Summary: In Asian acute stroke registries, women had poorer prognosis than men after receiving recombinant tissue plasminogen activator for acute ischemic stroke, which worsened with age. Women older than 70 appear to have a worse outcome than men which could be explained by greater stroke severity, more AF, and cardioembolic stroke.

Summary: In the decision-making process about acute treatment, it is important to consider four main questions about a given prediction model: (1) what outcome is being predicted, (2) what patients contributed to the model, (3) what variables are in the model (considering their quantifiability, knowability at the time of decision-making, and modifiability), and (4) what is the intended purpose of the model? By acknowledging the limits of outcome prediction for acute stroke therapies, we can incorporate them into our decision-making more meaningfully, critically examining their contents, outcomes, and intentions before heeding their predictions. By rigorously identifying and optimizing modifiable variables in such models, we can be empowered rather than paralyzed by them.

Summary: This article reports the results of the BASILAR registry, which aimed to evaluate the impact of endovascular treatment and conventional treatment on the clinical outcomes of patients with acute basilar artery occlusion in China. The findings suggest that endovascular treatment is more beneficial in terms of functional outcome at one year compared to conventional treatment, and the one-year mortality rate is lower in the endovascular treatment group.

Summary: It has been found that remote ischemic conditioning (RIC) can attenuate blood-brain barrier (BBB) disruption caused by recombinant tissue plasminogen activator (rtPA) treatment for acute ischemic stroke. RIC reduces BBB injury by decreasing PDGF-CC/PDGFR alpha pathway activation, suggesting its potential as a treatment approach for rtPA-aggravated BBB disruption.

The study also revealed that the cross-sectional area (CSA) of the IC is only associated with sex and not age. Summary: This study aimed to evaluate the effectiveness of HD-SCS after conversion from standard SCS and develop a model for predicting the long-term response of HD-SCS after unsatisfactory standard SCS. Results showed significant time effects for both low back and leg pain responders after conversion to HD-SCS. Logistic regression models highlighted the importance of pain intensity scores, medication use, paresthesia coverage (for back pain), and EQ5D (for leg pain) as predictors of being a responder after 12 months of HD-SCS.
