Summary: This study aimed to report the long-term findings of the Italian programme of cardiovascular preparticipation screening (PPS) in young, competitive athletes. The study assessed the diagnostic yield for diseases at risk of sudden cardiac death (SCD), the costs of serial evaluations, and the long-term outcomes of PPS in a large population of Italian children. Over an 11-year study period, 69 children (0.3%) were identified with cardiovascular diseases at risk of SCD, and one child with normal PPS findings experienced an episode of resuscitated cardiac arrest during sports activity.

Summary: Anomalous aortic origin of right coronary artery (AAORCA) is a congenital heart lesion that can lead to coronary ischemia and sudden death. This study analyzed patients with AAORCA managed at the center, finding that 30% underwent surgical management while 70% were monitored nonsurgically. The outcomes in the midterm were excellent with functional testing showing no evidence of ischemia in the surgical patients.

Summary: Due to advancements in cardiovascular medicine, patients with significant disease burden now have a better prognosis. However, aging-related syndromes like frailty become more common among these patients. Frailty leads to exercise intolerance and compromised adherence to cardiovascular rehabilitation. Recent technological advancements in telemedicine have made it possible to remotely monitor and supervise cardiovascular patients, providing promising benefits. This article aims to provide a framework for developing and implementing a tele-rehabilitation program for managing prevention and rehabilitation in cardiovascular disease patients with frailty.

Summary: This study aimed to evaluate the tolerability, clinical outcomes, and changes in NT-proBNP levels and glomerular filtration rate (GFR) in transthyretin amyloid cardiomyopathy (ATTR-CM) patients treated with dapagliflozin. After 3 months of follow-up, a numerical decrease in NT-proBNP levels was observed in some patients in the dapagliflozin treatment group, while other disease parameters remained stable and no adverse events occurred.

Summary: In this case report, a 58-year-old male patient with cardiomyopathy and palmoplantar keratosis is described. Genetic testing revealed a novel variant in the desmoplakin gene, suggesting the presence of Cardiocutaneous syndrome. Repeated genetic testing is clinically significant, especially in patients with a high probability of a specific inherited cardiac disease, to improve diagnostic accuracy.

Summary: Myocardial bridging (MB) is a segment of coronary arteries that is usually free from atherosclerosis, while the adjacent proximal segment is prone to it due to local endothelial shear stress (ESS). This study used coronary computed tomography angiography and computational fluid dynamics to analyze the ESS in coronary arteries with MBs and investigate the association between atherosclerosis and hemorheological characteristics. The results showed that there was no significant difference in ESS between the proximal 5 mm to MBs and the region within MBs. No significant ESS variances were observed around the MB entrance in arteries with plaques, regardless of the morphological features of the bridged segment. MBs have homogeneous, atheroprotective ESS, which may explain the absence of atherosclerosis within bridged segments. The interplay between ESS and atherosclerosis is potentially similar in arteries with MB compared to those without bridges.
