Summary: Virtual care is crucial for VHA's effort in expanding veterans' access to care, and research on refining virtual care delivery models is needed. A Think Tank convened in December 2019 to address challenges in virtual care research and practice, discuss novel approaches, and develop priorities to enhance virtual care in VHA. The Think Tank identified four key priorities: scaling evidence-based practices, centralizing virtual care, creating high-value care within VHA with virtual care, and identifying appropriate patients for virtual care.

Summary: Patients with persistent Lyme disease/chronic Lyme disease (PLD/CLD) face difficulties in accessing medical care, and clinicians who provide care for this group of patients encounter multiple challenges related to clinical, regulatory, and financial aspects. These barriers may discourage clinicians from providing care to PLD/CLD patients and make it difficult to retain and recruit healthcare professionals for this rapidly expanding population.

Summary: This article introduces a novel strategy for conceptualizing integrated care and presents a typology of milestones based on analysis of cases in Canada and New Zealand. By analyzing interviews with managers and leaders, the authors generate a detailed timeline of integrated care development and propose a comprehensive framework of developmental milestones. This framework can assist policymakers and healthcare leaders in supporting the implementation of integrated care.

Summary: This study uses the Health Care Access Barrier Model (HCAB) to identify barriers and themes impacting access to health care in rural Ecuador. The study found financial, structural, and cognitive barriers to health care access, such as cost of medications, transportation, difficulty scheduling appointments, and misconceptions in health literacy. The study suggests implementing programs like health care subcenters, improving transportation, and introducing a 24/7 call center to address these barriers.

Summary: In a rural health system with limited dermatology appointment availability, primary care provider diagnostic accuracy may be a key barrier to receiving dermatology care and achieving health outcomes. Older or non-working patients are more likely to remain unevaluated rather than seek care that is more convenient but geographically further away.

Summary: The study demonstrates that increasing endovascular stroke therapy (EVT) availability through the implementation of an integrated stroke system (ISS) decreases the time from stroke onset to EVT-performing hospital (EPH) arrival. Following the implementation of ISS, the proportion of patients undergoing interhospital transfer decreased. Results show that ISS implementation resulted in a reduction of treatment time, with comparable outcomes in quality metrics at high and low volume hospitals.

Summary: This study explores the views of public health experts on the adverse trends in life expectancy in England and Wales over the past decade, their causes and possible solutions, as well as their opinions on how the pre-pandemic situation influenced the UK's COVID-19 response. The findings show a lack of consensus among experts on the significance and explanations of the decline in life expectancy during this period. Some attributed it to data misinterpretation, widening health inequalities, and disinvestment in public services, while others emphasized social factors. The majority called for increased investment and implementation of existing evidence on reducing health inequalities, highlighting the need to address these underlying issues for pandemic preparedness.

Summary: Addressing the Commercial Determinants of Health (CDoH) is crucial for achieving health equity and meeting the health-related targets of the SDGs. This study presents a visual heuristic of three core aspects of CDoH and highlights the need for more research and evidence from the Global South, particularly on effective strategies to curb harmful impacts. It also proposes an agenda and actions for different stakeholders to address CDoH, emphasizing the importance of focusing on system-level changes to improve health equity.

Summary: Recognising the lack of preparedness for the COVID-19 pandemic, international organisations are calling for additional funding to strengthen pandemic preparedness and response systems in low-income and middle-income countries. This article examines the prospects of national health budgets increasing in such a context, revealing that public spending will decrease for almost half of these countries by 2024. The increase in public spending on repaying external debt is a key driver of this austerity wave, potentially leading to deteriorating population health and reduced public health services.

Summary: An unprecedented mortality crisis occurred in Eastern Europe during the 1990s, resulting in around seven million excess deaths. This study examines the relationship between deindustrialization and mortality in Eastern Europe and proposes a theoretical framework that identifies deindustrialization as a process of social disintegration caused by shock therapy. The findings show that deindustrialization directly influenced male mortality rates and was indirectly mediated by hazardous drinking as a coping strategy.

Summary: This study aimed to identify relevant outcomes for people living with NCDs from refugee and host communities in northern Lebanon and define the processes needed to achieve them through an integrated model of care. Two distinct pathways to integrated NCD primary care were identified: a multidisciplinary service pathway and a patient and family support pathway. This paper provides a theory-driven road map of implementation pathways to support the integration of NCD care for crises-affected populations in Lebanon and globally.

Summary: The number of deaths occurring in private homes in England and Wales, particularly from 2020, has been increasing rapidly. Media reports and research have linked decomposing bodies found in private homes with pandemic-related social isolation. This study aims to determine whether these incidents are isolated or part of a larger trend.
