Intravitreal Diclofenac plus Bevacizumab versus Bevacizumab alone in treatment-naive diabetic macular edema: a randomized double-blind clinical trial

The aim of this study is to evaluate the short-term effects of a single intravitreal injection of 1.25 mg Bevacizumab combined with 300 lg/0.1 mL Diclofenac (IVB/D) versus 1.25 mg intravitreal Bevacizumab (IVB) alone in the treatment of naive diabetic macular edema (DME). In this prospective, randomized clinical trial, 80 eyes were included in the final analysis; 42 and 38 of which in the IVB and IVB/D groups, respectively. The primary outcome measure was a change in best-corrected visual acuity (BCVA) in logMAR at week 4. The secondary outcomes included changes in central macular thickness (CMT), macular volume, and potential injection-related complications. Significant improvement of BCVA was demonstrated in both study arms (mean reductions in LogMAR: -0.088 +/- 0.278, -0.228 +/- 0.330 for IVB and IVB/D, respectively). The difference in BCVA changes was in favor of IVB/D; however, not to a statistically significant level (P = 0.160). Significant reduction of CMT was documented in both study arms (mean reductions: 82.43 +/- 160.09 and 153.26 +/- 163.85 for IVB and IVB + IVD, respectively). Comparison of CMT changes between groups showed that IVB/D reduced CMT more than that of IVB (P = 0.04). Effects on macular volume corresponded to those of CMT. No injection-related complications or significant alterations in intraocular pressure were observed in any of the study arms. In treatment-naive DME, superiority of IVB/D combination therapy over IVB monotherapy may exist; especially as regards anatomical features. In our therapeutic arsenal for DME, IVD can be added as an adjunct to Bevacizumab.

RONA Technique: A Novel ILM Peeling Method for Treatment of Large Full-Thickness Macular Holes

Summary: This study presents a new technique called RONA for internal limiting membrane peeling and shows its comparable success rate in closure of large full-thickness macular holes. The advantage of this technique is that there is no need to manipulate the macular hole edge or underlying tissues, and there is no need to worry about flap unfolding during the surgery.

Summary: This randomized clinical trial compared the efficacy of intravitreal bevacizumab (IVB) and oral acetazolamide (OA) combination therapy with IVB monotherapy in patients with macular edema secondary to retinal vein occlusion (RVO). The results showed that both regimens were effective in improving visual acuity and reducing macular thickness, but combination therapy did not provide additional short-term benefits.

Retinal ischemic cascade: New insights into the pathophysiology and imaging findings

Summary: This article reviews the current knowledge on the pathophysiology and risk factors of paracentral acute middle maculopathy (PAMM), the vasculature and blood flow of the middle and inner retinas, and the vulnerability of the middle retina in vaso-occlusive disorders. The inner nuclear layer (INL) is easily affected by retinal hypoperfusion and ischemia, leading to skip PAMM lesions and fern-like appearance. The progression of INL infarction can result in diffuse globular PAMM and transmural infarction of the middle and inner retinas, resembling an ischemic cascade.

Headache-a challenge across medical students' life: a systematic review and meta-analysis

Are inner nuclear layer ischemic lesions hidden indicators of retinal vein occlusion risk? A case-control study

Summary: The study aimed to investigate the association between retinal vein occlusion (RVO) and evidence of previous, unnoticed inner nuclear layer (INL) infarction in the fellow eye. The results showed a higher prevalence of INL lesions in the fellow eye of hypertension patients with RVO compared to those without RVO. These INL lesions may serve as early markers of increased RVO risk in individuals with hypertension.

Intravitreal Injections and Face Masks: Endophthalmitis Risk Before and During the COVID-19 Pandemic

Summary: To assess the additional risk of acute endophthalmitis after intravitreal injections associated with face masks during the COVID-19 pandemic, a retrospective study was conducted comparing cases during the pre-COVID era (March 1st, 2013 to October 31st, 2019) and the COVID era (March 1st, 2020 to April 1st, 2021) of intravitreal bevacizumab (IVB) injections. The incidence of post-IVB endophthalmitis was compared between the two eras and found to be statistically similar, suggesting that the use of face masks does not increase the risk of post-IVB endophthalmitis.

The effect of vitamin D supplementation on the outcome of treatment with bevacizumab in diabetic macular edema: a randomized clinical trial

Summary: This study aimed to investigate the effect of correcting vitamin D deficiency on the treatment outcome of DME patients. The results showed that correcting vitamin D deficiency, in addition to intravitreal bevacizumab (IVB) injections, may improve visual acuity and central macular thickness (CMT) in these patients. However, the beneficial effect may be delayed by several months.
