Changes in lung volumes and gas trapping in patients with large hiatal hernia

Background and AimsStudies assessing hiatal hernia (HH)-related effects on lung volumes derived by body plethysmography are limited. We aimed to evaluate the effect of hernia size on lung volumes (including assessment by body plethysmography) and the relationship to functional capacity, as well as the impact of corrective surgery. MethodsSeventy-three patients (7010 years; 54 female) with large HH [meanstandard deviation, intra-thoracic stomach (ITS) (%): 63 +/- 20%; type III in 65/73] had respiratory function data (spirometry, 73/73; body plethysmography, 64/73; diffusing capacity, 71/73) and underwent HH surgery. Respiratory function was analysed in relation to hernia size (groups I, II and III: 50, 50%-75% and 75% ITS, respectively) and functional capacity. Post-operative changes were quantified in a subgroup. ResultsTotal lung capacity (TLC) and vital capacity (VC) correlated inversely with hernia size (TLC: 97 +/- 11%, 96 +/- 13%, 88 +/- 10% predicted in groups I, II and III, respectively, P=0.01; VC: 110 +/- 17%, 111 +/- 14%, 98 +/- 14% predicted, P=0.02); however, mean values were normal and only 14% had abnormal lung volumes. Surgery increased TLC (93 +/- 11% vs 97 +/- 10% predicted) and VC (105 +/- 15% vs 116 +/- 18%), and decreased residual volume/total lung capacity (RV/TLC) ratio (39 +/- 7% vs 37 +/- 6%) (P<0.01 for all). Respiratory changes were modest relative to the marked functional class improvement. Among parameters that improved following HH surgery, decreased TLC and forced expiratory volume in 1s and increased RV/TLC ratio correlated with poorer functional class pre-operatively. ConclusionsIncreasing HH size correlates with reduced TLC and VC. Surgery improves lung volumes and gas trapping; however, the changes are mild and within the normal range.

Summary: Platelets are capable of rapidly releasing IL-1 beta as an inflammatory mediator in response to specific agonists, independent of NLRP3 and caspase-1. This rapid release occurs in minutes following exposure to ADP and thrombin receptor agonists. Immature and mature IL-1 beta proteins were found in low abundance within human platelets and localized to various parts of the platelet structure.

Comparison of P2Y12 Inhibitors in Acute Coronary Syndromes in the Australian Population

Summary: This study aimed to explore the patterns of P2Y12 inhibitor use and their association with outcomes in the Australian population. The results showed that in Australia, ticagrelor and prasugrel were mainly used in younger patients, while clopidogrel was mainly used in patients with higher baseline risks. The ticagrelor and prasugrel groups had reduced in-hospital deaths, major adverse cardiac events, and bleeding compared to the clopidogrel group, but these differences were no longer apparent after multivariable adjustment. There were no differences in outcomes between the treatment groups at 12 months, but the findings suggest room for improvement in guideline-driven usage of P2Y12 inhibitors.

Achieving lipid targets within 12 months of an acute coronary syndrome: an observational analysis

Summary: This study assessed lipid levels in ACS patients six or 12 months after discharge and found that almost half of the patients did not achieve target lipid levels. Factors associated with not reaching targets included age, use of lipid-lowering therapy, and cholesterol levels on admission. These patients are at increased risk of recurrent cardiovascular disease, highlighting the importance of optimizing therapy to improve outcomes.

Perspective: Collagen induced platelet activation via the GPVI receptor as a primary target of colchicine in cardiovascular disease

Summary: Colchicine has been proven to reduce cardiovascular death, myocardial infarction, ischemic stroke, and coronary revascularization in patients with coronary artery disease (CAD), even in those already on antiplatelet therapy. It not only has anti-inflammatory effects, but also exerts antiplatelet effects by primarily targeting collagen-induced platelet activation via glycoprotein (GP)VI, a critical receptor for arterial thrombosis formation. In vitro studies have shown that therapeutic concentrations of colchicine decrease collagen-induced platelet aggregation and modify GPVI signaling. Clinical studies have demonstrated a significant reduction in serum GPVI levels after 6 months of colchicine treatment in CAD patients, potentially reducing thrombotic risk. Future evaluation of colchicine's effects should include assessment of its impact on collagen-mediated platelet activation, in addition to its well-known anti-inflammatory effects.

Safety and efficacy of bronchial thermoplasty in Australia 5 years post-procedure

Summary: In a cohort of severe asthmatics in the era of biological therapy, the safety and efficacy of bronchial thermoplasty (BT) was evaluated 5 years post-treatment. The results showed sustained improvement in symptoms control and reduced steroid requirement, but no change in spirometry. Mild localized radiological bronchiectasis was identified in some patients without clinical features.

Effects of pre-eclampsia on HDL-mediated cholesterol effiux capacity after pregnancy
