Predictive value of the Groningen Frailty Indicator for treatment outcomes in elderly patients after head and neck, or skin cancer surgery in a retrospective cohort

Objectives Careful selection of patients eligible for extensive head and neck cancer surgery is extremely important. A reliable predictor for postoperative outcomes in the vulnerable elderly population is not yet available. The concept of frailty describes a clinical state of increased vulnerability and can be assessed using frailty tests, such as the Groningen Frailty Indicator. In the current study, the influence of Groningen Frailty Indicator-measured frailty on clinical outcome was investigated in elderly patients surgically treated for head and neck cancer. Design Retrospective, explorative cohort study. Setting Tertiary referral centre. Participants A total of 90 patients of 65years and older receiving surgical treatment for head and neck cancer with different primary sites. Main outcome measures The influence of frailty (Groningen Frailty Indicator) on postoperative complications (Clavien-Dindo classification), subjective postoperative experience of both patient and surgeon and survival were analysed. Results A total of 36 patients were considered as frail (40%). Postoperative complications could not be predicted by frailty status. However, the Groningen Frailty Indicator dimension health problems' was a significant predictor for postoperative complications (P=0.020). Unlike age and comorbidity, frailty was associated with a poor subjective patients' experience of the postoperative recovery (P<0.01). Although not statistically significant, survival analysis showed a worse 5-year overall survival in the frail group (33%) versus the non-frail group (74%). Conclusions Analysis of frailty could identify elderly patients who might suffer more than expected during the postoperative period after head and neck cancer surgery. In this study, frailty was not identified as a new predictor of complications after head and neck cancer surgery.

SCC Antigen Concentrations in Fine-Needle Aspiration Samples to Detect Cervical Lymph Node Metastases: A Prospective Analysis

Summary: The diagnostic value of measuring SCC-Ag and CA15-3 concentrations in FNA samples for the detection of SCC metastases in cervical lymph nodes was evaluated. SCC-Ag measurement showed comparable sensitivity to FNAC and could be used as a cost-effective screening tool in samples with nondiagnostic or indeterminate FNAC results or when multiple lymph nodes are sampled. However, SCC-Ag in FNA samples had lower specificity compared to FNAC assessed by experienced pathologists in head and neck oncology. The addition of CA15-3 measurement did not enhance accuracy.

Influence of Frailty and Life Expectancy on Guideline Adherence and Outcomes in Cutaneous Squamous Cell Carcinoma of the Head and Neck: A Prospective Pilot Study

Summary: This study evaluated the impact of frailty and life expectancy on treatment decision, outcomes, and quality of life in older patients with cutaneous squamous cell carcinoma. The results showed that deviating from guidelines towards less aggressive treatment did not negatively affect short-term outcomes or quality of life in frail patients with a limited life expectancy. Further studies with larger sample sizes and longer follow-up periods are needed to confirm these results.

3D assessment of initial fracture displacement of tibial plateau fractures is predictive for risk on conversion to total knee arthroplasty at long-term follow-up

Summary: A novel 3D measurement method was developed to assess fracture displacement and its association with the risk of conversion to total knee arthroplasty (TKA). The study found that an initial 3D gap area ≥550 mm² was independently associated with conversion to TKA. Different prognostic groups based on the range of 3D gap area were identified, and the survival rate of the knee without conversion to TKA was assessed through Kaplan-Meier curves.

Narrow-Band Imaging in Transoral Laser Surgery for Early Glottic Cancer: A Randomized Controlled Trial

Summary: This study aimed to assess whether the additional use of narrow-band imaging (NBI) in transoral laser surgery (TOLS) for early laryngeal cancer improves clinical outcomes. The results showed that the use of NBI significantly reduced the number of positive resection margins and potentially decreased the recurrence rate and improved recurrence-free survival in patients treated with NBI-supported TOLS. Further studies with larger patient groups are needed to confirm these findings.

Radiologically Defined Sarcopenia as a Biomarker for Frailty and Malnutrition in Head and Neck Skin Cancer Patients

Summary: The aim of this study was to assess the utility of radiologically defined sarcopenia, or a low skeletal muscle index (SMI), as a biomarker for frailty and postoperative complications (POC) in patients with head and neck skin cancer (HNSC). The study used retrospectively collected data and found that low SMIs were related to frailty and malnutrition. However, only frailty based on Geriatric 8 (G8) score was related to POC. The study concluded that low SMI is a practical biomarker for frailty and malnutrition in HNSC, and further research should focus on interventions based on low SMI scores.

Summary: This study aimed to assess potential prognostic markers for rare sinonasal intestinal-type adenocarcinoma. The results showed that there were 21 biomarkers associated with a significant negative effect on survival, but none had a positive effect. The findings can help specialists better understand the risks and prognosis of this disease, leading to improved treatment and patient outcomes.

EGFR-targeted fluorescence molecular imaging for intraoperative margin assessment in oral cancer patients: a phase II trial

Summary: Using tumor-specific fluorescent tracers, fluorescence molecular imaging (FMI) can accurately visualize tumor tissues. This study evaluated the diagnostic accuracy of an EGFR-targeted FMI for intraoperative margin assessment in oral squamous cell carcinoma patients. The results showed that FMI with cetuximab-800CW had high sensitivity and specificity in detecting tumor-positive margins, providing a safe method for intraoperative adjustment of surgical margins.

Association of Tumor Microenvironment with Biological and Chronological Age in Head and Neck Cancer

Summary: There is often a discrepancy between the chronological and biological age of head and neck cancer patients. Treatment is based on chronological age, but biological age may be a more accurate predictor of treatment tolerability. This study examined the association between tumor characteristics and chronological and biological age, as well as their relation to survival. The findings suggest that biological age may have a stronger impact on the tumor microenvironment than chronological age, highlighting the importance of investigating treatment response based on biological age.
