Background-Fibroblast growth factor 23 (FGF23) is emerging as a novel biomarker of bone metabolism, chronic kidney disease, and cardiovascular disease (CVD). However, its clinical correlates and potential predictive role in a community-based setting are incompletely understood. Methods and Results-We evaluated participants of the Framingham Heart Study (seventh examination cycle of the Offspring cohort plus second examination cycle of the multiethnic Omni cohort) to identify clinical correlates of plasma FGF23 (N=3236) and examine its cross-sectional association with vascular function (N=2209), and longitudinal association with 10-year incidence of CVD (N=2823), and all-cause mortality (N=3223). Circulating FGF23 concentrations were positively related to African-American and Asian ethnicity, waist circumference, current smoking, serum glucose, history of CVD, and antihypertensive medication use; and negatively related to male sex, hormone replacement therapy, and estimated glomerular filtration rate. Multivariable-adjusted cross-sectional analyses showed no consistent association of FGF23 with vascular function measures. During a median follow-up time of 10.8 years, 347 incident CVD events and 412 deaths occurred. Multivariable-adjusted Cox regression models revealed a positive association of FGF23 with all-cause mortality (hazard ratio [HR] per SD increase, 1.31; 95% CI, 1.20-1.42), but not with incident CVD (HR per SD increase, 1.05; 95% CI, 0.94-1.17). Conclusions-In our large, community-based sample, FGF23 was associated with mortality risk, but not with vascular function or incident CVD.

Longitudinal Trends in 30-Day Mortality Between Multi-Site and Single-Site Surgeons

Thomas C. Tsai, Ava Ferguson Bryan, Jie Zheng, Susan Haas, E. John Orav, Evan Benjamin

Summary: Background: Quality leaders are concerned about the potential negative outcomes of multi-hospital health systems due to fragmented surgical care. However, little empirical data exist on the outcomes of multi-site surgeons. Methods: National Medicare data from 2011 to 2016 were analyzed to compare multi-site and single-site surgeons in terms of mortality rates. Results: The number and percentage of multi-site surgeons decreased over time. Overall, multi-site surgeons had lower 30-day mortality rates than single-site surgeons. The mortality advantage of multi-site surgeons decreased over time, and by 2016, there was no significant difference in outcomes between the two groups. Conclusion: Surgeons operating at multiple hospitals can provide surgical care without increased mortality rates.

Summary: Current cardiac biobanking practices are insufficient to capture the diverse array of phenotypes present in cardiovascular diseases (CVDs) and the spatial heterogeneity across cardiac tissue sites. We have developed a rigorous and systematic protocol for the dissection and preservation of human cardiac biospecimens to enhance the availability of whole organ tissue for multiple applications. When combined with longitudinal clinical phenotyping, our protocol will enable multiomics in hearts to deepen our understanding of CVDs.

The Association of Protein Biomarkers With Incident Heart Failure With Preserved and Reduced Ejection Fraction

Katherine S. S. Takvorian, Dongyu Wang, Paul Courchesne, Ramachandran S. S. Vasan, Emelia J. J. Benjamin, Susan Cheng, Martin G. G. Larson, Daniel Levy, Jennifer E. E. Ho

Summary: We identified 5 protein biomarkers associated with incident HFpEF, including NT-proBNP, growth differentiation factor-15, adrenomedullin, uncarboxylated matrix Gla protein, and C-reactive protein. We also found 14 biomarkers associated with incident HFrEF, including myeloperoxidase, resistin, and paraoxanase-1. These findings provide insights into similarities and differences in the development of HF subtypes.

Retaining Faculty from Underrepresented Groups in Academic Medicine: Results from a Needs Assessment

Ellen Childs, Korede Yoloye, Robina M. Bhasin, Emelia J. Benjamin, Sabrina A. Assoumou

Summary: Academic medical centers can improve care quality and address health inequities by recruiting and retaining faculty from underrepresented in medicine (URiM) groups. However, the retention of URiM faculty is a barrier to achieving equity-related goals. This study aimed to identify factors influencing the retention of URiM faculty at large academic centers.

Summary: This study aimed to explore the effectiveness of a postpartum health coaching intervention for reducing postpartum weight gain in Latina populations. The results showed that the intervention did not significantly reduce postpartum weight gain. However, stratified analyses revealed that the intervention had more favorable effects among English speakers and those with higher perceived diabetes risk.

Summary: The study found that clonal hematopoiesis of indeterminate potential (CHIP) is associated with an increased risk of liver inflammation and chronic liver disease progression. Furthermore, experiments showed that mice transplanted with Tet2-deficient hematopoietic cells exhibited more severe liver inflammation and fibrosis. These findings suggest that CHIP contributes to liver inflammation and the development of chronic liver disease through an aberrant inflammatory response.

Global distributions of age- and sex-related arterial stiffness: systematic review and meta-analysis of 167 studies with 509,743 participants

Yao Lu, Sophia J. Kiechl, Jie Wang, Qingbo Xu, Stefan Kiechl, Raimund Pechlaner, Global Pulse Wave Velocity Study Grp

Summary: This study investigated the age and sex trajectories, regional differences, and global reference values of arterial stiffness. The results showed that the global age-standardized means of pulse wave velocity were 12.5 m/s for brachial-ankle PWV and 7.45 m/s for carotid-femoral PWV. Males had higher levels than females, and there were regional differences across different countries.

Summary: Among adolescents in the US, sole use of e-cigarettes is the most common pattern, but concurrent use with other tobacco products is not uncommon and may be associated with high-risk behaviors. The study reveals that the prevalence of sole e-cigarette use, dual use, and poly use among youth is 23.2%, 4.2%, and 3.3% respectively, while nonuse of any tobacco product is reported by 62.9% of youth. Furthermore, youth who use tobacco products have higher prevalence of substance misuse compared to non-users.

Inflammatory biomarkers and MRI visible perivascular spaces: The Framingham Heart Study

Oluchi Ekenze, Adlin Pinheiro, Serkalem Demissie, Andreas Charidimou, Alexa S. Beiser, Ramachandran S. Vasan, Emelia J. Benjamin, Charles DeCarli, Sudha Seshadri, Jose R. Romero

Summary: This study investigated the relationship between inflammatory biomarkers and magnetic resonance imaging-visible perivascular spaces (PVS) in participants without stroke and dementia. The study found significant associations between certain biomarkers and PVS burden in the basal ganglia and centrum semiovale. The findings suggest that inflammation may play a role in the pathogenesis of cerebral small vessel disease and perivascular drainage dysfunction represented by PVS, with different biomarkers depending on PVS topography.

Trends among platelet function, arterial calcium, and vascular function measures

Summary: Arterial tonometry and vascular calcification measures are valuable in assessing cardiovascular disease (CVD) risk. This study examined the relationship between arterial tonometry, platelet function, arterial calcium, and aortic diameter. The results suggest that higher arterial calcium may be associated with increased platelet reactivity, while larger aortic diameter may be linked to lower platelet reactivity. These findings indicate the potential interplay between subclinical atherosclerosis, platelet activation, and vascular structure.
