Surgery performed 8-12 hours after diagnosis carries the highest mortality, potentially exacerbated by a longer interval since symptom onset. Time-dependent effects should be taken into consideration, especially in cases where inter-facility transfer is necessary.

Summary: The effect of an aggressive approach on the aortic root in acute type A aortic dissection (ATAAD) remains insufficiently explored. The study analyzed a retrospective cohort of ATAAD patients aged 18 years and older between 1992 and 2020. Patients were divided into two groups: aortic root replacement (ARR) and conservative root approach (CRA). The results showed that over the years, ARR increased and was associated with better long-term survival and fewer reinterventions compared to CRA.

The study suggests improving the treatment and referral system for TAAD in China, standardising diagnosis and treatment training, establishing a fast channel for prioritising TAAD treatment, popularising aortic dissection-related knowledge, and improving the funding system.

Summary: This study aimed to explore the virulence of Staphylococcus species in infectious endocarditis (IE) compared to other commonly causative bacteria in terms of short-term and long-term mortality. The results showed that patients with Staphylococcus IE had significantly higher mortality rates than those with IE caused by other bacteria. It is recommended to make an early diagnosis and refer patients with Staphylococcus infection to specialized centers for surgical treatment to reduce the risk of septic embolism and stroke.

Summary: This study aimed to investigate the epidemiology, mortality, diagnosis, and clinical management of HIT occurring in VA-ECMO patients treated with UFH. The prevalence of HIT in VA-ECMO patients was found to be 3.5%, and it was not associated with a higher mortality rate. The functional assay was able to exclude HIT in 75% of VA-ECMO patients with clinical suspicion and positive anti-PF4/heparin antibody test. Argatroban appears to be a suitable and safe therapeutic option for confirmed HIT-positive patients on VA-ECMO support.

Summary: In this retrospective study, the attrition rate in the Austrian Myeloma Registry (AMR) was analyzed, and the results showed lower attrition rates compared to previous data, especially in elderly patients and those receiving optimal frontline treatment. The importance of achieving optimal treatment response was highlighted, not only in terms of attrition rates, but also in terms of treatment-free intervals. These promising results support the key role of liberal universal drug access and reimbursement.

Summary: In this study, the efficacy and safety of suture-based and plug-based vascular closure devices for VA-ECMO decannulation were compared. The results showed that plug-based devices had a higher rate of technically correct placement and a lower incidence of severe bleeding events during decannulation. Therefore, plug-based vascular closure should be considered as the preferred option for VA-ECMO decannulation.

The postoperative mortality rate was 8.6%, and the 10-year survival rate was 80%.
