Frequency of Aminoglycoside-Resistance Genes in Methicillin-Resistant Staphylococcus aureus (MRSA) Isolates from Hospitalized Patients

Background: Staphylococcus aureus is one of the most important causative agents in community-and hospital-acquired infections. Aminoglycosides are powerful bactericidal drugs that are often used in combination with beta-lactams or glycopeptides to treat staphylococcal infections. Objectives: The main objective of the present study was to determine the prevalence of aminoglycoside resistance among methicillin-resistant Staphylococcus aureus (MRSA) isolates in hospitalized patients in Sari and Tehran, Iran. Methods: In this study, 174 MRSA strains isolated from different clinical samples, such as blood, sputum, tracheal exudates, bronchus, pleura, urine, wounds, and catheters, were collected from hospitalized patients in Tehran and Sari during 2014. Antibiotic susceptibility testing was performed against nine antibiotics with the Kirby-Bauer disk diffusion method according to CLSI guidelines. The MRSA strains were examined with oxacillin and cefoxitin disks. MRSA was then validated by detection of the mecA gene. PCR was used to evaluate the prevalence of the aminoglycoside-resistance genes aac (6')-Ie/aph (2 ''), aph (3')-IIIa, and ant (4') among the MRSA isolates. Results: The results of drug susceptibility testing showed that the highest rate of resistance was against erythromycin in Tehran (84.4%) and gentamicin (71.7%) in Sari. All isolates were sensitive to vancomycin, and all strains harbored the mecA gene. The aac (6')-Ie/aph (2 ''), aph (3')-IIIa, and ant (4')-Ia genes were detected among 134 (77%), 119 (68.4%), and 122 (70.1%) of the isolates, respectively. Conclusions: The present study showed a high prevalence of aminoglycoside-resistance genes among MRSA isolates in two cities in Iran.

Mupirocin-Resistant Staphylococcus aureus in Iran: A Biofilm Production and Genetic Characteristics

Summary: The genetic features of high-level mupirocin-resistant S. aureus isolates in Tehran were investigated. The study found that these isolates had multidrug resistance, toxinogenicity, and strong biofilm production ability. Various clonal complexes and sequence types were identified, with CC/ST22 being the most prevalent.

In silico Investigation of Lon Protease as a Promising Therapeutic Target

Summary: Considering the widespread occurrence of antibiotic resistance, the need for new therapeutic strategies is inevitable. This study focuses on the crucial role of Lon protease in the regulation of toxin-antitoxin systems in E. coli and designs inhibitory peptides against the action of this protease. The results show that Lon protease plays a central role in controlling these systems and is a conserved protein.

Antibacterial activity of chitosan-based nanohybrid membranes against drug-resistant bacterial isolates from burn wound infections

Summary: Chitosan, a biocompatible and non-toxic material, shows excellent application prospects in wound dressing conjugate compounds. This study evaluated the antibacterial activity of six different chitosan-based nanohybrid membranes against multidrug-resistant bacterial isolates. The results suggest that chitosan/montmorillonite/ciprofloxacin nanocomposite has the strongest antibacterial effect.

Summary: Understanding the extent of inadequate reprocessing of gastrointestinal (GI) endoscopes is crucial due to severe outbreaks caused by multidrug-resistant microorganisms. This multicenter study in Tehran, Iran, evaluated the microbial contamination of reprocessed GI endoscopes and found a higher-than-expected contamination rate. The results suggest a potential risk of deadly outbreaks associated with endoscopy procedures in Iran.

Prevalence and genetic characteristics of fusidic acid resistant Staphylococcus aureus clinical isolates: Emergence of t030 strains carrying fusB in Tehran, Iran

Summary: This descriptive cross-sectional study provides insights into the molecular characteristics and antimicrobial resistance of fusidic acid resistant Staphylococcus aureus strains isolated from hospitalized patients in Iran. The prevalence of fusidic acid resistance in S. aureus isolates was found to be 15.1%, with fusB being the most predominant acquired gene. The study highlights the importance of understanding the local prevalence of FA-resistant MRSA for effective treatment, control, and reduction of these isolates.

Molecular characterizations of antibiotic resistance, biofilm formation, and virulence determinants of Pseudomonas aeruginosa isolated from burn wound infection

Summary: Burn wounds are often colonized by Pseudomonas aeruginosa, which produces biofilms and exhibits antibiotic resistance. This study identified carbapenem resistance in 40% of the P. aeruginosa isolates, with bla(TEM) being the most common beta-lactamase gene. High resistance was observed to cefotaxime, ceftazidime, meropenem, imipenem, and piperacillin.

Tigecycline-Containing Regimens and Multi Drug-Resistant Acinetobacter baumannii: A Systematic Review and Meta-Analysis

Summary: This systematic review and meta-analysis aimed to explore the safety and efficacy of tigecycline for the treatment of multi drug-resistant Acinetobacter baumannii. The results showed that tigecycline had similar efficacy compared to other antibiotics, but it had a higher mortality rate and lower microbiological eradication rate. Therefore, the use of tigecycline for the treatment of multi drug-resistant A. baumannii is not recommended.

Molecular Characteristics of Staphylococcus aureus Strains to Carry Panton-Valentine Leukocidin Genes Isolated from Hospitalized Patients in Tehran, Iran

Summary: This study investigated the genetic characteristics of PVL-positive S. aureus strains isolated from clinical samples. The emergence of vancomycin-resistant S. aureus strains among PVL-positive S. aureus strains in Iran is a serious alarm and poses the greatest concern in the treatment of staphylococcal infections in the healthcare setting. Direct transfers between community and nosocomial PVL-positive S. aureus types were observed.

Genetic diversity of Staphylococcus aureus isolated from ear infections in Iran: Emergence of CC8/ST239-SCCmec III as major genotype

Summary: The study investigated the antibiotic resistance profile and genetic variability of Staphylococcus aureus isolates from ear infections in adult patients in Tehran, Iran. The results showed high resistance to antibiotics and strong biofilm formation ability in these strains. Different genetic differences were observed in strains from different types of ear infections.

Summary: This study investigated the use of therapeutic ultrasound in the treatment of chronic rhinosinusitis (CRS) patients. The results showed that both continuous and pulsed ultrasound could significantly reduce the quantity of clinically important bacteria in CRS patients. CT scan and questionnaire scores also supported the effectiveness of therapeutic ultrasound in promoting patient recovery.
