Aortic dissection: A life-threatening stroke mimicker

Aortic dissection is a life-threatening cardiovascular emergency, and despite medical advances, its diagnosis is often shrouded by the diversity of its clinical presentation. Physician must have high index of suspicion, especially in those with unusual combination of features that may occur in acute painless dissection. We report a case of a man presented with right-sided weakness and syncope associated with pulse deficits at multiple levels. Physical examination of the lower limb is consistent with acute limb ischemia. Computed tomogram thorax done after emergency embolectomy revealed extensive Stanford type A aortic dissection down to the level of abdominal aortic bifurcation. The diagnosis of aortic dissection is delayed due to his atypical symptoms, which is likely resulted from malperfusion syndrome of aortic dissection.

Summary: Antimicrobial resistance remains a significant public health issue, largely due to misuse of antimicrobials. This study in a tertiary care teaching hospital in Malaysia investigated trends and compliance of antimicrobial prescribing, finding that ICU had the highest prevalence of antimicrobial use, with many prescriptions being empirically prescribed for pneumonia. The study also highlighted the need to improve documentation of review/stop date recorded and surgical prophylaxis prescribing practices.

Summary: Periodic surveillance of hospital-acquired pathogens is crucial for antimicrobial control and infection prevention. The second MRSA surveillance at HCTM in 2017 found a dominant strain resistant to multiple antibiotics, suggesting clonal replacement in the hospital within 8 years. Ongoing investigation of the new MRSA clone's phenotype changes is underway.

Comparative Study of Digital Breast Tomosynthesis (DBT) with and without Ultrasound versus Breast Magnetic Resonance Imaging (MRI) in Detecting Breast Lesion

Summary: This study evaluates the diagnostic performance of digital breast tomosynthesis (DBT) compared to breast magnetic resonance imaging (MRI) in detecting breast lesions. The results show that DBT with ultrasound (USG) has lower sensitivity but higher accuracy compared to MRI. The correlation with histopathological examination is statistically significant.

Summary: Excessive antibiotic consumption is still common among critically ill patients in intensive care units, especially during the COVID-19 period. However, there is limited information on antimicrobial consumption in ICUs in Southeast Asia. This study aimed to determine antibiotics utilization in ICUs by measuring antibiotics consumption over the past six years and specifically evaluating carbapenems prescribed in a COVID-19 ICU and a general intensive care unit during the second year of the COVID-19 pandemic.

Summary: This study aims to develop a generalized and reliable prognostic model for ICU admission for both COVID-19 and non-COVID-19 patients. By analyzing the features of patients at admission, important variables were identified to improve the accuracy of predicting ICU requirement risk. The stacking machine learning model demonstrated the best performance, providing effective prediction of ICU requirement risk for patients.

Summary: This study examines the effect of the BNT162b2 vaccine on blood pressure measurements. The results show that after the first dose, a percentage of individuals experienced a decrease in diastolic blood pressure, which decreased further after the second dose. Additionally, abnormal changes in pulse pressure were also observed.
