Longitudinal study of patients with chronic Chagas cardiomyopathy in Brazil (SaMi-Trop project): a cohort profile

Purpose We have established a prospective cohort of 1959 patients with chronic Chagas cardiomyopathy to evaluate if a clinical prediction rule based on ECG, brain natriuretic peptide (BNP) levels, and other biomarkers can be useful in clinical practice. This paper outlines the study and baseline characteristics of the participants. Participants The study is being conducted in 21 municipalities of the northern part of Minas Gerais State in Brazil, and includes a follow-up of 2years. The baseline evaluation included collection of sociodemographic information, social determinants of health, health-related behaviours, comorbidities, medicines in use, history of previous treatment for Chagas disease, functional class, quality of life, blood sample collection, and ECG. Patients were mostly female, aged 50-74years, with low family income and educational level, with known Chagas disease for >10years; 46% presented with functional class >II. Previous use of benznidazole was reported by 25.2% and permanent use of pacemaker by 6.2%. Almost half of the patients presented with high blood cholesterol and hypertension, and one-third of them had diabetes mellitus. N-terminal of the prohormone BNP (NT-ProBNP) level was >300pg/mL in 30% of the sample. Findings to date Clinical and laboratory markers predictive of severe and progressive Chagas disease were identified as high NT-ProBNP levels, as well as symptoms of advanced heart failure. These results confirm the important residual morbidity of Chagas disease in the remote areas, thus supporting political decisions that should prioritise in addition to epidemiological surveillance the medical treatment of chronic Chagas cardiomyopathy in the coming years. The SAo Paulo-Minas Gerais Tropical Medicine Research Center (SaMi-Trop) represents a major challenge for focused research in neglected diseases, with knowledge that can be applied in primary healthcare. Future plans We will continue following this patients' cohort to provide relevant information about the development and progression of Chagas disease in remotes areas, with social and economic inequalities. The results showed that pregnant women can serve as a suitable population for SARS-CoV-2 serosurveillance. The study found a high seroprevalence among women at delivery in the early period of the pandemic, indicating their exposure to the virus.

Summary: The prevalence of leg ulcers in this study was 21% among adult participants. Factors associated with leg ulcers included increasing age, male sex, chronic transfusion therapy, higher indirect bilirubin levels, and low hemoglobin levels. Participants who self-reported as Black, mixed, and other/unknown were more likely to have leg ulcers compared to those who self-reported as White.

Summary: This study used the gPhage platform to compare antigens and antibodies before and after treatment in 20 patients with chronic Chagas disease. The results showed a decrease in antibody levels and recognition of specific antigens after treatment. The gPhage data correlated with patient PCR-status and could predict patient response to treatment. In conclusion, the use of the gPhage platform contributes to the development of novel diagnostic tools for Chagas disease.

Estimated glomerular filtration rate in Brazilian adults with sickle cell disease: results from the REDS-III multicenter cohort study

Summary: Chronic kidney disease significantly affects the morbidity and mortality of sickle cell disease. This study aimed to evaluate the prevalence and risk factors for reduced estimated glomerular filtration rate (eGFR) among adults with sickle cell disease in Brazil. The results showed that one-quarter of the adults had eGFR below 90, and older age, male sex, higher diastolic blood pressure, lower hemoglobin, and lower reticulocyte levels were associated with decreased eGFR.

Summary: In Brazil, the National Immunization Program includes PCV10 for children < 2 years old and PPV23 for high-risk patients >= 2 years and institutionalized >= 60 years. PCV13 is only used in private clinics. A retrospective study on IPD patients in 5 hospitals in Brazil revealed that high-risk comorbidities were more common than at-risk conditions. This study suggests the need for expanded valence vaccines to provide broad prevention of IPD.

Summary: This study investigates the development of electrocardiographic abnormalities in elderly Chagas disease patients and its impact on their survival. The study found that Chagas disease is associated with a higher risk of developing new ECG abnormalities, which in turn increases the risk of death. The newly developed cardiomyopathy in the elderly is as serious as in earlier stages.
