Summary: This study investigated the CT characteristics of endogenous endophthalmitis (EE) associated with Klebsiella pneumoniae (K. pneumoniae)-related pyogenic liver abscess (PLA). It found that diabetes mellitus (DM), involvement of the left or both hepatic lobes, and the adjacent vessels on CT were significant predictors of EE. These findings provide valuable information for early diagnosis of EE.

Summary: We report a case of a previously healthy man who presented with a multi-loculated liver abscess caused by a hypervirulent strain of Klebsiella pneumoniae following a minor motor vehicle collision. The patient was successfully treated with percutaneous drainage and antimicrobial therapy, without evidence of metastatic infection. It is important for clinicians to be aware of the nonspecific presentation of Klebsiella pneumoniae liver abscesses and the increasing prevalence of this condition in North America, as delayed diagnosis can lead to increased morbidity and mortality.

Summary: This study analyzed the clinical data of 127 cases of community-acquired Klebsiella pneumoniae liver abscess (CA-KPLA) and constructed a predictive model. The results showed that a SOFA score >2 was the most significant factor affecting pulmonary affection in CA-KPLA patients, followed by a maximum liver abscess diameter >3 cm, multiple liver abscesses, bacteremia, and poorly controlled diabetes. Early drainage of liver abscess, combination antibiotic therapy, and early use of carbapenem antibiotics were found to be effective strategies for the treatment of pulmonary affected CA-KPLA patients.

The majority of cases involve males with diabetes who have traveled from Southeast or East Asia. Extrahepatic involvement and bacteremia are common, with the lungs, ocular structures, and central nervous system being commonly affected. Treatment includes percutaneous drainage and the use of third generation cephalosporins, but the pooled fatality rate remains at 9%.
