Surgical management of bisphosphonate-related osteonecrosis of the jaw stages II and III

Objective. The value of surgery in advanced stages of bisphosphonate-related osteonecrosis of the jaw (BRONJ) is still controversial. Hence, we evaluated the effect of surgical therapy in BRONJ stages II and III in combination with a standardized perioperative adjuvant treatment. Study Design. We included 39 patients who presented with BRONJ in a total of 47 locations and stages II (n = 23) and III (n = 24). All patients had exclusively received a monthly intravenous application of zoledronic acid. Surgical therapy consisted of complete removal of the necrotic jaw, accompanied by a standardized perioperative adjuvant treatment including intravenous antibiotic prophylaxis, gastric feeding, and an antimicrobiologic mouth rinsing. Results. Overall, 35 (74.5%) of the 47 BRONJ sites were treated successfully, with success defined as complete mucosal healing of the exposed jaw (n = 24) or as relative healing when surgical therapy downscaled BRONJ II or III to asymptomatic BRONJ stage I (n = 11). Interestingly, perioperative adjuvant treatment or bisphosphonate therapy parameters showed no statistical effect on the treatment outcome. Conclusions. The results of the present study prove the effectiveness of surgical therapy for BRONJ stage II or III.

The value of prebent reconstruction plates and in-house 3D printing

Summary: Prebent reconstruction plates on 3D printed models can improve the accuracy of condylar position after mandibular resection. However, the recent changes in the European Medical Device Regulation affect the use of 3D printing in hospitals, raising the need for discussion on its legitimization. This retrospective observational case-control study found that prefabricated osteosynthesis significantly reduced rotational deviations in the coronal and sagittal planes, with Jewer class-L defects benefiting more.

Comparison of Different Antibiotic Regimes for Preventive Tooth Extractions in Patients with Antiresorptive Intake-A Retrospective Cohort Study

Summary: This study compared the success rates of three different antibiotic regimes in patients undergoing preventive tooth extraction during or after antiresorptive therapy. Retrospective analysis included patients who had tooth extraction from 2009 to 2019 according to specified preventive conditions. Three antibiotic regimens were distinguished: intravenous for 7 days, oral for 14 days, and oral for 7 days. The primary endpoint was the occurrence of medication-related osteonecrosis of the jaw at 12 weeks after surgery. The study found no significant difference in the development of medication-related osteonecrosis among the different antibiotic groups, suggesting that a reduced, oral administration of antibiotics may be sufficient for preventive tooth extraction.

Full block or split block?-Comparison of two different autogenous block grafting techniques for alveolar ridge reconstruction

Both ALT flaps and vastus lateralis flaps are suitable options for large scalp defect closure, with high success rates and short surgery times.

Summary: Carcinomas of the minor salivary glands of the oral cavity are a rare and heterogeneous group of malignant tumors. In this study, we compared clinical and pathological features and oncological outcomes in patients with different kinds of minor salivary gland cancer who received primary surgical therapy. We found different rates of cervical metastases and disease recurrence in dependence on the tumor entity, suggesting the need for different treatment approaches for each tumor entity.

Virtual three-dimensional planning in orthognathic surgery From orthognathic to orthofacial surgery

Summary: Recent decades have witnessed significant advancements in orthognathic surgery techniques and the application of new osteosynthesis methods. Virtual 3D planning has become a routine practice in clinics, offering higher accuracy compared to conventional planning, particularly for patients with asymmetry. Soft tissue simulation has greatly influenced the planning process and improved communication between practitioners and patients. The trend is shifting towards orthofacial surgery, emphasizing facial harmony as well as function. The use of patient-specific implants enables precise implementation of the 3D plan without the need for positioning splints. However, close cooperation between orthodontists and surgeons remains crucial for successful treatment.

Peri-implant health after microvascular head and neck reconstruction-A retrospective analysis
