Understanding Experiences of Youth Growing Up with Anorectal Malformation or Hirschsprung's Disease to Inform Transition Care: A Qualitative In-Depth Interview Study

Introduction The impact of anorectal malformation (ARM) or Hirschsprung's disease (HD) in children continues into adulthood despite early surgical correction. We aimed to explore the physical, social, and emotional impacts of these conditions on youth to inform best transition care strategies. Materials and Methods Eligible participants were those aged between 14 and 21 years who had undergone surgery for ARM/HD in our institution. We conducted one-on-one in-depth interviews to saturation using a question guide developed from literature review and clinician expertise. Deidentified transcripts were coded by four coders (two pediatric surgeons, one psychiatrist, and one qualitative expert) for major themes using a constant comparison approach. A theoretical model for understanding the transition experience was developed using grounded theory. Results Out of 120 patients identified as eligible, 11 youth (6 males) participated in the study. Interviews lasted from 50 to 60 minutes. Four major themes emerged: (1) social support (subthemes: family as core, friends as outer support), (2) cognitive and emotional change (subthemes: realization/recognition of illness, matching emotional response), (3) impact of physical symptoms (subthemes: adverse effects of abnormal bowel habits, gaining bowel continence control leading to overall feeling of control, need to keep disease private), and (4) healthcare providers (viewed as important information sources). Themes did not differ by gender. Conclusion Our model suggests that participants' understanding of bowel disease evolved over time with a lightbulb moment in preteens or early teens accompanied by increasing disease ownership and self-management. Clinicians should independently engage with patients in late childhood to address evolving emotional and information needs and encourage increasing autonomy. Future studies should explore communication approaches to meet transition needs of patients.

SARS-CoV-2 infection and venous thromboembolism after surgery: an international prospective cohort study

Summary: Surgical patients with peri-operative or recent SARS-CoV-2 infection are at increased risk of postoperative venous thromboembolism compared to those without a history of SARS-CoV-2. However, the optimal prophylaxis and treatment for venous thromboembolism in this patient population remains unknown. Venous thromboembolism is independently associated with 30-day mortality.

Intra-abdominal hypertension in neonates following congenital diaphragmatic hernia repair: Correlation with early postoperative respiratory and gastrointestinal outcomes

Summary: The aim of this study was to determine the incidence of intra-abdominal hypertension (IAH) after congenital diaphragmatic hernia (CDH) repair and its relationship with duration of ventilatory support and gastrointestinal function. The study found that IAH occurred within the first 3 days post surgery, with a strong correlation between the duration of IAH and the duration of ventilation, as well as a moderate correlation between the duration of IAH and the time taken to achieve full enteral feeding.

Effectiveness of table salt versus copper sulphate in treating umbilical granuloma: A pilot randomized controlled trial

Summary: This study compared the effectiveness of table salt (NaCl) and copper sulphate (CuSO4) in treating umbilical granuloma. The results showed that the complete resolution rate was significantly higher in the NaCl group compared to the CuSO4 group, and no complications were observed in the NaCl group. Therefore, table salt is an ideal choice for treating umbilical granuloma.

'It's going to change the way we train': Qualitative evaluation of a transformative faculty development workshop

Meta-analysis of donor-recipient gender profile in paediatric living donor liver transplantation

Sociodemographic factors affecting paediatric surgical training in Malaysia: Gender matters

Summary: This study developed and validated a novel index (SPI) to support the strengthening of local elective surgical systems and address growing backlogs. The findings showed that hospitals with a higher SPI were associated with an increased planned surgical volume ratio, independent of income status, COVID-19 burden, and hospital type. Annual self-assessment of surgical preparedness is recommended to identify areas for improvement and enhance the resilience of local surgical systems.

Summary: This study aimed to determine the all-cause mortality rates for childhood cancers during the COVID-19 pandemic and identify factors associated with mortality. The results showed that children infected with SARS-CoV-2 were more likely to die within 30 days, but timely treatment reduced the odds of death.

Management of acute testicular pain in children: changing trends with improvements in scrotal ultrasonography over 18 years

Summary: We evaluated the performance of scrotal ultrasonography in diagnosing testicular torsion over the past 18 years in our institution and identified factors contributing to longer wait times. The results showed a sensitivity of 100% and a specificity of 98.8% for scrotal ultrasonography in diagnosing testicular torsion. Reducing wait times for patients with acute scrotum is crucial in improving the quality of diagnostic ultrasonography.
