Summary: The human endometrium is a complex tissue that undergoes a monthly cycle of thickening and shedding without scarring or loss of function. Endometriosis is a reproductive disease where endometrium is found outside the uterus, affecting 6-10% of reproductive-age women worldwide. It causes pain, heavy bleeding, and infertility. The disease is believed to originate from viable endometrial cells that flow back into the peritoneum during retrograde menstruation. Treatment options include surgery, hormonal therapy, medication, diet and exercise, and in severe cases, hysterectomy.

Summary: This article demonstrates the technique of discoid excision of bowel endometriosis followed by closure of the bowel defect using sutures. It highlights the advantages of using sutures instead of the transanal stapler, specifically in cases where the pathology is located higher than 15 cm from the anal verge or the diameter of the nodule is more than 30 mm.

Summary: This study demonstrated the surgical steps and outcomes of a totally laparoscopic segmental rectal resection in a 31-year-old woman with deep endometriosis. The operation was successful with no intraoperative or postoperative complications, and the patient was discharged on the fifth day postoperatively. Standardization of the surgical technique is crucial for ensuring surgical success.

Summary: This retrospective study analyzed all surgical procedures conducted at Cologne Weyertal Academic Hospital from January 2014 to December 2022, including 33 patients with bladder endometriosis. The results showed that 92% of patients experienced improvement in symptoms after surgery, while only 8% reported no change. Surgical treatment is an effective method for improving symptoms of bladder endometriosis, but it requires excellent laparoscopic skills.

Summary: Colorectal surgery for endometriosis exposes patients to a risk of severe complications such as rectovaginal fistula, anastomotic leakage, anastomotic stenosis, and voiding dysfunction. Rectal shaving seems to be less associated with postoperative complications than disc excision and segmental colorectal resection. Disc excision has advantages over segmental colorectal resection in terms of operating time, hospital stay, and risk of postoperative bowel stenosis.

Summary: Endometriosis is a common disease affecting women of fertile age, with deep infiltrating endometriosis of the rectosigmoid colon presenting symptoms mimicking intestinal obstruction or neoplasm. This case report presents a 39-year-old female patient with pelvic pain and anemia during the menstrual cycle, who was found to have ectopic endometrial tissue in the rectum and underwent a surgical procedure.

Summary: Diaphragmatic endometriosis is a rare condition that accounts for a small percentage of all endometriosis cases. This study retrospectively analyzed the patient characteristics and long-term treatment outcomes of 23 diaphragmatic endometriosis patients. The majority of patients had coexisting deep pelvic endometriosis. The study found that treatment led to significant improvement in symptoms, but patients who received postoperative hormonal therapy had higher rates of chest, abdominal, and shoulder pain. Therefore, the study recommends treating symptomatic patients only.

Summary: Cell-free fetal DNA (cffDNA) analysis is a non-invasive prenatal diagnostic test used to screen for chromosomal or monogenic pathologies of the fetus. A narrative review of 45 studies was conducted to critically assess its diagnostic value, and it was found to be well-established for screening purposes. The review also highlighted the need for validation studies and discussions involving all stakeholders to address the legal and ethical complexities associated with these techniques.

The results showed that LSH followed by in-bag transvaginal specimen extraction is a promising technique, which could be considered a reliable and safe option to further reduce the invasiveness of the procedure.

Summary: This study aimed to evaluate the feasibility of an enhanced recovery protocol in a large cohort of patients undergoing emergency surgery and to identify possible factors impacting postoperative protocol compliance. The study found that an enhanced recovery protocol in emergency surgery is feasible and safe. Laparoscopy was associated with an earlier recovery, whereas preoperative hyperglycemia, fluid overload, and abdominal drain were associated with a delayed recovery.

Summary: Embryonal rhabdomyosarcoma (ERMS) is a rare malignancy primarily affecting individuals in the first two decades of life. Botryoid rhabdomyosarcoma, an aggressive subtype of ERMS, commonly presents in the genital tract of female infants and children. Treatment typically involves personalized approaches, combining local debulking surgery with adjuvant or neoadjuvant chemotherapy, while effort is made to preserve fertility and avoid radiation. Radical surgeries and radiation may still be necessary in cases of extensive disease or relapse. Despite its rarity and aggressiveness, ERMS has excellent disease-free survival and overall prognosis, especially when diagnosed early. A multidisciplinary approach is deemed appropriate for optimal management, but larger-scale studies are needed to establish a consensus.

Summary: This practice guideline provides updated evidence for gynecologists who perform endometrial biopsy (EB) and discusses various techniques and considerations for different patient groups. Hysteroscopy is recommended for targeted biopsies, while blind techniques can be used in low-resource settings. The guideline emphasizes the importance of adequate tissue sampling and highlights specific situations where EB is necessary for diagnosis and management. In postmenopausal women, EB is recommended for uterine bleeding, and in women using tamoxifen, hysteroscopic EB should be considered for those with a thickened endometrium.

Summary: The objective of this study was to evaluate whether different surgical approaches in FIGO stage IB1-IB2 cervical cancer led to different patterns of disease recurrence. The results showed that minimally invasive surgery was not associated with different recurrence patterns compared to abdominal radical hysterectomy, and there was no higher risk of distant metastasis or significant difference in terms of disease-free survival (DFS) and overall survival (OS).

Summary: The study investigates the effects of low luteinizing hormone concentrations on oocyte retrieval, fertilization, and embryo development in patients undergoing in vitro fertilization/intracytoplasmic sperm injection. The results suggest that serum luteinizing hormone levels progressively decrease during controlled ovarian stimulation, particularly in the early phase. This decrease may be due to the negative feedback of ovarian hormones from multiple follicular developments or the suppressive effects of gonadotropin-releasing hormone antagonists.

Summary: This case report describes a 60-year-old patient who was referred to our department due to an abdominal mass. A cystic mass measuring 26 cm originating from the right adnexa was detected, and it was surgically removed and diagnosed as an ovarian endometrioma. Although such cases are rare, considering the potential risk of malignant transformation, oophorectomy should be considered as the treatment of choice even in asymptomatic patients.

Summary: This prospective observational study aimed to compare placental oxygenation in low-risk, uncomplicated term pregnancies using near-infrared spectroscopy (NIRS) and ultrasound assessment. The results showed a correlation between placental maturity grade and placental oxygenation. However, due to technical limitations, NIRS cannot be implemented as a routine clinical tool yet.

Summary: In addition to HPV infection, complex molecular disturbances and chronic oxidative stress may contribute to the development of uterine cervical intraepithelial abnormalities and cancer. Reactive oxygen species and free radicals are potentially important factors for CIN and cancer development.
