Radiomic Texture Analysis Mapping Predicts Areas of True Functional MRI Activity

Individual analysis of functional Magnetic Resonance Imaging (fMRI) scans requires user-adjustment of the statistical threshold in order to maximize true functional activity and eliminate false positives. In this study, we propose a novel technique that uses radiomic texture analysis (TA) features associated with heterogeneity to predict areas of true functional activity. Scans of 15 right-handed healthy volunteers were analyzed using SPM8. The resulting functional maps were thresholded to optimize visualization of language areas, resulting in 116 regions of interests (ROIs). A board-certified neuroradiologist classified different ROIs into Expected (E) and Non-Expected (NE) based on their anatomical locations. TA was performed using the mean Echo-Planner Imaging (EPI) volume, and 20 rotation-invariant texture features were obtained for each ROI. Using forward stepwise logistic regression, we built a predictive model that discriminated between E and NE areas of functional activity, with a cross-validation AUC and success rate of 79.84% and 80.19% respectively (specificity/sensitivity of 78.34%/82.61%). This study found that radiomic TA of fMRI scans may allow for determination of areas of true functional activity, and thus eliminate clinician bias.

Summary: The current treatment strategy for pilonidal sinus disease (PSD) in Germany mainly consists of primary excision and open wound healing. However, the use of flap procedures and off-midline techniques has increased over the past 20 years. Despite this increase, minimally invasive approaches are still underutilized. Compliance with national guidelines for PSD treatment remains poor.

Outcome in patients with open abdomen treatment for peritonitis: a multidomain approach outperforms single domain predictions

Summary: This study aimed to predict survival outcomes in peritonitis patients treated with open abdomen surgery using a multidomain prediction model. The results indicated that the surgical predictor skin closure, along with the physiological predictors SAPS-II and MPI, were key factors in predicting survival. The integrative prediction model outperformed individual predictors in predicting survival outcomes.

Treatment for Brain Metastases: ASCO-SNO-ASTRO Guideline

Summary: The purpose of this study is to provide guidelines for clinicians on treating patients with brain metastases from solid tumors. The study conducted a systematic review of the literature and found that surgery is a reasonable option for patients with brain metastases. Local therapy should be given to patients with symptomatic brain metastases, regardless of the systemic therapy used. For patients with asymptomatic brain metastases, local therapy should not be deferred unless specifically recommended. Several regimens were recommended for different types of cancer. For patients with no systemic therapy options, stereotactic radiosurgery (SRS) alone should be offered. Memantine and hippocampal avoidance should be considered for patients receiving whole brain radiation therapy. Patients with low Karnofsky Performance Status and no systemic therapy options do not benefit from radiation therapy.

Long term outcomes following surgery for pineal region tumors

Summary: This is the first modern surgical series providing long term follow-up for patients undergoing surgical resection of pineal region tumors. Obtaining a gross total resection of these challenging tumors is beneficial for improving progression free survival and overall survival. Higher grade tumors have decreased survival rates and are often treated with adjuvant chemotherapy and/or radiotherapy.

Summary: Postcraniotomy headache (PCH) is a common and underappreciated adverse event. The use of opioids for pain relief may interfere with neurologic evaluation and should be minimized. Special attention should be given to pain control for both acute and chronic PCH, particularly in neurosurgical settings.

Summary: Endothelial progenitor cells (EPC) have the potential to influence the integrity and stability of the vascular endothelium, but their role and applicability as biomarkers in clinical settings are still uncertain. Evaluating endothelial dysfunction remains a major challenge, largely due to the heterogeneity of this cell population. To use EPC levels as biomarkers for vascular function and cardiovascular risk, defining their phenotype accurately and updating current isolation methods are urgent priorities.

Summary: The study aimed to investigate the effectiveness of C-clamp application in patients with unstable pelvic fractures and hemodynamic instability. The results showed that early application of C-clamp can effectively stabilize hemodynamics and reduce volume substitution. This finding is important for severely injured patients.
