Summary: Iron metabolism disorder is closely related to acute-on-chronic liver failure (ACLF). This study aimed to analyze the levels of soluble transferrin receptor (sTfR) in hepatitis B virus (HBV)-related ACLF and evaluate its predictive value for short-term prognosis. The results showed that decreased sTfR levels were independently associated with 30-day mortality in HBV-ACLF patients, suggesting its potential as a predictor.

Summary: This study aimed to determine the methylation status of T beta 4 promoter in patients with acute-on-chronic hepatitis B liver failure (ACHBLF) and evaluate its prognostic value. The results showed that the methylation frequency of T beta 4 was significantly higher in ACHBLF patients compared to pre-ACHBLF, chronic hepatitis B (CHB), or healthy controls. Additionally, T beta 4 promoter methylation status was negatively correlated with mRNA levels. The 3-month mortality rate was significantly higher in the methylated group, and T beta 4 methylation status had better predictive value for ACHBLF incidence and mortality compared to the model for end-stage liver disease (MELD) score.

Summary: This study found that the diversity and abundance of the gut microbiome significantly decreased as hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) progressed, with potentially pathogenic bacteria showing increased abundance in HBV-ACLF patients compared to healthy controls. The abundance of Bacteroidetes was negatively correlated with serum alpha fetoprotein, while the abundance of Veilonella was positively correlated with serum total bilirubin levels. These findings suggest that the gut microbiota plays an important role in the development of HBV-ACLF.
