Laparoscopic Pringle maneuver: how we do it?

Laparoscopic liver resection (LLR) is technically possible with new devices which allow a relatively bloodless liver parenchymal transection. Despite, the main concern remains intraoperative hemorrhage. Currently, perioperative excessive blood loss during LLR is difficult to control with necessity of laparotomy conversion. Moreover, major blood loss requires transfusion and increases postoperative morbidity and mortality. When in-flow is limited by the hepatic pedicle clamping, it reduces intraoperative blood loss. The Pringle maneuver, first described in 1908, is the simplest method of inflow occlusion and currently can be achieved during LLR. The purpose of this note was to describe two different modalities of Pringle maneuver used by two different teams during LLR.

Summary: Vascularized composite allotransplantation allows for the reconstruction of complex anatomical structures with loss of substance. Although hand and face transplants remain rare worldwide, they are limited by the burden of immunosuppressive treatments and their consequences. Current research focuses on improving surgical techniques, graft transportation, and immunological treatment in order to achieve better transplant outcomes.

Summary: Patients with heparin-induced thrombocytopenia or confirmed allergies to heparin have a contraindication to heparin therapy for intraoperative free flap irrigation. The use of argatroban as an alternative to heparin in leg salvage and breast reconstruction surgeries showed reliability and effectiveness, without microvascular complications.

Summary: The study aimed to describe the venous anatomy of the digits and the hand, providing detailed results and photographs of anatomical specimens. The study found a consistent and reliable vascular anatomy of the venous system in the digits and hand, which should be taken into consideration by surgeons when performing hand surgeries to preserve normal venous physiology as much as possible.

Anatomy of the axillary arch: from its incidence in human to an embryologic and a phylogenetic explanation of its origins

Summary: The study reported a 2.5% incidence of axillary arch in female cadavers, and conducted embryological and comparative studies. While embryology failed to explain the variation, comparative anatomy argued in favor of an origin from the panniculus carnosus. The axillary arch, a relatively common variant, should be recognized by clinicians and surgeons.
