Full-Thickness Excision versus Shaving by Laparoscopy for Intestinal Deep Infiltrating Endometriosis: Rationale and Potential Treatment Options

Endometriosis is defined as the presence of endometrialmucosa (glands and stroma) abnormally implanted in locations other than the uterine cavity. Deep infiltrating endometriosis (DIE) is considered the most aggressive presentation of the disease, penetrating more than 5mm in affected tissues, and it is reported in approximately 20% of all women with endometriosis. DIE can cause a complete distortion of the pelvic anatomy and it mainly involves uterosacral ligaments, bladder, rectovaginal septum, rectum, and rectosigmoid colon. This review describes the state of the art in laparoscopic approach for DIE with a special interest in intestinal involvement, according to recent literature findings. Our attention has been focused particularly on full-thickness excision versus shaving technique in deep endometriosis intestinal involvement. Particularly, the aim of this paper is clarifying from the clinical andmethodological points of view the best surgical treatment of deep intestinal endometriosis, since there is no standard of care in the literature and in different surgical settings. Indeed, this review tries to suggest when it is advisable to manage the full-thickness excision or the shaving technique, also analyzing perioperative management, main complications, and surgical outcomes.

Summary: Cell-free fetal DNA (cffDNA) analysis is a non-invasive prenatal diagnostic test used to screen for chromosomal or monogenic pathologies of the fetus. A narrative review of 45 studies was conducted to critically assess its diagnostic value, and it was found to be well-established for screening purposes. The review also highlighted the need for validation studies and discussions involving all stakeholders to address the legal and ethical complexities associated with these techniques.

Summary: We evaluated the feasibility of in-bag transvaginal specimen retrieval after laparoscopic subtotal hysterectomy (LSH) through a case series. The results showed that LSH followed by in-bag transvaginal specimen extraction is a promising technique, which could be considered a reliable and safe option to further reduce the invasiveness of the procedure.

Summary: This study aimed to evaluate the feasibility of an enhanced recovery protocol in a large cohort of patients undergoing emergency surgery and to identify possible factors impacting postoperative protocol compliance. The study found that an enhanced recovery protocol in emergency surgery is feasible and safe. Laparoscopy was associated with an earlier recovery, whereas preoperative hyperglycemia, fluid overload, and abdominal drain were associated with a delayed recovery.

Sarcoma Botryoides: Optimal Therapeutic Management and Prognosis of an Unfavorable Malignant Neoplasm of Female Children

Summary: Embryonal rhabdomyosarcoma (ERMS) is a rare malignancy primarily affecting individuals in the first two decades of life. Botryoid rhabdomyosarcoma, an aggressive subtype of ERMS, commonly presents in the genital tract of female infants and children. Treatment typically involves personalized approaches, combining local debulking surgery with adjuvant or neoadjuvant chemotherapy, while effort is made to preserve fertility and avoid radiation. Radical surgeries and radiation may still be necessary in cases of extensive disease or relapse. Despite its rarity and aggressiveness, ERMS has excellent disease-free survival and overall prognosis, especially when diagnosed early. A multidisciplinary approach is deemed appropriate for optimal management, but larger-scale studies are needed to establish a consensus.

Patterns of recurrence in FIGO stage IB1-IB2 cervical cancer: Comparison between minimally invasive and abdominal radical hysterectomy

Summary: The objective of this study was to evaluate whether different surgical approaches in FIGO stage IB1-IB2 cervical cancer led to different patterns of disease recurrence. The results showed that minimally invasive surgery was not associated with different recurrence patterns compared to abdominal radical hysterectomy, and there was no higher risk of distant metastasis or significant difference in terms of disease-free survival (DFS) and overall survival (OS).

Summary: The study investigates the effects of low luteinizing hormone concentrations on oocyte retrieval, fertilization, and embryo development in patients undergoing in vitro fertilization/intracytoplasmic sperm injection. The results suggest that serum luteinizing hormone levels progressively decrease during controlled ovarian stimulation, particularly in the early phase. This decrease may be due to the negative feedback of ovarian hormones from multiple follicular developments or the suppressive effects of gonadotropin-releasing hormone antagonists.

A Large Ovarian Endometrioma Occupying the Abdominal Cavity in a Postmenopausal Patient: A Case Report

Summary: This case report describes a 60-year-old patient who was referred to our department due to an abdominal mass. A cystic mass measuring 26 cm originating from the right adnexa was detected, and it was surgically removed and diagnosed as an ovarian endometrioma. Although such cases are rare, considering the potential risk of malignant transformation, oophorectomy should be considered as the treatment of choice even in asymptomatic patients.
