Summary: This study investigates the surgical outcomes of non-prompt aortic surgery for acute type A aortic dissection (ATAAD) patients. The results show that urgent but non-prompt aortic surgery could provide acceptable surgical results for ATAAD patients without preoperative shock and malperfusion who did not seek medical attention or who could not make their minds to undergo surgery immediately after symptom onset, as long as they are hospitalized with intensive care.

Summary: Despite being more common in men, women with thoracic aortic aneurysm and dissection (TAAD) receive lower rates of treatment and surgical intervention, leading to worse outcomes. Additionally, an increasing number of women with TAAD desire pregnancy, which poses an increased risk of aortic complications. Understanding the sex-specific differences in TAAD holds the potential to enhance care delivery, reduce treatment disparities, and optimize outcomes for women with TAAD.

Summary: This study investigated the association between the macroscopic mechanical response of aortic dissection (AoD) flap, its fibre features, and patient physiological features and clinical presentations. The study found that fibre dispersion and waviness in the aortic dissection flap changed with patient age and clinical presentations. The elastin and collagen area percentages were positively correlated with material constants, while the elastin and collagen waviness were negatively correlated with material constants. The study also found that D2 was an effective parameter to differentiate patient groups with different age and clinical presentations.

Summary: This study examines the ratio between ascending and descending aorta diameters as a potential surrogate marker for decision making in cases of ascending aortic dilatation. A retrospective observational cohort study was conducted on 50 patients who underwent surgery for acute type A aorta dissection. The results suggest that the ratio between descending and ascending aorta diameters may be a useful tool for stratifying the risk of dissection.

Summary: Management of type A aortic dissection complicated by cerebral malperfusion presents a challenge. Patients with internal carotid artery (ICA) occlusion have poor outcomes, while those with common carotid artery (CCA) occlusion or comatose state should not be dismissed for surgery. Prompt neck computed tomography angiography in patients with cerebral malperfusion may be necessary.

Summary: This study evaluated the diagnostic capacity of intestinal fatty acid-binding protein (IFABP) levels in detecting visceral malperfusion after open thoracoabdominal aortic repairs. The results showed that elevated IFABP concentrations in serum accurately detected visceral malperfusion during the first 12 hours after surgery. Therefore, IFABP measurements can be a valuable tool for reliable and timely detection of visceral malperfusion.

Summary: Antimicrobial resistance remains a significant public health issue, largely due to misuse of antimicrobials. This study in a tertiary care teaching hospital in Malaysia investigated trends and compliance of antimicrobial prescribing, finding that ICU had the highest prevalence of antimicrobial use, with many prescriptions being empirically prescribed for pneumonia. The study also highlighted the need to improve documentation of review/stop date recorded and surgical prophylaxis prescribing practices.

Summary: This study evaluates the diagnostic performance of digital breast tomosynthesis (DBT) compared to breast magnetic resonance imaging (MRI) in detecting breast lesions. The results show that DBT with ultrasound (USG) has lower sensitivity but higher accuracy compared to MRI. The correlation with histopathological examination is statistically significant.

This study aimed to determine antibiotics utilization in ICUs by measuring antibiotics consumption over the past six years and specifically evaluating carbapenems prescribed in a COVID-19 ICU and a general intensive care unit during the second year of the COVID-19 pandemic.

Summary: This study aims to develop a generalized and reliable prognostic model for ICU admission for both COVID-19 and non-COVID-19 patients. By analyzing the features of patients at admission, important variables were identified to improve the accuracy of predicting ICU requirement risk. The stacking machine learning model demonstrated the best performance, providing effective prediction of ICU requirement risk for patients.
