Persistence of Nonceliac Wheat Sensitivity, Based on Long-term Follow-up

We investigated how many patients with a diagnosis of nonceliac wheat sensitivity (NCWS) still experienced wheat sensitivity after a median follow-up time of 99 months. We collected data from 200 participants from a previous study of NCWS, performed between July and December 2016 in Italy; 148 of these individuals were still on a strict wheat-free diet. In total, 175 patients (88%) improved (had fewer symptoms) after a diagnosis of NCWS; 145 of 148 patients who adhered strictly to a gluten-free diet (98%) had reduced symptoms, compared with 30 of 52 patients who did not adhere to a gluten-free diet (58%) (P <.0001). Of the 22 patients who repeated the double-blind, placebo-controlled challenge, 20 reacted to wheat. We conclude that NCWS is a persistent condition. Summary: This study aimed to evaluate the occurrence of histologically verified thyroid pathologies in breast cancer survivors. The results showed that some women experienced pathological changes in the thyroid after breast cancer treatment, including both benign and malignant conditions. Therefore, breast cancer survivors should be aware of the risk of thyroid pathologies and undergo regular screening.

Summary: Klippel-Trenaunay syndrome is a rare vascular malformation with a wide range of symptoms, including rare involvement of the uterus which can cause severe menorrhagia. Diagnosis is based on physical signs and symptoms, and CT and MRI are the preferred methods for evaluating the extent of the lesion and tissue infiltration. Treatment is generally conservative, but surgery may be considered.

Familial Mediterranean Fever and Diet: A Narrative Review of the Scientific Literature

Summary: This narrative review analyzed the correlation between diet and clinical outcomes of familial Mediterranean fever (FMF). Conflicting results were found regarding the association between high-fat and high-salt food intake and FMF attack recurrence. Wheat was suggested to potentially play a role in FMF. A diet rich in antioxidants and anti-inflammatory supplements may partly reduce symptoms and improve the well-being of FMF patients. However, further studies are needed to clarify the impact of diet on FMF symptom triggering.

Potential tolerability of ancient grains in non-celiac wheat sensitivity patients: A preliminary evaluation

Summary: This observational study found that Non-celiac Wheat Sensitivity (NCWS) patients who consume ancient grains may have a delayed diagnosis due to the clinical benefit (tolerability) obtained with these grains. Even after diagnosis, some patients continue to consume ancient grains and have mild or no symptoms.

Colorectal infiltrating deep endometriosis: Laparoscopic treatment. A case report

Summary: Endometriosis is a common disease affecting women of fertile age, with deep infiltrating endometriosis of the rectosigmoid colon presenting symptoms mimicking intestinal obstruction or neoplasm. This case report presents a 39-year-old female patient with pelvic pain and anemia during the menstrual cycle, who was found to have ectopic endometrial tissue in the rectum and underwent a surgical procedure.

Can Baseline IL-6 Levels Predict Long COVID in Subjects Hospitalized for SARS-CoV-2 Disease?

Summary: The immune response to COVID-19 is important, but some patients experience persistent symptoms that lead to a global health and economic burden. This study investigates the potential role of IL-6 levels as a marker for long COVID. The analysis of 184 patients admitted to a hospital in Italy revealed that higher IL-6 levels were associated with a doubled risk of long COVID, mobility decline, and PTSD, indicating the significance of IL-6 in disease severity, unfavorable outcomes, and long COVID development.

Fecal Calprotectin in Self-Reported Milk Intolerance: Not Only Lactose Intolerance

Summary: Inflammatory/allergic conditions should be considered in self-reported milk intolerance patients who test negative and/or are asymptomatic at Lactose Hydrogen Breath Test. Fecal calprotectin values were analyzed in these patients and suggest the existence of an allergic/inflammatory pathogenetic mechanism in a subset of self-reported milk intolerance subjects. Fecal calprotectin results are in keeping with this hypothesis, but cannot differentiate lactose tolerant from intolerant patients.

Summary: This case report describes the successful extraction of a giant gastric phytobezoar through a combined laparoscopic and endoscopic surgery. The laparoscopic approach appears to have better postoperative outcomes, but is criticized for the risk of abdominal spillage and longer operative times. In this case, a laparoscopic and endoscopic cooperative surgery was performed to overcome the limitations of a single technique.

Summary: This retrospective study evaluated the utility of US-guided percutaneous puncture (PP) in the diagnosis and therapy of malignant pleural effusion (PE). The results showed that the US-guided PP fluid examination had high diagnostic accuracy and sensitivity for malignant PE, with minimal complications.
