding of healthcare system structures, processes, and their impact on accessing care.
