Evaluation of use and cost of medical care of common lumbar disorders in Korea: cross-sectional study of Korean Health Insurance Review and Assessment Service National Patient Sample data

Objectives: To assess medical care and costs of the 3 highest prevalence lumbar disorders-non-specific low back pain (nLBP), intervertebral disc disorder (IDD) and spinal stenosis (SS)-from national billing data to provide basic information for standards of appropriate management. Design: Retrospective analysis of National Health Insurance National Patient Sample data provided by the Korean Health Insurance Review and Assessment Service (HIRA). Setting: 2011 claims data from all medical institutions which filed billing statements to HIRA. Participants: A total of 135 561 patients with lumbar disorder who received medical services during 2011. Outcome measures: Patient characteristics, medical procedures, medication, cost, injection and surgery. Results: In the nLBP and IDD groups, the 50-59 age range had the highest prevalence, whereas prevalence increased with age in SS. All 3 groups showed a higher percentage in women. The average treatment cost was 196 552 KRW in the nLBP and 362 050 KRW in the IDD group, and highest in the SS group at 439 025 KRW. While in the nLBP group women spent more on medical expenses, in the other 2 groups men showed higher expenditure. Expenditure grew with age in the nLBP and SS groups, whereas that of the IDD group peaked in their 40s. Analgesics were used in 73.43% of patients with nLBP, 82.64% of patients with IDD and 86.46% of patients with SS, and opioids in 4.12% of patients with IDD and 5.36% of patients with SS. Surgery rates were highest in the SS group at 4.85%, with 0.9% for nLBP and 4.59% for IDD. The most frequent injection code was lumbar/caudal epidural nerve block. Expenditure and surgery rates were higher in the injection than in the non-injection subgroup in all 3 groups. Conclusions: Patterns of medical care of most frequent lumbar disorders from HIRA data showed significant difference between groups and provide a basic standard for future usual care guidelines linked with health policy and budget appropriation.

Complex Extract of Polygonatum sibiricum and Nelumbinis semen Improves Menopause Symptoms via Regulation of Estrogen Receptor Beta in an Ovariectomized Rat Model

Summary: Menopause is a hormone-deficiency state that causes various symptoms such as facial flushing, vaginal dryness, and depression. Hormone-replacement therapy is commonly used, but it has side effects. In this study, a complex extract of Polygonatum sibiricum and Nelumbinis semen was found to improve menopause symptoms without side effects by increasing the expression of ER beta and restoring vaginal epithelial cell thickness. It also improved blood-lipid metabolism and alleviated osteoporosis. Therefore, it could be a natural treatment option for menopause.

Effectiveness of Pharmacopuncture Therapy in Adhesive Capsulitis: a Study Protocol for a Pragmatic Randomized Controlled Trial

Summary: This study aims to evaluate the effectiveness and safety of pharmacopuncture therapy compared to physiotherapy for treating adhesive capsulitis. The study will be conducted as a randomized controlled trial with 50 participants receiving either pharmacopuncture therapy or physiotherapy over a 6-week period. The primary outcome measure is the numeric rating scale for shoulder pain, and secondary outcomes include various other measures of pain and disability. Analysis will be based on the intention-to-treat principle.

Healthcare utilization for atopic dermatitis: An analysis of the 2010-2018 health insurance review and assessment service national patient sample data

Summary: This study analyzed the distribution and healthcare usage patterns of patients with atopic dermatitis using the 2010-2018 Health Insurance Review and Assessment Service data. The prevalence of atopic dermatitis showed a decreasing trend over time among patients aged <5, 5-14, and 15-24 years. The most commonly used Western medicine treatments were injections and oral medications involving topical corticosteroids, antihistamine agents, and oral steroids, while acupuncture therapy was the most used Korean medicine treatment.

Risk of Lymphedema and Death after Lymph Node Dissection with Neoadjuvant and Adjuvant Treatments in Patients with Breast Cancer: An Eight-Year Nationwide Cohort Study

Summary: Limited knowledge exists on the impact of neoadjuvant and adjuvant treatments on post-surgery lymphedema (LE) in breast cancer patients. A study using a national cohort from the Korean National Health Insurance Service database found that while minimal lymph node dissection reduced LE risk, neoadjuvant chemotherapy followed by lymph node dissection did not. Adjusting for adjuvant chemotherapy decreased LE risk in the lymph node dissection group, but not mortality risk. Longer duration of neoadjuvant chemotherapy, especially taxane-based, combined with lymph node dissection increased LE risk. This highlights the importance of early and long-term surveillance to reduce LE risk.

Analysis of Conservative Treatment Trends for Lumbar Disc Herniation with Radiculopathy in Korea: A Population-Based Cross-Sectional Study

Summary: This study aimed to analyze the trends and associated medical costs of conservative treatment for lumbar intervertebral disc disorders with radiculopathy in Korea. The number of patients receiving conservative treatment increased by more than 30% over a 10-year period, and medical costs also increased. Non-surgical treatments mainly included medication and physiotherapy, with non-opioid analgesics being the most commonly prescribed medication.

RoBANS 2: A Revised Risk of Bias Assessment Tool for Nonrandomized Studies of Interventions

Summary: Assessment of the risk of bias is essential for systematic reviews. The Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS) is a widely used tool for assessing bias in nonrandomized studies. The revised version, RoBANS 2, has shown acceptable reliability and validity, providing a comprehensive framework for evaluating bias in nonrandomized studies of interventions.

Trends of Surgical Service Utilization for Lumbar Spinal Stenosis in South Korea: A 10-Year (2010-2019) Cross-Sectional Analysis of the Health Insurance Review and Assessment Service-National Patient Sample Data

Summary: This study analyzed the trend of surgical service utilization in patients with lumbar spinal stenosis (LSS) in Korea from 2010 to 2019. The findings showed an increase in the number of claims and medical expenses for surgeries, with decompression accounting for a higher proportion of first surgeries and fusion being more common in second surgeries. Patients who underwent fusion procedures had longer hospital stays and higher medical expenses across all sex and age groups.
