Purpose To determine the predictability of gestational diabetes mellitus (GDM) during the first trimester using the degree of insulin resistance and anthropometric measurements and to assign the risk of developing GDM by weight gained during pregnancy (WGDP). Methods A total of 250 singleton pregnancies at 7-12 gestational weeks were studied. Body mass index (BMI), waist/hip ratio (WHR), quantitative insulin sensitivity check index (QUICKI), homeostasis model assessment-insulin resistance (HOMA-IR) scores and WGDP were determined. The backward stepwise method was applied to estimate possible associations with GDM. Cutoff points were estimated using receiver operating characteristic curve analysis. Results GDM was found in 20 of 227 singleton pregnancies (8.8 %). The calculated HOMA-IR, QUICKI, BMI, WHR, WGDP, and parity were significantly associated with GDM. Logistic regression analyses showed that three covariates (HOMA-IR, BMI, WGDP) remained independently associated with GDM. It was calculated as OR 1.254 (95 % CI 1.006-1.563), AUC 0.809, sensitivity 90 %, specificity 61 % with cutoff = 2.08 for HOMA-IR; OR 1.157 (CI 1.045-1.281), AUC 0.723, sensitivity 80 %, specificity 58 % with cutoff = 25.95 for BMI; OR 1.221, (CI 1.085-1.374), AUC 0.654, sensitivity 80 %, specificity 46 % with cutoff = 4.7 for WGDP. Despite a HOMA-IR score of > 3.1 in pregnant women, GDM was detected in only three of 29 patients (10.3 %) if WGDP was < 4.7 kg at weeks 24-28. Conclusions First trimester screening for GDM can be achieved based on maternal anthropometric measurements and HOMA-IR. In particular, if BMI is > 25.95 kg/m(2) and the HOMA-IR score > 2.08, controlling weight gain may protect against GDM.

The effect of attention deficit hyperactivity disorder comorbidity on cognitive functions and severity of addiction in opioid use disorder

Zeliha Donmez, Sakir Gica, Basak Unubol, M. Sinan Iyisoy, Huseyin Gulec

Summary: This study aims to determine changes in social cognition and other cognitive domains in ADHD comorbidity and investigate their moderating role in OUD. Findings suggest that changes in social environment/cognition are important in the follow-up/treatment of patients.

Summary: Elevated levels of acute-phase reactants and lymphopenia are indicative of disease severity in COVID-19. This study focused on investigating the role of apoptosis in lymphopenia in COVID-19 patients. Results showed an increase in apoptosis in B and T lymphocytes, particularly in severe cases, suggesting a potential association between lymphocyte apoptosis and disease severity in COVID-19.

Effects of Combined Sertraline and Magnesium in Rat Atrium

Summary: The study found that the combination of the antioxidant ion Mg+2 and the selective serotonin reuptake inhibitor sertraline has synergistic effects in the treatment of major depression and regulation of inotropic effect in the early postoperative period. However, this combined treatment may lead to shortened blood clotting time and significant decrease in blood lipids, as well as an increase in plasma enzymes, posing potential risks. Further studies are needed to evaluate the safety and efficacy of using magnesium combined with antidepressants for pre or post-operative depression.

Summary: Volumetric changes in the aneurysm sac were evaluated in intact AAA patients following EVAR, showing a gradual decrease in total aneurysm volume, a temporary increase and subsequent decrease in thrombus-coated aneurysm wall volume, and a continuous decrease in patent lumen volume after 6, 12, and 24 months postoperatively.

Reduced Monocyte Subsets, Their HLA-DR Expressions, and Relations to Acute Phase Reactants in Severe COVID-19 Cases

Summary: This study aimed to evaluate monocyte subsets and HLA-DR expressions in COVID-19 patients. The results showed that monocyte and HLA-DR expressions were decreased in COVID-19 patients compared with healthy controls. The expressions of CL and INT monocytes were further decreased in severe patients. NC monocytes were more affected in patients with different clinical features. These findings suggest that decreased monocyte HLA expressions may indicate immune suppression in severe COVID-19 patients.

Summary: This study found that CRT is significantly prolonged in patients with SSc, and in patients with pathological NFC, CRT results are also prolonged and accompanied by higher pulmonary artery pressure levels. Therefore, evaluating CRT with NFC positivity may predict pulmonary progression in patients with SSc, especially in dSSc patients.

The Crucial Role of Awareness and Education in Promoting Effective Diabetes Self-Management and Reducing the Risk of Complications

Hilal Akay Cizmecioglu, Mevlut Hakan Goktepe, Ahmet Cizmecioglu

Summary: This study found that diabetic patients have a high level of awareness of disease complications, with the awareness being highest for ocular-related complications and lowest for neuropathy. Increased education levels can improve awareness of combined drug use, but may decrease adherence. Doctors play a significant role in raising awareness of disease complications.
