Sarcoidosis is a systemic disease, with lung involvement in almost all cases. Abnormal chest radiography is usually a key step for considering diagnosis. Lung impact is investigated through imaging, pulmonary function, and, when required, 6-minute walk test, cardiopulmonary exercise testing, or right heart catheterization. There is usually a reduction of lung volumes, and forced vital capacity is the most accurate parameter to reflect the impact of pulmonary sarcoidosis with or without pulmonary infiltration at imaging. Various evolution patterns have been described. Increased risk of death is associated with advanced pulmonary fibrosis or cor pulmonale, particularly in African-American patients.

Incidence, management, and outcome of lung cancer in patients with long-term oxygen therapy

Summary: This study aimed to assess the characteristics of lung cancer in patients receiving long-term oxygen therapy (LTOT) and compare them with lung cancer patients without LTOT. The results showed that the treatment outcomes and survival rates were similar between the two groups, suggesting that LTOT does not significantly affect lung cancer outcomes.

Summary: This study describes immune-related adverse events (irAE) leading to ICU admissions in solid cancer patients treated with immune checkpoint inhibitors (ICI). The results show that half of the ICU admissions were associated with irAE and could be treated with steroids.

Holistic management of patients with progressive pulmonary fibrosis

Summary: This article summarizes the main needs of patients with progressive pulmonary fibrosis (PF) and their caregivers and proposes a supportive care approach. Personalized care, education, emotional and psychological support, specialized treatments, and better access to information and resources are necessary. Treatment should start at diagnosis and be tailored to the patient's needs, including individualized pharmacological and non-pharmacological interventions such as oxygen therapy and pulmonary rehabilitation.

Summary: This study confirms the diagnostic value of the Sarcoidosis Diagnostic Score (SDS) in differentiating sarcoidosis from alternative diagnoses such as infectious and noninfectious granulomatous diseases. The SDS Clinical and SDS Biopsy showed good to excellent performance in discriminating sarcoidosis from alternative diagnoses.

Summary: In locally advanced non-small-cell lung cancer, mediastinal staging is crucial for therapeutic decision-making. This case report presents a rare case of endobronchial involvement by metastatic lymph node cells after endobronchial ultrasound. The need for careful endoscopic control before surgical resection is highlighted in cases of prior positive endobronchial ultrasound with a time interval.
