Summary: Erectile dysfunction is common in adult men, especially in those with Klinefelter syndrome (KS). Higher estradiol/testosterone ratio is associated with impaired erectile function in KS patients, with age and E2/T ratio being the main predictors of erectile function. Larger studies are needed to better understand the pathophysiology of ED in KS patients.

Summary: Sexual function and testosterone levels decline in aging men, but the specific contribution of low testosterone to sexual impairment is still uncertain. Testosterone replacement therapy is effective but has variable effects. Lifestyle interventions can also yield similar outcomes. Therefore, phosphodiesterase type 5 inhibitors and lifestyle measures should be considered as the first line of treatment for erectile dysfunction, with testosterone replacement therapy reserved for cases where other interventions fail.

Summary: A multidisciplinary panel from BSSM reviewed the literature on testosterone deficiency (TD) and provided evidence-based statements for clinical practice. Recent studies have shown that low testosterone levels in men are associated with an increased risk of type 2 diabetes, worse outcomes in chronic kidney disease, COVID-19 infection with increased mortality, and significant impacts on quality of life. These guidelines will aid practitioners in diagnosing and managing primary and age-related TD effectively.

Summary: Disorders of male sexual health and functioning can have significant negative effects on patients' psychological well-being and relationships. Patient-reported outcome measures (PROMs) aim to improve communication and understanding of these issues. This article presents a range of PROMs for key andrological disorders, providing clinicians with options for assessing and addressing these conditions.

Summary: The study aims to assess the impact of MetAge and body composition on male sexual and gonadal status, especially in men younger than 40 years old. The results showed that age and body composition parameters significantly affected the androgenic state. Erectile dysfunction in men younger than 40 years was associated with lower testosterone levels, older MetAge, and higher BMI and degree of obesity.

Summary: Vitamin D deficiency and erectile dysfunction are common in males, especially in the elderly, suggesting a possible relationship between the two conditions. Studies indicate that vitamin D affects erectile function by regulating endothelial function and interfering with comorbidities associated with erectile dysfunction. Low levels of vitamin D may worsen erectile function through various mechanisms. However, the quality and heterogeneity of clinical trials evaluating the effects of vitamin D on erectile function and related comorbidities require further investigation.

Summary: Testosterone is the most important hormone in male health, and aging can result in testosterone deficiency. Testosterone replacement therapy can restore testosterone levels in men, improving sexual dysfunction, increasing bone density and muscle strength, and providing cardioprotective effects. However, it may be contraindicated in men with untreated prostate cancer and may have other potential side effects.

Summary: In men with Klinefelter syndrome, the pooled prevalence of erectile dysfunction (ED) and decreased libido (DL) is 28% and 51% respectively, with significant heterogeneity. ED prevalence increases with age, while DL becomes more common as serum testosterone levels decrease.

Summary: Testosterone is crucial for maintaining physical and mental function. Age-related testosterone depletion leads to various conditions such as angina, arteriosclerosis, obesity, metabolic syndrome, dementia, and frailty. Late-onset hypogonadism (LOH) refers to the condition of age-related testosterone depletion and its associated clinical symptoms, which can be treated with testosterone replacement therapy. The indications for this therapy depend on the evaluation of symptoms and physical signs.

Summary: Several hormones play a role in ensuring penile erection, with nitric oxide being key. Erectile dysfunction can be caused by various factors including arteriogenic, neurogenic, iatrogenic, and endocrinological causes. The hypothalamus-pituitary axis is central to the endocrine system, with dysregulation potentially compromising sexual function.

Summary: Diagnosis of functional hypogonadism should lead to a thorough evaluation and optimization of general health, followed by a consideration of testosterone replacement therapy. Testosterone treatment can improve libido, but may not be as effective for erectile dysfunction. It can also have positive effects on insulin resistance, bone strength, physical strength, and mild depressive symptoms, although the long-term clinical significance of these improvements remains uncertain.

Summary: Male infertility and male sexual dysfunction often coexist and are influenced by factors such as psychosexual, sociocultural, and relationship dynamics. The Asia Pacific Society of Sexual Medicine and the Asian Society of Men's Health and Aging aim to provide clinical management recommendations for MI and MSD in the AP region based on current evidence. A comprehensive review of literature identified various forms of MSD and their association with MI. The panel suggests comprehensive fertility evaluation and multidisciplinary management for each domain of MSD. Individualized treatment options should be discussed and implemented considering patients' needs, beliefs, and local expertise.

Summary: This review describes the relationship between metabolic syndrome and male sexual disorders, as well as the underlying pathophysiological mechanisms. Current evidence shows strong associations between male sexual dysfunction and risk factors such as central obesity, poor glycemic control, hyperlipidemia, and hypertension. Effective interventions targeting these pathophysiological mechanisms have the potential to greatly improve patient sexual function and quality of life.

Summary: This study assessed the impact of different osteotomy and pelvic fixation methods on the outcomes of primary closure surgery for cloacal exstrophy. It was found that multi-stage procedures were more common and had higher success rates than single-stage procedures, and any osteotomy was associated with successful closure. The fixation methods also changed, with an increase in Buck's traction and external fixation and a decrease in spica casting. A multidisciplinary approach with a combination osteotomy was found to improve surgical outcomes.

Summary: This study reviewed the long-term outcomes and experience of using the tunica vaginalis flap (TVF) for complex epispadias repair. The study found that in patients who underwent TVF, 7 developed urethrocutaneous fistulae, 1 had dehiscence, and 1 had both. The results suggest that the use of the tunica vaginalis as an adjunct to epispadias repair can effectively prevent the development of urethrocutaneous fistulae.

Summary: This study describes a bladder neck reconstruction (BNR) with a modified Tanagho anterior detrusor flap for the treatment of posterior urethral stricture after repair of classic bladder exstrophy. Five patients underwent proximal urethral reconstruction with modified Tanagho flap, and four male patients with classic bladder exstrophy were successfully treated for posterior urethral strictures.
