Endoscopic vs Open Vein Harvest in Drug-Eluting Stents or Bypass Surgery for Left Main Disease Trial

Craig M. Jarrett, Marc Pelletier, Yasir Abu-Omar, Cristian Baeza, Yakov Elgudin, Alan Markowitz, Pablo Ruda Vega, Ovidiu Dressler, Arie Pieter Kappetein, Patrick W. Serruys, Gregg W. Stone, Joseph F. Sabik

Summary: The study compared the outcomes of coronary artery bypass grafting (CABG) with endoscopic vein harvest (EVH) vs open vein harvest (OVH). The patients with different vein harvesting methods had different rates of ischemia-driven revascularization at 5 years. No significant differences were found in other endpoints, such as graft stenosis or occlusion, death, stroke, myocardial infarction, bleeding, blood product transfusion, major arrhythmia, and infection.

Intravascular imaging assessment of pharmacotherapies targeting atherosclerosis: advantages and limitations in predicting their prognostic implications

Vincenzo Tufaro, Patrick Washington Serruys, Lorenz Raber, Martin Richard Bennett, Ryo Torii, Sophie Zhaotao Gu, Yoshinobu Onuma, Anthony Mathur, Andreas Baumbach, Christos Vasileios Bourantas

Summary: Intravascular imaging plays an important role in assessing the effectiveness of drug therapies targeting atherosclerosis. Serial intravascular ultrasound, optical coherence tomography, or near-infrared spectroscopy-intravascular ultrasound studies allow for evaluation of the impact of different treatments on plaque burden and morphology. However, established intravascular imaging surrogate endpoints have limited efficacy in predicting the prognostic value of these therapies, necessitating the development of alternative imaging endpoints based on multimodality/hybrid intravascular imaging.

Moon, Jouke Dijkstra, Patrick W. Serruys, Anthony Mathur, Andreas Baumbach, Ryo Torii, Francesca Pugliese, Christos Bourantas

Summary: This study evaluated the application of different coronary computed tomography angiography (CCTA) reconstruction methods in coronary atherosclerosis using intravascular ultrasound as the reference standard. The results showed that CCTA reconstruction with thinner slice thickness, smooth kernel, and highest strength advanced IR enabled more accurate quantification of the lumen and plaque in coronary atherosclerosis. Impact of residual angina on long-term clinical outcomes after percutaneous coronary intervention or coronary artery bypass graft for complex coronary artery disease

Masafumi Ono, Patrick W. Serruys, Hideyuki Kawashima, Mattia Lunardi, Rutao Wang, Hironori Hara, Chao Gao, Scot Garg, Neil O'Leary, Joanna J. Wykrzykowska, Jan J. Piek, David R. Holmes, Marie-Claude Morice, Arie Pieter Kappetein, Thilo Noack, Piroze M. Davierwala, John A. Spertus, David J. Cohen, Yoshinobu Onuma

Summary: The aim of this study was to investigate the impact of patient-reported anginal status at 1 year following percutaneous coronary intervention (PCI) or coronary artery bypass graft (CABG) on 10-year survival in patients with left main coronary artery disease (LMCAD) and/or three-vessel CAD (3VD). The study found that while angina at 1 year was associated with repeat revascularization at 5 years, it did not significantly increase 10-year mortality.

Discordance between Invasive and Non-Invasive Coronary Angiography: An In-Depth Functional and Anatomical Analysis

Summary: A 79-year-old male patient with chronic coronary syndrome and complex coronary artery disease was enrolled in a clinical trial assessing the use of coronary computed tomography angiography (CCTA) and CCTA-derived fractional flow reserve (FFRCT) for surgical revascularization. Discordance between CCTA and initial FFRCT results required further analysis using invasive and non-invasive coronary angiography. The findings revealed that the stenosis in the proximal left anterior descending artery (LAD) was physiologically significant in angiography-derived fractional flow reserve (FFR), while it remained functionally negative in the second assessment of FFRCT, possibly due to extensive calcification.

Summary: Paravalvular leak (PVL) is a drawback in transcatheter valve replacement (TAVR), and a readily applicable method to accurately quantify PVL (aortography) is appealing to operators. The regurgitation fraction (RF) is determined by the ratio between the time-density curves in the aorta and left ventricular outflow tract (LVOT-AR). This technique has been validated in mock circulation and tested in animal models, and compared with echocardiography and cardiac magnetic resonance imaging in clinical settings.

Summary: Coronary computed tomographic angiography (CCTA) has become the preferred investigation for determining the presence and haemodynamic significance of coronary artery disease. It can rule out atherosclerosis or detect subclinical plaque in patients without significant epicardial obstruction, and provide risk classification. For ischaemic non-obstructive coronary arteries, non-invasive imaging, including CCTA, is also expected. In patients with significant epicardial obstruction, CCTA can assist in planning revascularisation by determining disease complexity, vessel size, lesion length, tissue composition of the atherosclerotic plaque, and the best fluoroscopic viewing angle.
