Extended criteria donors in liver transplantation Part I: reviewing the impact of determining factors

The definition and factors of extended criteria donors have already been set; however, details of the various opinions still differ in many respects. In this review, we summarize the impact of these factors and their clinical relevance. Elderly livers must not be allocated for hepatitis C virus (HCV) positives, or patients with acute liver failure. In cases of markedly increased serum transaminases, donor hemodynamics is an essential consideration. A prolonged hypotension of the donor does not always lead to an increase in post-transplantation graft loss if post-OLT care is proper. Hypernatremia of less than 160 mEq/L is not an absolute contraindication to accept a liver graft per se. The presence of steatosis is an independent and determinant risk factor for the outcome. The gold standard of the diagnosis is the biopsy. This is recommended in all doubtful cases. The use of HCV+ grafts for HCV+ recipients is comparable in outcome. The leading risk factor for HCV recurrence is the actual RNA positivity of the donor. The presence of a proper anti-HBs level seems to protect from de novo HBV infection. A favourable outcome can be expected if a donation after cardiac death liver is transplanted in a favourable condition, meaning, a warm ischemia time < 30 minutes, cold ischemia time < 8-10 hours, and donor age 50-60 years. The pathway of organ quality assessment is to obtain the most relevant information (e.g. biopsy), consider the co-existing donor risk factors and the reserve capacity of the recipient, and avoid further technical issues.

Summary: A systematic review and meta-analysis revealed that the risk of major complications and mortality after resection of intrahepatic cholangiocarcinoma was assessed. The study found that in-hospital, 30-day, and 90-day mortality rates were 5.9%, 4.6%, and 6.1% respectively, while the proportion of major complications was 22.2%.

Summary: This study evaluated the surgical and oncological outcomes of laparoscopic procedures for suspected gallbladder carcinoma. The data of 129 patients with suspected GBC who underwent laparoscopic radical cholecystectomy before 2020 in Japan were analyzed. The results showed that laparoscopic radical cholecystectomy has favorable outcomes in selected patients with suspected GBC.

A hybrid procedure of living donor liver transplantation for a pediatric patient with citrin deficiency

Summary: This study reports the first case of a pediatric patient undergoing a hybrid procedure for living donor liver transplantation. The surgery was successful and the patient achieved a rapid recovery, with normal liver function and restored secondary sexual characteristics.

Summary: This study compared the efficacy of cyclosporine and tacrolimus after liver transplantation, and found that tacrolimus (T0) had a lower incidence of acute rejection, higher patient and graft survival rates, and similar tolerability and safety compared to cyclosporine (C2). Therefore, tacrolimus might be a better choice after liver transplantation.

Japanese national survey on declined liver allografts from brain-dead donors: High decline rate but promising outcomes in allografts with moderate steatosis

Summary: This study investigated liver allografts from brain-dead donors that were declined and not transplanted due to various marginal factors in Japan. The results showed that pathological abnormalities of steatosis and fibrosis were the most common factors leading to transplant rejection. Although allografts with moderate steatosis were highly declined, they achieved promising outcomes after transplantation.

Summary: This study summarizes the data of gastroenterological surgery in the National Clinical Database (NCD) of Japan from 2011 to 2020. It found that the number of surgeries has been increasing in the past decade, except for a decrease in 2020 due to the COVID-19 pandemic. The average age of surgical patients is steadily increasing, with more than 20% of patients aged 80 years or older. Overall, Japanese gastroenterological surgeries have shown good safety.

The clinical impact of COVID-19 on endoscopic surgery in Japan: Analysis of data from the National Clinical Database

Summary: This study evaluated the impact of COVID-19 on elective endoscopic surgeries in Japan. The number of surgeries, including laparoscopic cholecystectomy, laparoscopic distal gastrectomy, and laparoscopic low anterior resection, decreased in 2020 compared to previous years. Although the use of robot-assisted surgeries increased, the growth rate was lower than in 2019. The infection rate in different prefectures did not significantly affect the surgery numbers. The surgeries decreased from May to June but gradually recovered.

Summary: This study compared outcomes after liver transplantation for perihilar cholangiocarcinoma using strict selection criteria, with or without neoadjuvant chemoradiotherapy. The results showed that patients who received neoadjuvant chemoradiotherapy had lower post-transplant survival rates but lower tumor recurrence rates. Adjustments in neoadjuvant chemoradiotherapy may improve outcomes in these patients. Rating: 7 out of 10.

Combined liver-kidney transplantation with delayed kidney implantation: A case report

Summary: This article presents a rare case of combined liver-kidney transplantation, highlighting the importance of considering the differences in peri-and postoperative care for liver and kidney transplant recipients. The delayed kidney transplantation in this case resulted in a successful recovery for the patient.

Outcomes of living donor liver transplant recipients receiving grafts with the graft-to-recipient weight ratio less than 0.6%: A matched pair analysis

Summary: This study retrospectively analyzed 280 cases of adult living donor liver transplantation (LDLT) and found that a graft-to-recipient weight ratio (GRWR) of less than 0.6% was associated with an increased risk of small-for-size syndrome (SFSS). Although overall graft survival rates were similar, appropriate donor and recipient selection is crucial for successful LDLT with very small grafts.

Summary: Small-for-size syndrome (SFSS) following living donor liver transplantation is a serious complication that may result in poor graft function and graft loss. Portal hyperperfusion of a small graft and hyperdynamic splanchnic circulation are the main causes of this syndrome. The initial treatment focuses on pharmacological therapy and supportive care, while interventional radiology and/or surgery may be considered if medical management fails. Retransplantation should be considered if postoperative laboratory parameters are abnormal.

Summary: This study aimed to measure humoral responses to COVID-19 vaccine among liver patients and determine the factors predicting breakthrough infection. The results showed that liver transplant patients had the lowest response to the vaccine, while reduced response in cirrhosis patients was associated with age, stage of liver disease, and systemic inflammation.

Economic Impact of European Liver and Intestine Transplantation Association (ELITA) Recommendations for Hepatitis B Prophylaxis After Liver Transplantation

Summary: The European Liver and Intestine Transplant Association (ELITA) held a Consensus Conference involving experts worldwide to update guidelines on HBV prophylaxis in liver transplant candidates and recipients. A simulation model was developed to compare the economic impact of implementing the new ELITA guidelines. The model showed that the guidelines could save around euro 235.65 million after 5 years and euro 540.73 million after 10 years. These savings could be used to allocate resources to different needs.
